β oscillation during slow wave sleep and rapid eye movement sleep in the electroencephalogram of a transgenic mouse model of Huntington's disease.

STUDY OBJECTIVES: To search for early abnormalities in electroencephalogram (EEG) during sleep which may precede motor symptoms in a transgenic mouse model of hereditary neurodegenerative Huntington's disease (HD). DESIGN: In the R6/1 transgenic mouse model of HD, rhythmic brain activity in EEG recordings was monitored longitudinally and across vigilance states through the onset and progression of disease. MEASUREMENTS AND RESULTS: Mice with chronic electrode implants were recorded monthly over wake-sleep cycles (4 hours), beginning at 9-11 weeks (presymptomatic period) through 6-7 months (s

Separation surgery for total vertical craniopagus twins.

OBJECTIVE: A pair of conjoined twins aged 11 months underwent investigations, followed by surgical separation in Singapore General Hospital in April 2001. They were joined at the skull vertex and facing in opposite directions. METHODS: Radiological investigations including cerebral angiography, magnetic resonance imaging and computerized tomographic scans were performed, leading to the diagnosis of total vertical craniopagus. There were two separate brains, with separate arterial circulations, but with a common superior sagittal sinus. Tissue expanders were inserted in the subgaleal space for

Perioperative multimodality neuromonitoring: an overview.

Neurologic damage after cardiac surgery remains an important cause of postoperative morbidity. In addition to a wide variety of procedural risks, patient-specific factors such as the presence of extracranial or intracranial atherosclerotic disease, either alone or together, have a fundamental impact on the risk of brain injury developing after cardiovascular surgery. A variety of neurophysiologic monitoring techniques have been used during cardiovascular surgery in hopes of averting neurologic injury. In this issue of Seminars, the strengths and weaknesses of each are discussed by a group of h

Unconscious amygdalar fear conditioning in a subset of chronic fatigue syndrome patients.

Here, a novel hypothesis for chronic fatigue syndrome (CFS) is proposed. CFS may be a neurophysiological disorder focussing on the amygdala. During a 'traumatic' neurological event often involving acute psychological stress combined with a viral infection or other chemical or physiological stressor, a conditioned network or 'cell assembly' may be created in the amygdala. The unconscious amygdala may become conditioned to be chronically sensitised to negative symptoms arising from the body. Negative signals from the viscera or physiological, chemical and dietary stressors, become conditioned st

Brain oscillations as biomarkers in neuropsychiatric disorders: following an interactive panel discussion and synopsis.

This survey covers the potential use of neurophysiological changes as a biomarker in four neuropsychiatric diseases (attention deficit hyperactivity disorder (ADHD), Alzheimer's disease (AD), bipolar disorder (BD), and schizophrenia (SZ)). Great developments have been made in the search of biomarkers in these disorders, especially in AD. Nevertheless, there is a tremendous need to develop an efficient, low-cost, potentially portable, non-invasive biomarker in the diagnosis, course, or treatment of the above-mentioned disorders. Electrophysiological methods would provide a tool that would refle

Whole-Brain Dynamic Causal Tractography Atlas of Auditory and Visual Speech Networks Derived From Intracranial Neurophysiologic Data.

BACKGROUND AND OBJECTIVES: Since Penfield's era, invasive neurophysiology has laid a lasting foundation for functional neuroscience by elucidating brain regions necessary for speech. Yet, whole-brain studies have not distinguished the millisecond-scale dynamics and specific white matter pathways that support rapid naming in the auditory and visual domains. We developed a whole-brain dynamic causal tractography atlas of speech using intracranial neurophysiologic data. METHODS: This observational study analyzed intracranial EEG during naming tasks in patients with drug-resistant focal epilepsy

Neurophysiological responses of globus pallidus internus during the auditory oddball task in Parkinson's disease.

Parkinson's disease can be associated with significant cognitive impairment that may lead to dementia. Deep brain stimulation (DBS) of the subthalamic nucleus is an effective therapy for motor symptoms but is associated with cognitive decline. DBS of globus pallidus internus (GPi) poses less risk of cognitive decline so may be the preferred target. A research priority is to identify biomarkers of cognitive decline in this population, but efforts are hampered by a lack of understanding of the role of the different basal ganglia nuclei, such as the globus pallidus, in cognitive processing. Comparison of Single and Dual Monitoring during Carotid Endarterectomy.

We compared the rate of selective shunt and pattern of monitoring change between single and dual monitoring in patients undergoing carotid endarterectomy (CEA). A total of 121 patients underwent 128 consecutive CEA procedures. Excluding five procedures using internal shunts in a premeditated manner, we classified patients according to the monitoring: Group A (n = 72), patients with single somatosensory evoked potential (SSEP) monitoring; and Group B (n = 51), patients with dual SSEP and motor evoked potential (MEP). Among the 123 CEAs, an internal shunt was inserted in 12 procedures (9.8%) due

Intracranial EEG for Detecting Epileptic Activity in Acute Brain Injury: A Systematic Review and Meta-analysis.

Intracranial electroencephalography (iEEG) has been increasingly explored as part of multimodal neuromonitoring in acute brain injury, yet its diagnostic benefit compared with continuous surface EEG (suEEG) remains uncertain. We have performed a meta-analysis to quantify seizure and rhythmic and periodic pattern detection rates and concordance with suEEG. Secondary objectives were the assessment of correlations with brain injury, safety, and techniques. Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, PubMed, Scopus, and Embase were searched

Augmented and prematurely occurring H-reflex recovery curve shows enhanced spinal network excitability in ALS patients when employing the double stimulation paradigm.

Amyotrophic Lateral Sclerosis (ALS) is a fatal neurodegenerative disorder in which motoneuron loss in the brain and spinal cord induces complex neuroplastic changes. Although these alterations hold considerable potential for clinical diagnosis and disease monitoring, they remain underutilized due to the lack of sensitive and non-invasive assessment methods. The H-reflex is a monosynaptic spinal reflex arc and represents the neurophysiological analog of the Achilles tendon reflex. A modified H-reflex double-stimulation paradigm enables differentiation of the temporal dynamics of spinal inhibito

Assessing effects of microwave electromagnetic field in APP/PS1 mice by in vivo GABA-weighted imaging via variable delay multipulse-chemical exchange saturation transfer MRI.

Electromagnetic fields (EMFs) have been shown to be beneficial in treating Alzheimer's disease (AD), but the underlying neurophysiological mechanisms remain unclear. It has been proposed that EMF promotes GABAergic neurogenesis and that abnormal GABA levels are an important influence on the progression of AD. To achieve in vivo GABA-weighted imaging in the brain in APP/PS1 mice, we utilized variable delay multi-pulse (VDMP)-chemical exchange saturation transfer (CEST)- magnetic resonance imaging (MRI) and pathological validation to measure GABA levels in APP/PS1 mice under microwave EMF and ex

Serum albumin and uric acid: biomarkers of neurocognitive and physical function in early Parkinson's disease.

Serum albumin and uric acid (UA), both recognized for their antioxidant properties, have been linked to Parkinson’s disease (PD) progression. However, their associations with neurocognitive performance and physical fitness in early-stage PD remain unclear. To examine relationships between albumin and UA with cognitive performance, neurophysiological indices, and physical function in early-stage PD. Forty-eight individuals with early-stage PD were stratified into high and low groups based on median serum albumin and UA levels. Supplementary regression analyses examined dose-response association

Open-skill vs. closed-skill sports and executive function in children and adolescents: a mini review of mechanisms and moderating factors.

Executive function (EF), comprising inhibitory control, working memory, and cognitive flexibility, undergoes critical maturation during childhood and adolescence and predicts academic achievement, social adjustment, and mental health. Although physical exercise is now broadly accepted as a cognitive enhancer, growing evidence suggests that the type of motor activity matters: open-skill sports (e.g., basketball, badminton), performed in unpredictable, externally-paced environments demanding rapid perception-action coupling, may differ from closed-skill sports (e.g., running, swimming), performe

Robotic Training and Neural Reorganization in Stroke: A Systematic Review.

INTRODUCTION: Stroke is a primary contributor to global adult morbidity, frequently resulting in debilitating gait disturbances that impede independence. While standard neurorehabilitation remains essential, the advent of robot-assisted gait training (RAGT) provides a platform for high-intensity, reproducible interventions designed to stimulate neuroplasticity. This systematic review evaluated the literature from 2015 to 2025 to determine the influence of RAGT on neural reorganization and functional outcomes in stroke survivors. METHODOLOGY: A systematic search of MEDLINE, PubMed, Scopus, and

Speech and Language Changes During Rapid Eye Movement (REM) Sleep with Potential Diagnostic Markers: A Systematic Review.

Background: Rapid Eye Movement (REM) sleep behavior disorder (RBD) is a parasomnia resulting from degeneration of pontine and medullary circuits responsible for muscle atonia during REM sleep, leading to dream-enactment behaviors and vocalizations. It is strongly linked to α-synucleinopathies, particularly Parkinson's disease. Current biomarkers such as neurophysiological measures and imaging support diagnosis and monitoring, but remain invasive or costly. Aim: This study aims to evaluate vocal and speech alterations as exploratory, non-validated candidate biomarkers of REM sleep behavior diso
