Reflective narrative and dementia care.

This study was an enquiry into how clinical supervision could enable a team of six community psychiatric nurses to provide improved patient-centred care. It also examined how feelings of burn-out, stress and tedium when caring for people with dementia could be relieved. The methodology used in the study analysed the process, enabling the findings to be studied. This is new paradigm research, requiring the researcher and participants to be equal and active coenquirers into the process facilitating the enquiry. Six community psychiatric nurses allowed their period of supervision to be recorded o

Theoretical background, first stage development and adaptation of a novel Integrative Adapt Therapy (IAT) for refugees.

AIMS: Refugees are confronted with the task of adapting to the long-term erosion of psychosocial systems and institutions that in stable societies support psychological well-being and mental health. We provide an overview of the theoretical principles and practical steps taken to develop a novel psychotherapeutic approach, Integrative Adapt Therapy (IAT), which aims to assist refugees to adapt to these changes. This paper offers the background informing ongoing trials of IAT amongst refugees from Myanmar. METHODS: A systematic process was followed in formulating the therapy and devising a tre

Why do family doctors prescribe potentially inappropriate medication to elderly patients?

BACKGROUND: Based on changes in pharmacokinetics and -dynamics in elderly patients, there are potentially inappropriate medications (PIM) that should be avoided in patients aged ≥ 65 years. Current studies showed prescription rates of PIM between 22.5 and 28.4 % in the primary care setting. The evidence concerning reasons for PIM prescription by FPs is limited. METHODS: This mixed method study consisted of three research parts: 1) semi-standardized content analysis of patients' records, 2) qualitative interviews with FPs using a) open questions and b) selected patient-specific case vignettes

Clinical ethics case report: questionable capacity and the guidance of living wills.

After falling from a roof, an older man lost neurological function below his face. In two days, the patient regained consciousness, but it was unclear whether he could communicate his preferences, whether due to injuries or difficulties with language. His family believed he could communicate with them, and that he was capable of making treatment decisions. The staff did not think to contact the hospital's largely inactive ethics consultation service for assistance, and instead looked to the patient's living will for guidance, even though the patient was not terminally ill, and his lack of capa

Fetal alcohol spectrum disorders and the criminal justice system.

The life-long neurological impairments found in people with fetal alcohol spectrum disorders (FASDs), including learning disabilities, impulsivity, hyperactivity, social ineptness, and poor judgment, can increase susceptibility to victimization and involvement in the criminal justice system (CJS). Individuals with FASDs become involved in the CJS as complainants, witnesses, and accused. Their disabilities, resulting from the prenatal alcohol exposure, must be considered at all stages in the legal process. Adverse experiences, such as having a dysfunctional family background, mental health prob

The misdiagnosis of bipolar disorder as a psychotic disorder: some of its causes and their influence on therapy.

OBJECTIVES: Looking at chart records bipolar disorder is often misdiagnosed as a psychotic disorder but no study has ever systematically looked into the reasons. One reason for misdiagnoses could be that clinicians use heuristics like the prototype approach in routine practice instead of strictly adhering to the diagnostic criteria. Using an experimental approach we investigated if the use of heuristics can explain when a diagnosis of psychotic disorder is given instead of bipolar disorder. We systematically varied information about the presence or absence of specific symptoms, i.e. hallucinat

[Subjective illness beliefs of Turkish migrants with mental disorders--specific characteristics compared to german patients].

OBJECTIVE: Knowledge about culture- and migration-specific characteristics of subjective illness beliefs in Turkish patients is necessary for adequate treatment. METHODS: Analysis of subjective illness beliefs in Turkish patients (F3; F4; n = 79) in comparison to matched Germans (n = 79) using a modified version of the Illness Perception Questionnaire IPQ-R. Differences were explored by t-tests and chi(2)-tests. RESULTS: Turkish patients believed significantly stronger in a chronical timeline of illness and in negative illness consequences, while German patients believed significantly strong

Relationship between general health status and the change in chewing ability: a longitudinal study of the frail elderly in Japan over a 3-year period.

OBJECTIVES: The aim of the present study was to identify the onset predictors of a change in chewing ability over a 3-year period in the frail elderly. METHODS: The subjects were frail elderly people living in southern Japan. Data were collected at baseline (n = 92) and 3 years later (n = 60). The dependent variable was a change in self-rated chewing ability. The independent variables were age, gender, number of present teeth, dentition, maximum bite force (evaluated using a pressure-sensitive foil), basic activities of daily living, self-rated general health status, higher level of competenc

Kidney transplants from young children and the mentally retarded.

Kidney donation by young children and the mentally retarded has been supported by court decisions, arguments based on obligations inherent in family relationships, an array of contextual factors, and the principle of beneficence. These justifications for taking organs from people who cannot protect themselves are problematic and must be weighed against our obligation to protect the vulnerable. A compromise solution is presented that strongly protects young children and the mentally retarded but does not abdicate all responsibility to relieve suffering. Guidelines are proposed that prohibit the

Advance directives: can patients' stated treatment choices be used to infer unstated choices?

Advance directives have been widely endorsed as a method to match medical interventions to patients' preferences. However, applying advance directives to unspecified future decisions may be problematic. We wondered how well scenario-based treatment choices in advance directives can be used to infer other choices specified by the same patients. We asked 495 out-patients from the Massachusetts General Hospital to state 11 treatment choices in each of four illness scenarios. We calculated likelihood ratios (LRs) to see how well one choice could predict another. Predictions within a given scenario

Breaking the vicious cycle of delayed healthcare seeking for people who use drugs.

BACKGROUND: People who use drugs (PWUD) are at increased mortality risk, yet they typically avoid healthcare settings due to stigma and shunning. Understanding the healthcare journey from the viewpoint of PWUD has been understudied, although it is essential for informing solutions to increase healthcare access to improve their healthcare outcomes. We aimed to understand the process of accessing healthcare for PWUD, including perceived barriers and facilitators, by exploring their experiences, attitudes, and beliefs. METHODS: We employed purposive sampling to recruit PWUD to participate in nin

Study protocol for ELders AT Ease (ELATE): a cluster randomised controlled trial of cognitive behaviour therapy to reduce depressive symptoms in aged care residents.

BACKGROUND: This protocol describes a study of the effectiveness of cognitive behaviour therapy (CBT) for reducing depressive symptoms in older adults living in residential aged care (RAC) facilities in Australia. Depressive symptoms are highly prevalent in this population, yet the benefits of CBT for reducing such symptoms in RAC facilities have not been widely investigated. Elders at Ease (ELATE) is a 16-session CBT intervention designed for implementation in RAC facilities. The intervention includes cognitive, behavioural and reminiscence strategies and is delivered by mental health trainee

Counselling training for speech-language therapists working with people affected by post-stroke aphasia: a systematic review.

BACKGROUND: Speech-language therapists use counselling to address the psychological well-being of people affected by post-stroke aphasia. Speech-language therapists report low counselling knowledge, skill and confidence for working in post-stroke aphasia which may be related to a lack of counselling training specific to the needs of this client group. AIMS: To identify current training in counselling for speech-language therapists to address psychological well-being in people affected by post-stroke aphasia. Specifically, the intent was to establish the objectives, content, amount, teaching m

A web-based clinical decision tool to support treatment decision-making in psychiatry: a pilot focus group study with clinicians, patients and carers.

BACKGROUND: Treatment decision tools have been developed in many fields of medicine, including psychiatry, however benefits for patients have not been sustained once the support is withdrawn. We have developed a web-based computerised clinical decision support tool (CDST), which can provide patients and clinicians with continuous, up-to-date, personalised information about the efficacy and tolerability of competing interventions. To test the feasibility and acceptability of the CDST we conducted a focus group study, aimed to explore the views of clinicians, patients and carers. METHODS: The C

Using artificial intelligence for physical exercise and healthy ageing among older adults: a scoping review.

BACKGROUND: Promoting regular physical exercise is crucial for healthy ageing, yet many older adults remain physically inactive. Artificial intelligence (AI), evolving from rule-based to data-driven systems, offers a promising tool to address this challenge. This scoping review systematically maps existing research on AI in older adult exercise, focusing on intervention characteristics, types of assessed exercise motivation, physical function, mental health outcomes, and underlying theoretical frameworks. METHODS: Adhering to Arksey & O’Malley’s framework, this review systematically searched S

A realist review of the contribution of person-centred cultures to the management of depression among older persons in nursing homes.

Background Person-centeredness is espoused in many national and international policies and standards and are viewed to inform cultures of care in nursing homes and influence mental health of older adults. In spite of this, studies have shown mixed results regarding the impacts of person-centred cultures on depression with outcomes depending on contexts and implementation approach. In addition, the prevalence of depression in nursing homes remains high. Objective This review aimed to understand how, why and in what contexts person-centred cultures, the expected cultures of care in nursing homes

Strengthening the patient perspective in quality development: a mixed-methods observational study on suitability of the PROMIS GH-10 in Swiss inpatient rehabilitation.

OBJECTIVE: Healthcare systems increasingly use patient-reported outcome measures (PROMs) to complement traditional clinical and performance-based outcomes by capturing health outcomes from the patient perspective and supporting quality improvement and patient-centred care. In Switzerland, the National competence centre for quality measurements in hospitals and clinics ANQ performs the external quality development program. We conducted a nationwide pilot study in inpatient rehabilitation using the generic PROMIS GH-10 to assess its suitability as an outcome for quality development by evaluating
