Accommodation and ciliary muscle muscarinic receptors after echothiophate.

Twice daily topical administration of echothiophate for 2 weeks to the eyes of living cynomolgus monkeys produced profound subsensitivity of the accommodative response to pilocarpine and an approximately 50% decrease in the number of specific binding sites for 3H-quinuclidinyl benzilate (3H-QNB) in the ciliary muscle without a change in their affinity. When echothiophate treatment was discontinued, functional cholinergic sensitivity and the number of QNB binding sites both returned to normal over a similar 4-8 week period. Most animals had a modest overshoot of both functional sensitivity and

Skeletal muscle reinnervation by reduced axonal numbers results in whole muscle force deficits.

Patients sustaining a peripheral nerve injury will frequently experience residual muscle weakness after muscle reinnervation, even if the nerve repair is performed under optimal circumstances to allow rapid muscle reinnervation. The mechanisms responsible for this contractile dysfunction remain unclear. It is hypothesized that after peripheral nerve injury and repair, a reduced number of axons are available for skeletal muscle reinnervation that results in whole muscle force and specific force deficits. A rat model of peroneal nerve injury and repair was designed so that the number of axons av

Rhabdomyolysis caused by tocolytic therapy with ritodrine hydrochloride.

We report a case of rhabdomyolysis with severe generalized weakness and muscle pain after administration of ritodrine hydrochloride, in a pregnant patient without history of neuromuscular disease. Laboratory tests showed an increase of blood CK value and myoglobinuria. An electromyography was performed, revealing a typical myogenic pattern and diffuse denervation activity. Muscular biopsy allowed to rule out inflammatory and metabolic myopathy. After delivery, the patient underwent intensive rehabilitation with progressive improvement of her clinical situation, until complete recovery. Three m

Effects of chronic nerve cuff and intramuscular electrodes on rat triceps surae motor units.

In order to assess the long-term effects of implanted electrodes on motor unit properties, we studied triceps surae (TS) motor units in rats … at was always present in unimplanted rats. These differences were consistent with a small degree of muscle denervation and subsequent reinnervation. Further analyses ascribed this effect to the nerve cuff rather than to the intramuscular electrodes. Comparable changes in motor unit properties may occur in humans with implanted nerve cuffs.

Peripheral nerve damage during limb lengthening. Neurophysiology in five cases of bilateral tibial lengthening.

Limb lengthening is used to correct leg length discrepancy and to increase stature. The reported frequency of peripheral nerve complications varies from 5% to 30%, but is probably underestimated. Damage may be direct or be caused by overstretching of the nerves. We have used electrophysiological tests to evaluate five patients during bilateral tibial lengthening by the Ilizarov method. Results after 24 to 107 days of lengthening showed electromyographic evidence of partial muscle denervation in all 10 limbs, with reduced motor conduction velocities in two tibial nerves and three common peronea…

Assessment, patient selection, and rehabilitation of nerve transfers.

Peripheral nerve injuries are common and can have a devastating effect on physical, psychological, and socioeconomic wellbeing. Peripheral nerve transfers have become the standard of care for many types of peripheral nerve injury due to their superior outcomes relative to conventional techniques. As the indications for, and use of, nerve transfers expand, the importance of pre-operative assessment and post-operative optimization increases. There are two principal advantages of nerve transfers: (1) their ability to shorten the time to reinnervation of muscles undergoing denervation because of p

[Sciatic xeuropathy secondary to compression by a heterotopic ossification due to myositis ossificans traumatica. (Case report and review of the literature)].

We describe a case of sciatic nerve entrapment neuropathy due to a posttraumatic heterotopic ossification (myositis ossificans traumatica). A 52 y.o. man was examined complaining from right lower limb pain and weakness progressing for one year after a blunt trauma and a hematoma of the hip. Clinical examination demonstrated sciatic nerve palsy with pain and paresthesia felt distal to the lesion. Electrodiagnostic studies show severe denervation changes in the tibialis anterior, extensor digitorum brevis and gastrocnemius muscles. Both radiography and CT scan revealed new bone formation in the

Observations on the interstitial cells of Cajal and neurons in a recovered case of equine dysautonomia (grass sickness).

This paper describes the histopathological and immunohistochemical changes in the central, autonomic and enteric nervous systems in a well-documented case of equine dysautonomia (ED), after the animal had recovered without significant residual clinical signs. Evidence of neuronal degeneration, such as neuronal chromatolysis, glial scars or a decrease in density of neurons, was not observed in the midbrain, pons, medulla oblongata or spinal cord, including the nuclei of cranial nerves III, V, VII, X and XII. In addition, no evidence of muscle denervation or re-innervation, such as group atrophy…

Determination of the optimal conditions for laryngeal pacing with the Itrel II implantable stimulator.

OBJECTIVE: To determine the optimal stimulus paradigm, electrode orientation, and configuration of an implantable stimulator used to reanimate the posterior-cricoarytenoid (PCA) muscle in case of bilateral vocal fold paralysis (BVFP). STUDY DESIGN: Acute studies were conducted on 13 canines implanted with Itrel II systems with or without PCA innervation. PCA stimulus-response characteristics were obtained by measuring stimulated vocal fold displacement endoscopically. RESULTS: The denervated PCA was only 10% to 25% as responsive to stimulation as the innervated PCA. However, the response cou

"Supraspinatus slide" for rotator cuff repair.

The supraspinatus slide procedure described by Debeyre et al. (1965) offers an opportunity for the surgeon to repair certain large and late … e muscle remained viable in all patients. Electro-myographical studies failed to show supraspinatus muscle denervation in any case. The procedure proved to be useful in the repair of large rotator cuff ruptures that are not amenable to other methods of repair. A young well motivated patient with chronic disability may benefit from this procedure.

Direct neurectomy of the motor branches of the tibial nerve in hemiplegic adults: an assessment with a mean follow-up period of 11 years.

INTRODUCTION: Neurectomy of the tibial nerve plays a major role in the relief of disabling spasticity, which is refractory to drug treatment and physiotherapy. Although the immediate postoperative results are generally satisfactory, few evaluations of the procedure's long-term efficacy have been published. OBJECTIVE: To estimate the long-term efficacy of total or partial neurectomy of the motor branches of the tibial nerve (combined with additional orthopaedic surgery in some cases). METHOD: A descriptive, retrospective study of 25 brain-damaged patients having undergone neurectomy at least

[Botulinum toxin type A in the treatment of neurogenic dislocation of the temporomandibular joint].

Botulinum toxin leads to paresis of the skeletal muscle lasting 2-4 months via an inhibition of acetylcholine release at the neuromuscular junction. Since 1995, botulinum toxin injections have been used in the treatment of recurrent dislocation of the temporomandibular joint (TMJ). The chemical denervation of the external pterygoid muscle restricts the angle of mouth opening, thus helping to prevent dislocation. TMJ dislocations that occur as a result of increased tone in the protracted masticatory muscles were recently defined as neurogenic dislocations of the TMJ. We conducted a clinical stu

Dynamic upper vocal tract articulations during zebra finch song - acoustic effects and independence of instantaneous auditory feedback.

In birds, the sound generated in the syrinx is modified by upper vocal tract filter properties prior to being emitted. Filtering of upper harmonics, for example, allows birds to produce tonal sounds. The main dynamic filter component is the oropharyngeal-esophageal cavity (OEC), whose volume can be adjusted to track the fundamental frequency of modulated sounds. It is less well understood how birds that use harmonically rich sounds, such as the zebra finch (Taeniopygia guttata), engage upper vocal tract structures to produce the complex spectral composition of their vocalizations. Furthermore,

A cross-sectional imaging study to identify organs at risk of thermal injury during renal artery sympathetic denervation.

BACKGROUND: The technology used to perform catheter-based renal artery sympathetic denervation has evolved: catheters can now access arteries as small as 3mm in diameter and create ablation zones of up to 10mm in depth. Recent evidence suggests that the procedure may be more effective if a more thorough ablation strategy is employed. Limited data are available regarding inadvertent soft tissue thermal injury during such procedures.  We used computed tomography (CT) to identify structures lying within the expected thermal ablation field or the 'at risk zone' (ARZ). METHODS: 63 consecutive CT a

Skin-derived precursors generate enteric-type neurons in aganglionic jejunum.

PURPOSE: Skin-derived precursor cells (SKPs) may regenerate the enteric nervous system in Hirschsprung's disease. SKPs migrate and differentiate into myenteric ganglia in aganglionic intestine. We sought to characterize the time-course of SKP gangliogenesis and enteric neurotransmitter synthesis in vivo. METHODS: Adult Lewis rat jejunal segments were isolated and denervated with benzalkonium chloride (BAC). Denervation was evaluated by immunohistochemical (IHC) stains for markers of mature neuronal and glial cells. Green fluorescent protein (GFP)-expressing neonatal rat SKPs were cultured in

Single fibre electromyography in multifocal motor neuropathy with persistent conduction blocks.

OBJECTIVE: To study the process of denervation-reinnervation in multifocal motor neuropathy with persistent conduction blocks in clinically affected and unaffected muscles. METHOD: Volitional single fibre electromyography (SFEMG) was performed in the extensor digitorum communis (EDC) of seven patients. The jitter, the fibre density, and the mean interpotential interval were determined. The results before and after treatment with intravenous immunoglobulin (IVIg) between the unaffected EDC and affected EDC examined during the same SFEMG session were also compared. In addition the values of jit

Noradrenergic and peptidergic innervation of secondary lymphoid organs: role in experimental rheumatoid arthritis.

Noradrenergic (NA) and peptidergic nerve fibres are present in both primary and secondary lymphoid organs, distributing with the vasculature, trabecular and capsular smooth muscle, and within the parenchyma among cells of the immune system. NA nerve terminals directly abut lymphocytes and macrophages in spleen and lymph nodes. In these organs, norepinephrine has fulfilled the basic criteria for neurotransmission with cells of the immune system as targets. In vitro and in vivo studies have demonstrated NA modulation of primary and secondary antibody responses, cytotoxic T cell responses, natura

Laryngeal reinnervation for unilateral vocal fold paralysis in adults; a systematic review of the literature for the clinician.

PURPOSE: Unilateral vocal fold paralysis (UVFP), resulting from recurrent laryngeal nerve injury, significantly impairs phonation and reduces quality of life. Laryngeal reinnervation, including ansa-to-recurrent (ansaNSR) and nerve-muscle pedicle reinnervation combined with arytenoid adduction (NMPR + AA), has gained attention as a durable treatment option. This systematic review aims to evaluate the outcomes of laryngeal reinnervation, identify predictive factors and highlight knowledge gaps relevant to clinical practice. METHODS: A systematic search of PubMed, Embase, Web of Science, the Coc
