Non-group D streptococcal meningitis misidentified as enterococcal meningitis. Diagnostic and therapeutic implications of misdiagnosis by screening microbiology.

Two patients had nonhemolytic Gram-positive coccal meningitis. Both pathogens were initially misidentified as a group D enterococcus by growth in "selective" media, which led to the use of inappropriate and potentially toxic systemic and intrathecal aminoglycosides. Careful evaluation of the antibiotic sensitivity data and additional microbiological studies allowed correct identification of the organism. The important diagnostic and therapeutic considerations in differentiating true enterococcal infections, especially meningitis, from those caused by other alpha-hemolytic or nonhemolytic strep

Effect of a patient-centered planning intervention on postpartum visit attendance.

INTRODUCTION: Pregnant individuals with cardiometabolic conditions-including diabetes, hypertensive disorders, and obesity-are at elevated risk for postpartum complications. Timely postpartum care is critical for mitigating postpartum risks, yet postpartum visit attendance remains suboptimal, and few evidence-based interventions exist, especially among those receiving care in safety-net health systems. We aimed to evaluate whether a patient-centered postpartum care planning intervention increases comprehensive postpartum visit attendance among obstetric patients with at least one cardiometabol

Early detection and risk stratification in autoimmune-related interstitial lung disease: a state-of-the-art review.

Interstitial lung disease (ILD) is a major pulmonary complication of autoimmune rheumatic diseases (ARD) and a leading contributor to long-term morbidity and mortality. Although ARDs share underlying immune dysregulation, the onset, radiologic phenotype, and clinical course of ILD vary substantially across individual diseases. Consequently, early detection and structured risk stratification at baseline and during follow-up have become essential elements of care for patients at risk of ARD-related ILD.This review examines the principles and emerging strategies for early identification of ARD-re

Detection of Spatio-Temporal Dispersion Electrograms Using Complex Signal Identification-Labeled Abnormal Potential Areas in Patients With Persistent Atrial Fibrillation.

INTRODUCTION: Ablation targeting spatio-temporal dispersion electrograms (STDEs) in addition to pulmonary vein isolation (PVI) has been reported to reduce atrial fibrillation (AF) recurrence in patients with persistent AF. BACKGROUND: However, STDE identification remains subjective. Complex Signal Identification (CSI), integrated into the CARTO 3 mapping system, automatically quantifies electrogram characteristics. We evaluated whether CSI parameters can accurately identify STDEs. METHODS: We retrospectively analyzed intracardiac electrogram points from five consecutive patients who underwen

Comparison of inflammatory biomarker levels in neurodegenerative proteinopathies: a case-control study.

While diagnostic criteria have been established and validated for most neurodegenerative diseases, the considerable overlap between individual nosological entities remains a significant diagnostic challenge. Increasing evidence suggests that neurodegeneration is often initiated by inflammation within the central nervous system. The identification of inflammation could serve as a first signal of the pathophysiological process. As such, validated biological markers ("biomarkers") of neuroinflammation are critically important. This study aimed to assess the presence and levels of inflammatory bio

Gastrointestinal Dysfunction in Parkinson's Disease: Current and Potential Therapeutics.

Abnormalities in the gastrointestinal (GI) tract of Parkinson's disease (PD) sufferers were first reported over 200 years ago; however, the extent and role of GI dysfunction in PD disease progression is still unknown. GI dysfunctions, including dysphagia, gastroparesis, and constipation, are amongst the most prevalent non-motor symptoms in PD. These symptoms not only impact patient quality of life, but also complicate disease management. Conventional treatment pathways for GI dysfunctions (i.e., constipation), such as increasing fibre and fluid intake, and the use of over-the-counter laxatives

Molecular and Matrix-Assisted Laser Desorption Ionization-Time of Flight Mass Spectrometry-Based Characterization of Clinically Significant Melanized Fungi in India.

Melanized or black fungi are a heterogeneous group of fungi causing cutaneous to systemic diseases with high mortality. These fungi are rarely reported as agents of human infections, primarily due to difficulties in their classical identification. In this study, we examined, using molecular methods and matrix-assisted laser desorption ioniazation-time of flight mass spectrometry (MALDI-TOF MS), the diversity of melanized fungi (MF) isolated from patients in 19 medical centers in India. Overall, during a 4-year period, 718 (5.3%) clinical specimens yielded MF. Of these, 72 (10%) isolates had cl

Isolation of Candida Species from Gastroesophageal Lesions among Pediatrics in Isfahan, Iran: Identification and Antifungal Susceptibility Testing of Clinical Isolates by E-test.

BACKGROUND: Candida species can become opportunistic pathogens causing local or systemic invasive infections. Gastroesophageal candidiasis may depend on the Candida colonization and local damage of the mucosal barrier. Risk factors are gastric acid suppression, diabetes mellitus, chronic debilitating states such as carcinomas, and the use of systemic antibiotics and corticosteroids. The aim of this study is collection and molecular identification of Candida species from gastroesophageal lesions among pediatrics in Isfahan, and determination of minimum inhibitory concentration (MIC) ranges for

Intraoperative Laparoscopic Near-Infrared Fluorescence Cholangiography to Facilitate Anatomical Identification: When to Give Indocyanine Green and How Much.

Recent technological advances have enabled real-time near-infrared fluorescence cholangiography (NIRFC) with indocyanine green (ICG). Whereas several studies have shown its feasibility, dosing and timing for practical use have not been optimized. We undertook a prospective study with systematic variation of dosing and timing from injection of ICG to visualization. Adult patients undergoing laparoscopic biliary and hepatic operations were enrolled. Intravenous ICG (0.02-0.25 mg/kg) was administered at times ranging from 10 to 180 minutes prior to planned visualization. The porta hepatis was exa

Machine learning-based detection of missed inspiratory efforts using esophageal pressure during noisy pressure support ventilation.

BACKGROUND: Missed inspiratory efforts represent one of the most frequent and clinically relevant forms of patient-ventilator asynchrony during assisted mechanical ventilation. Their detection typically requires expert inspection of ventilator waveforms or the use of esophageal pressure (PESO), which limits continuous bedside recognition. Machine-learning approaches may enable automated, real-time identification of these events. METHODS: In this pilot observational physiological study, mechanically ventilated patients receiving pressure support ventilation were recorded under three levels of
