Comprehensive analysis of carotid triangle lymph node metastasis in papillary thyroid carcinoma.

BACKGROUND: Carotid triangle lymph nodes (CAT LN) represent a frequently overlooked region during lateral neck dissection (LND) in patients with papillary thyroid carcinoma (PTC), and their metastatic characteristics remain poorly defined. This study aimed to comprehensively evaluate the clinicopathological risk factors and metastatic patterns associated with CAT LN involvement. METHODS: This single-center retrospective study analyzed 410 lateral neck dissections performed in 380 patients with PTC. The lateral neck side, paired with the ipsilateral thyroid lobe and ipsilateral central compart…

Diagnostic value of sentinel lymph node biopsy in head and neck cancer: a meta-analysis.

This study aimed to evaluate the diagnostic reliability of sentinel lymph node biopsy in patients with squamous cell carcinoma of the oral c … ously treated necks. All patients had sentinel lymph node biopsy performed followed by a concurrent neck dissection. Twenty-six studies met our inclusion criteria (n = 766 patients). The pooled sensitivity and negative predictive value of SLNB for all head and neck tumors was 95 % (95 % CI 91-99 %) and 96 % (95 %CI 94-99 %), respectively. The overall sensitivity …

Therapeutic central lymph node dissection in papillary thyroid cancer: what is the optimal extent in unilateral nodal disease?

BACKGROUND: The optimal extent of central lymph node dissection (CLND) in patients with unilateral papillary thyroid cancer (PTC) and clinic … h transient (3/75) and permanent (1/75). CONCLUSIONS: Our findings suggest that restricting central neck dissection to the clinically affected side may be oncologically safe, minimizing complications associated with bilateral procedures.

Pattern of drainage in sentinel lymph nodes after intra-arterial chemotherapy for oral and oropharyngeal cancer.

PURPOSE: Prerequisite for lymphoscintigraphy-guided sentinel lymph node (SLN) biopsy in head and neck cancer patients has been a previously … formed in 50 neck sides, followed by radical resection of the primary in the same session. Elective neck dissection was not carried out. The pattern of lymphatic drainage was recorded and compared with the literature on well-known patterns found in lymphographic studies and metastatic disease in neck dissection specimens of patients without presurgical treatment. …

A Comparative Study of the Etiology of Intracranial Vertebral Artery Dissection and Carotid Artery Dissection.

BACKGROUND: Both extrinsic (trauma or violent movement) and intrinsic (structural abnormality, atherosclerosis, or hemodynamic instability) factors may result in arterial dissection. The role of these factors in the origin or progression of dissection remains unclear. This study aimed to characterize the clinical features, radiologic features, and outcomes of intracranial vertebral artery dissection compared with carotid artery dissection and to determine the major causative factors. METHODS: Consecutive patients with craniocervical dissection (n=127) were retrospectively analyzed. Patients w

Cystadenocarcinoma of the Sublingual Gland: A Rare Subvariant of Salivary Gland Carcinoma.

Cystadenocarcinoma is a malignant tumor that has undergone various classifications due to its wide variety of pathological forms since the World Health Organization (WHO) classification in 2005. We present a case involving a 72-year-old man who reported pain and swelling in the left floor of his mouth during eating. Examination demonstrated a thumb-sized, mobile mass with elastic softness. Resection of the sublingual gland tumor and left submandibular neck dissection were performed under general anesthesia. The lesion was encased in a thin fibrous capsule and consisted of tall columnar epithel…

Intraoperative quantitative assessment of parathyroid blood flow during total thyroidectomy using indocyanine green fluorescence imaging - surgical strategies for preserving the function of parathyroid glands.

OBJECTIVE: We investigated the factors affecting postoperative parathyroid gland (PTG) function and devised an objective index to predict th … n. RESULTS: Postoperative hypoparathyroidism is significantly associated with simultaneous central neck dissection (CND) and lateral neck dissection (LND) (p = .032). The Spearman correlation test showed a moderate correlation between the MIR and iPTH levels (p = .0047). The optimal MIR cutoff value for predicting postoperative hypoparathyroidism was 2.14 with a …

Parotidectomy in patients with head and neck cutaneous melanoma with cervical lymph node involvement.

BACKGROUND: Parotidectomy in melanoma of the coronal scalp and face with clinically involved cervical lymph node metastasis is based on predicted cervical lymphatic drainage described by O'Brien. METHODS: In total, 40 parotidectomies with en bloc therapeutic neck dissection were retrospectively analyzed. RESULTS: Lymphatic spread of melanoma to the parotid lymph nodes was observed in 10 of 40 specimens (25%). Eight of the 10 parotid-positive patients developed a recurrence vs 17 of the 30 parotid-negative patients (P = 0.28). There were no differences in overall survival, melanoma-specific s…

Larynx preservation surgery in pyriform sinus cancer.

As a general rule, surgery, whenever possible followed by irradiation, is considered to be a standard treatment for pyriform sinus cancer. In early primary lesions, many different approaches have been tried for larynx preservation. From 1990-1997, 7 patients with pyriform sinus carcinoma without the involvement of pyriform sinus apex were treated with partial laryngopharyngectomy at the Asan Medical Center. Three patients were T1 stage and four were T2. All patients had nodal metastases and were treated with neck dissection and postoperative radiotherapy. Follow-up period was 1-8 years. One re…
