Posterior Hip Dislocation with Ipsilateral Subtrochanteric Femur Fracture: A Rare Case Report.

INTRODUCTION: Posterior dislocation of the hip associated with an ipsilateral subtrochanteric femur fracture is a rare and technically challenging injury pattern, most commonly seen after high-energy trauma such as road traffic accidents. The associated proximal femoral fracture disrupts the normal femoral lever arm, making closed reduction difficult or impossible and often necessitating open reduction. Delay in diagnosis or treatment may result in complications such as avascular necrosis of the femoral head, post-traumatic arthritis, sciatic nerve injury, and poor functional outcomes. CASE R

MR Imaging of Pediatric Elbow Trauma.

Magnetic resonance imaging (MRI) of the pediatric elbow is important because it allows delineation of fracture type in a joint with complex anatomy, made more complex by the variable appearance of ossification centers of the distal humerus.1-4 Acute and chronic injuries can be evaluated with MRI using several classification systems. Accurate diagnosis of pediatric elbow trauma with MRI has management implications. For example, a Salter-Harris II fracture can be differentiated from a Salter-Harris IV physeal fracture by delineating intra-articular fracture extension and displacement, which requ

Which B2 Fractures Can Be Treated with ORIF? Validation of the "Beyond the Vancouver Classification".

Background and objectives: The objective was to validate the Beyond the Vancouver classification. Based on this algorithm, it was hypothesized that cemented polished tapered stems with an intact cement mantle and cementless stable stems with defined criteria could be classified as stable and therefore treated with open reduction and internal fixation (ORIF). Materials and Methods: This retrospective, single-center cohort study re-analyzed patients initially diagnosed with Vancouver type B2 fractures treated with ORIF between 2007 and 2020. Clinical and radiological outcomes were extracted from

Pediatric proximal tibial fractures: How does the trauma mechanism guide the operative strategy?

PURPOSE: Pediatric proximal tibial fractures (PTF) are rare but potentially debilitating. So far, no system for guiding surgical treatment based on injury-force mechanism has been documented, while adult tibial plateau fractures have benefited greatly from such an approach. This study reviews the diagnosis and treatment experience at a tertiary trauma center and introduces the reduction-traction method. METHODS: Pediatric patients (0-17 years old) diagnosed with PTF were identified in the hospital database from 2017 to 2021. Their injury mechanism, injury location, treatment type, and treatme

Epidemiology & management of complex ankle fractures in the United Kingdom: A multicentre cohort study.

BACKGROUND: Patient factors are known to contribute to decision making and treatment of ankle fractures. The presence of poor baseline mobility, diabetes, neuropathy, alcoholism, cognitive impairment, inflammatory arthritis or polytrauma can result in a higher risk of failure or complications. Limited evidence is available on the optimum management for this challenging cohort of patients herein described as complex ankle fractures. This UK multicentre study assessed and evaluated the epidemiology of ankle fractures complicated by significant comorbidity and patient factors and use of specialis

Treatment of Geriatric Acetabular Fractures-A Concise Review of the Literature.

Optimal management of acetabular fractures (AF) in the elderly has not been defined clearly. The incidence of such fractures is rising in the aging population. Advancements in implant technology have improved the longevity of combined or staggered total hip arthroplasty procedures for this patient population, thus allowing earlier weight bearing and continued functional independence. Perioperative/postoperative complication rates remain significantly high in all treatment arms. Overall, the best outcomes with the lowest complication rates are achieved when AF are treated by a surgeon or a team

A comparative study protocol of external fixation versus volar plate in treating distal radius fracture.

BACKGROUND: The superiority of the open reduction and internal fixation (ORIF) and external fixation remains uncertain owing to the limited sample size involved in the literature. This retrospective cohort research was implemented for the comparison of the efficiency of ORIF and external fixation utilizing the locked volar plating in treating the intra-articular fractures of distal radius. We hypothesized that compared with the external fixation, ORIF can improve the functional scores and reduce complications. METHODS: We conducted a retrospective and single-center cohort trial that was appro

VIS-IT: Visualizing the Injured Tibia-A Cadaveric Study of Limb Positioning for Posterolateral Tibial Plateau Fracture Visualization.

Posterolateral tibial plateau (PLTP) fractures are often associated with anterior cruciate ligament (ACL) incompetence, such as tibial eminence fractures. Both occur from a pivot shift like mechanism. Malreductions of the tibial plateau most frequently occur in the posterolateral quadrant. Acquiring adequate intraoperative visualization of the PLTP poses a challenge. We hypothesized that visualization of PLTP could be improved by positioning the knee at 110 degrees of flexion with the addition of a varus anterolateral rotatory vector. This position and maneuver take advantage of both the nonis

Posteromedial elbow dislocation with lateral humeral condylar fracture in children: Three case reports and a literature review.

RATIONALE: Posteromedial dislocations of the elbow with lateral humeral condylar fractures (LCFs) are uncommon, and only isolated cases have been reported in the English-language literature. Because of the complex radiolucent cartilaginous structures and late-appearing ossification centers, radiological diagnosis of elbow dislocations with LCF in children is challenging. PATIENT CONCERNS: We report three children with posteromedial elbow dislocation: two patients with Milch type I and one patient with Milch type II LCF. DIAGNOSES: In our report, radiographs showed only a small bone fragment,

Management of displaced femoral neck fractures in young adults (a group at risk).

25 young adults (age 15-50 years) with femoral neck fractures were operated on an ordinary operating table, using a Watson-Jones approach. Open reduction of the fracture site through an anterior capsular incision was performed and fixation with three cancellous screws was done. Patients were regularly assessed for clinical and radiological evidence of non-union and avascular necrosis. Average follow-up was 32 months. Non-union was seen in one case (4 per cent) and evidence of avascular necrosis was seen in three cases (12 per cent). The results were compared with available published series of

[Shoulder girdle injury in childhood. Operation or conservative procedure?].

The fracture of the clavicle is a very frequent injury to children and young adults. Using a figure-of-eight bandage for treatment leads to good functional result and acceptable cosmetical appearance. Indications for an open reduction are rare: they are confined to brachial plexus lesions, injuries to the subclavian vessels, open fractures or fractures of the outer third of the clavicle combined with an acromioclavicular separation. Fractures of the scapula merely require immobilization and subsequent physiotherapy to yield satisfactory results. Distorsions and subluxations of the joints of th

Irreducible extremity fractures in pediatric and adolescent rodeo-related trauma: A retrospective case series.

PURPOSE: Although most pediatric extremity fractures can be managed with closed reduction, even when operative fixation is required, youth rodeo exposes participants to high-energy livestock-related trauma that may produce fracture patterns associated with failed closed reduction. This study described operative management and fracture irreducibility in pediatric and adolescent extremity injuries sustained during organized rodeo participation. METHODS: A retrospective case series was performed at a Level I pediatric trauma center. Patients aged ≤18 years who sustained injuries during organized

Open dorsomedial fracture-dislocation of the medial cuneiform with concomitant tibialis anterior tendon rupture: a case report and review of the literature.

Medial cuneiform fracture-dislocations are rare and typically occur in the setting of high-energy midfoot trauma. We report the case of a 30-year-old woman who sustained an open dorsomedial fracture-dislocation of the medial cuneiform with concomitant tibialis anterior tendon avulsion after a 6-m fall. She underwent emergent irrigation and debridement, open reduction, and internal fixation of the midfoot, and primary repair of the tibialis anterior tendon. Intraoperatively, a longitudinal split tear with avulsion from the medial cuneiform insertion was identified and repaired through a bone tu

Open intercondylar fractures of the distal humerus: Management using a mini-external fixator construct.

Intercondylar fractures of the distal humerus are a challenging entity to treat. Open reduction-internal fixation with plating is well accepted as the standard treatment for these fractures, because it allows early mobilization and prevents joint stiffness. However, this technique may be associated with inferior postoperative results and a higher risk of infection in open fractures. External fixation appears to be an interesting option in such cases, where the results of open reduction-internal fixation with plating may be suboptimal. We present a bilateral uniplanar mini-external fixator cons

[Fracture dislocation of the ankle with retro-tibial luxation of the fibula (Bosworth fracture) . Apropos of a case].

The authors present a case of fracture dislocation of the ankle joint with the proximal part of the fibula entrapped behind the tibia ; Bosworth's fracture. This fracture was described for the first time in 1848 by Hugier, and classified by Bosworth in 1947. In our case, diagnosis was not done in a first time, and closed reduction failed. This fracture was operated on and open reduction was performed. At the present time, the functional result is good.

Treatment of neglected femoral neck fracture.

Intra-capsular femoral neck fractures are seen commonly in elderly people following a low energy trauma. Femoral neck fracture has a devastating effect on the blood supply of the femoral head, which is directly proportional to the severity of trauma and displacement of the fracture. Various authors have described a wide array of options for treatment of neglected/nonunion (NU) femoral neck fracture. There is lack of consensus in general, regarding the best option. This Instructional course article is an analysis of available treatment options used for neglected femoral neck fracture in the lit
