Spontaneous regeneration of the mandible following hemimandibulectomy for medication-related osteonecrosis of the jaw: A case report.

RATIONALE: Medication-related osteonecrosis of the jaw decreases quality of life of patients with cancer. The debate about it continues regarding the risk factors, etiology, and treatment methods, and so on. Also, spontaneous regeneration of the mandible is clinically rare. PATIENT CONCERNS: A 67-year-old woman presented to the authors' department complaining of pain, swelling, and pus discharge from a fistula. She previously had breast cancer bone metastases and had received antiresorptive intravenous bisphosphonate. DIAGNOSIS: The patient was diagnosed with medication-related osteonecrosis

Clear cell odontogenic carcinoma of the mandible: a treatment strategy.

Clear cell odontogenic carcinoma (CCOC) is a rare odontogenic tumor of the jaws, histologically characterized by the presence of agglomerate … tal resection of the mandible with a safety margin. After six months without recurrence, definitive mandibular reconstruction was performed using an iliac crest graft, followed by rehabilitation with implant-supported denture after five months. After three years of post-resection follow-up, the patient has shown no evidence of recurrence or metastasis. She continues to be …

Bilobed skin paddle fibula flap for large oromandibular defects.

Although the fibula free flap (FFF) is a useful choice for mandible reconstruction, its application for large oromandibular defects is still debated upon. We report the use of FFF with a bilobed perforator-based skin paddle for combined hemiglossectomy, floor-of-the-mouth, tonsillar pillar, and mandibular body defect. A case of an 84-year-old woman with a 2.5 × 3.5-cm exophytic ulcerated mass on the right side of the tongue, extended to omolateral gengival fornix, tonsillar pillar, and mandibular body, is reported. An osteocutaneus FFF with a 7-cm bone strut and a 17 × 10-cm bilobed shaped ski

Computer-Assisted CBCT Evaluation of Inferior Alveolar Nerve Canal Regeneration One Year Following Nerve Transposition.

Background: Rehabilitation of the severely atrophic posterior mandible remains surgically challenging, and inferior alveolar nerve (IAN) repositioning is a well-established technique that enables implant placement in anatomically compromised cases. Although neurosensory outcomes following nerve relocation have been extensively investigated, the regenerative capacity of the mandibular canal itself has not been previously evaluated. This study presents the first computer-assisted, cone-beam computed tomography (CBCT)-based assessment of bony canal regeneration after IAN transposition. Methods: T

High-resolution US of the facial vessels with new facial vein landmarks for reconstructive surgery and dermal injection.

BACKGROUND: Accurate knowledge of vessel anatomy is essential in facial reconstructive surgery. The technological advances of ultrasound (US) equipment with the introduction of new high-resolution probes improved the evaluation of facial anatomical structures. Our study had these objectives: the primary objective was to identify new surgical landmarks for the facial vein and to verify their precision with US, the secondary objective was to evaluate the potential of high-resolution US examination in the study of both the facial artery and vein. METHODS: Two radiologists examined a prospective

Do patients treated with bimaxillary surgery have more stable condylar positions than those who have undergone single-jaw surgery?

PURPOSE: Because condylar positioning after sagittal split ramus osteotomy of the mandible has been known to affect postoperative skeletal stability, accurate positional assessment of the temporomandibular joint after orthognathic surgery is vital to maximize stability of the surgery. The purpose of this study was to evaluate condylar changes after single-jaw and double-jaw surgeries in mandibular prognathism patients by comparing 3-dimensional angular and positional changes of the condylar heads in groups of patients receiving combined maxillary posterior impaction and mandibular setback and

[Vascularised Fibula and Tendon Transfer in the Comprehensive Treatment of Forearm Fracture with Gas Gangrene Complication].

Vascularized bone grafting was for the first time described in the 1970s. It has become a crucial component in the reconstruction of long bone defects and non-unions in a poorly vascularised environment. Although the vascularized bone is well described for the lower extremity and mandibular reconstructions, it started to be used for the upper extremity bone defects only recently. This publication presents a case of a young man with an open fracture of both the forearm bones. The treatment of the injury has been complicated with anaerobic gas gangrene of the forearm. This resulted in a non-unio…

Reconstruction of lateral lower lip defects with transverse lip advancement flap.

BACKGROUND: Lower lip defects between one and two thirds of the lower lip are usually reconstructed with neighboring tissues from the upper lip, cheeks, or the mentum or a combination of these tissues. In this article, we offer a simple and effective reconstruction option that can be used for lateral defects between one third and two thirds of lower lip tissue. Utilizing the tremendous flexibility and expandability of the lip tissue, we tried to minimize the incisions and scar, and maximize the sensation and function with transverse advancement flap. PATIENTS AND METHODS: Ten patients with la

Quantitative microanatomy of jaw muscle attachment in extant diapsids.

Muscular reconstructions in vertebrate paleontology have often relied heavily on the presence of "muscle scars" and similar osteological correlates of muscle attachment, a practice complicated by the fact that approximately half of tendinous muscle attachments to bone in extant vertebrates do not leave readily interpretable scars. Microanatomical and histological correlates of tendinous muscle attachment are much less ambiguous. This study examines the microanatomical correlates of muscle attachment for the mandibular adductors in six species of diapsids. Most prominent tendinous or aponeuroti

Retrospective analysis of external pin fixation of mandibular fractures: A 25-year single institution experience.

External fixation of mandible fractures is an infrequently utilized fixation treatment modality in comparison to open reduction internal fixation (ORIF) or closed reduction techniques. However, external fixation still provides a necessary modality in the management of select mandibular fractures. Therefore, we aim to draw trends, outcomes, and treatment timelines through a retrospective analysis to better guide, advise and predict realistic treatment outcomes in the future for our patients. This abstract shows trends in indications, complications, airway management and timeline to definitive r

Combined bilobed trapezius myocutaneous and scapula osteomyocutaneous flaps for the restoration of through-and-through defects of the mandible following salvage surgery for recurrent head and neck tumours.

This study was performed to evaluate the outcomes of combined bilobed trapezius myocutaneous and scapula osteomyocutaneous flaps for the restoration of through-and-through defects of the mandible. The subjects were six patients with recurrent malignant tumours who underwent salvage surgeries and reconstruction with combined flaps based on the transverse cervical vessels (TCVs). All patients had combined bone (four type H, one type LCL, and one type C) and extensive soft tissue defects (through-and-through defects) following salvage surgery. All tumours were removed and the complex defects rest

Conservative treatment and implant rehabilitation of the mandible in a case of craniofacial fibrous dysplasia: a case report.

We present a case of implant rehabilitation of the mandible in a young patient affected by craniofacial fibrous dysplasia. A computed tomography scan showed involvement of the left orbital contour and left maxilla, with total involvement of the mandible. We performed conservative surgical treatment from 2004 to 2011 to reconstruct and recontour the orbital and mandible region. Next, we performed implant rehabilitation of the mandible. This case emphasizes the importance of conservative and nondemolition treatment to obtain excellent functional and esthetic results.

Concurrent multiple odontogenic keratocysts and odontogenic myxoma in a patient with nevoid basal cell carcinoma: a case report and literature review.

Odontogenic myxoma and odontogenic keratocyst (OKC) rarely coexist, especially in association with Nevoid Basal Cell Carcinoma Syndrome (NBCCS). This report presents an unusual case of an NBCCS patient with simultaneous myxoma and OKC in the mandible. We introduce an innovative management strategy incorporating 5-fluorouracil (5-FU) as an adjunct treatment for OKC, paired with immediate mandibular reconstruction using a fibula-free flap, following the Jaw-In-A-Day (JIAD) protocol. The report details the clinical presentation, surgical approach, and follow-up, highlighting the potential benefit…

A benign tumor of substantial size: Mandibular epithelioid osteoblastoma in a socioeconomically challenged patient.

Epithelioid osteoblastoma (EOB) is a rare, benign bone tumor characterized by the proliferation of atypical epithelioid osteoblasts within a vascular stroma. It typically presents as a slow-growing painful mass and requires careful differentiation from more aggressive lesions like osteosarcoma for appropriate management. We present a case of an otherwise healthy 30-year-old man who presented with a mandibular EOB measuring 23 cm × 17 cm x 19 cm, causing significant disfigurement and functional impairment. Surgical management involved radical resection and immediate reconstruction using an oste

A Novel Technique for Resection and Reconstruction of the Temporomandibular Joint by Sliding Vertical Ramus Osteotomy using Only Submandibular Approach.

In this paper, an innovative technique for resection and reconstruction of the temporomandibular joint by sliding vertical ramus osteotomy using only a submandibular approach is presented. Before pulling the posterior mandibular border slightly downward to expose parts of the condyle, the vertical ramus osteotomy was performed. With the help of 3D simulation and surgical templates, the condylectomy was carried out using the ultrasonic osteotome through the submandibular approach. Our technique achieved the desired results while preventing complications of facial nerve paralysis, the occurrence

A combination of follicular and plexiform ameloblastoma: A rare case report.

Ameloblastoma is a benign but locally aggressive neoplasm of odontogenic epithelium origin. Follicular type is the most common variant of ameloblastoma followed by plexiform, acanthomatous, and granular subtypes. Synchronized existence of plexiform ameloblastoma with follicular ameloblastoma is a rare event. When the plexiform type coexists with other common variant, it is termed as hybrid ameloblastoma. Here, we present a case of hybrid ameloblastoma with follicular and plexiform variants occurring in a 43-year-old female patient having persistent swelling for 5-6 months in the left mandibula
