Cerebral blood flow in spinocerebellar degenerations: a single photon emission tomography study in 28 patients.

We used single photon emission tomography to study regional cerebral perfusion in patients with different forms of spinocerebellar degeneration: 6 patients with Friedreich's ataxia (FA), 6 with early-onset cerebellar ataxia with retained tendon reflexes (EOCA), 5 with autosomal dominant cerebellar ataxia type 1 (ADCA I) and 11 with idiopathic late-onset cerebellar ataxia (ILOCA). The results were related to clinical and magnetic resonance imaging (MRI) findings. Cerebellar hypoperfusion was constant in ADCA I and frequent in patients with other spinocerebellar degenerations. Brain stem hypoper

Peripheral autonomic involvement in the carpal tunnel syndrome.

The sympathetic skin response (SSR), evoked from the middle finger of both hands by electrical stimuli to the median nerve (MN) at the wrist, was studied in 21 patients with bilateral carpal tunnel syndrome (CTS) and in 16 patients with monolateral CTS (14 at the right and 2 at the left side) without clinical signs of autonomic involvement. In monolateral and bilateral CTS there was a decrease in the SSR areas of both sides. In monolateral CTS the decrease was greater contralaterally to the lesion. A decrease in the SSR in CTS generally indicates a local blockade of sympathetic nerve excitabil

[Six cases of anesthesia mumps].

Six cases of acute transient enlargement of the parotid gland under general anesthesia, so called anesthesia mumps, are reported. Patient 1 was placed in the left lateral position and the left parotid gland was subsequently observed to be diffusely enlarged. With five patients in the prone position, bilateral parotid gland enlargement was observed. Patient 1 complained of parotid swelling. Patient 4 suffered from slight tenderness of the right parotid gland. However, the others had no subjective symptoms. All cases of parotid swelling resolved spontaneously within 7 days. Changes in serum amyl

Idiopathic trigeminal sensory neuropathy.

Idiopathic trigeminal sensory neuropathy is a rare disorder characterized by transient sensory disturbances in the territory of one or more branches of the trigeminal nerve. Acute and chronic forms of the disease have been recognized, but the boundaries between these groups are not clearly defined. Six cases of idiopathic trigeminal sensory neuropathy are reported. All patients presented with sensory abnormalities limited to the territory of one trigeminal nerve, in the absence of other clinical features. The symptoms were on the left side of the face in five cases and on the right side in one

Effect of ultrafiltration on plasma concentrations of atrial natriuretic peptide in haemodialysis patients.

We have investigated the influence of body fluid volume status on plasma levels of immunoreactive atrial natriuretic peptide (irANP) in eight uraemic patients on chronic haemodialysis, including two diabetics with severely impaired reflex control of the heart. IrANP was significantly higher in volume-expanded uraemic patients (36 +/- 16 pg/ml) than in a group of seven age and sex-matched normal subjects (14 +/- 2 pg/ml), and fell consistently, approaching the normal range after the removal of 2.0-4.3 litres of isotonic plasma ultrafiltrate (by isolated ultrafiltration). Plasma levels of the ho

Cataract Extraction in a White-Bellied Caique (Pionites leucogaster): A Case Report.

A four-year-old female white-bellied caique (Pionites leucogaster) was presented with lens opacification and blindness in the right eye (OD) following a history of head trauma. The left eye (OS) was normal. The bird underwent general anesthesia. Intracameral atracurium was administered to induce pupil dilation. Following corneal incision, ophthalmic viscoelastic devices (OVDs) were injected, and an anterior capsulotomy was performed. The lens material was removed using a manual aspiration and irrigation technique with balanced salt solution. Systemic enrofloxacin and meloxicam were prescribed,

Activism during the COVID-19 pandemic: Racial reckoning for Asian and Asian American college students.

OBJECTIVES: The COVID-19 pandemic has intensified discrimination and racist attacks targeting Asian Americans. This challenging context has also created opportunities for individuals to engage in activism and collective action. Despite the significance of this issue, little research has explored how anti-Asian racism throughout COVID-19 has contributed to Asian and Asian American college students' engagement in activism. METHOD: Focus group interviews were conducted to explore how the COVID-19 pandemic shaped the experiences and perceptions of activism among 34 Asian and Asian American colleg

ADGRG6-related disorder: a novel mutation resulting in distal arthrogryposis and a patchy neuropathy.

Arthrogryposis multiplex congenita (AMC) is associated with >150 genes, including ADGRG6, which codes for an adhesion G protein-coupled receptor. Biallelic loss of function variants in ADGRG6 have been linked to lethal congenital contracture syndrome. Here we present an atypical, milder phenotype associated with a novel ADGRG6 variant. A 22-year-old female, born from non-consanguineous parents, presented with non-progressive, distal arthrogryposis of her limbs since birth. Clinical evaluation showed distal contractures of all four limbs, absent right biceps resulting in elbow flexion weakness

Pure Motor Isolated Finger Palsy by Cerebral Infarction: Tips for Diagnosis by Neurological Examination.

There are various causes of unilateral finger palsy. Its potential etiologies include peripheral neuropathy, carpal tunnel syndrome, and nerve root disorder due to myelopathy. In addition to them, similar paralysis has been reported in localized lesions of the cerebrum, classically referred to as pseudoperipheral palsy. In this report, we describe a case of an 80-year-old man who developed sudden clumsiness of the right fingers. Neurological examination showed muscle weakness mainly in the 1st and 2nd fingers (Medical Research Council grade 1-4) and normal reflexes in the extremities. The affe

Navigating Diagnostic Challenges in Acute Coronary Syndrome: A Case of Bezold-Jarisch Reflex and Wellens Pattern.

Acute coronary syndrome (ACS) presents significant diagnostic challenges, particularly in cases with atypical presentations and complex clinical scenarios. Here, we describe the case of a 59-year-old man who presented with presyncope, bradycardia, hypotension, and later syncope, attributed to the Bezold-Jarisch reflex. Electrocardiographic findings suggested both inferior and anterior wall infarction, with dynamic changes in T-wave morphology further complicating the diagnostic process. Despite a type A Wellens' pattern indicating critical stenosis in the proximal left anterior descending (LAD

Static cervico-ocular reflex in healthy humans.

BACKGROUND: Dynamic cervico- (COR) and vestibulo-ocular reflex (VOR) contribute to stabilise visual images in the retina. The gain in dynamic COR is small in healthy individuals but increases in patients with vestibular dysfunction. Conversely, static COR has not been directly observed in healthy individuals. OBJECTIVES: To elucidate the presence of static COR and quantify it in normal individuals in the roll plane. METHODS: Eleven healthy participants were included in the study. Eye torsions were measured using video oculography to evaluate the static COR induced by lateral neck flexion dur

Atypical Presentation of Tangier Disease-Expanding the Clinical Spectrum.

Tangier disease is an autosomal recessive multisystem metabolic disorder with neuromuscular manifestations including peripheral neuropathy such as multifocal mononeuropathy or pseudosyringomyelia patterns. We report a novel phenotype of Tangier disease with predominant anterior horn cell involvement. A 16-year-old adolescent girl born to consanguineous parents had a 1-year history of hip girdle weakness with waddling gait and progressive atrophy of the right leg. She had orange tonsils, prominent lingual tonsils, soft skin, distal joint laxity, diffuse hypotonia with asymmetric wasting of legs

Neurotrophic Keratitis in a Pediatric Patient With Goldenhar Syndrome and Trigeminal Aplasia Successfully Treated by Corneal Neurotization.

Herein, the authors report an unusual case of a 6-year-old boy with right-sided Goldenhar syndrome and trigeminal nerve aplasia who developed neurotrophic keratopathy (NK). Despite the use of therapeutic contact lenses and multiple temporary tarsorrhaphy, NK worsened showing a central corneal scar, neovascularization, and significant stromal thinning, with risk of corneal perforation. Cochet-Bonnet esthesiometry revealed complete corneal anesthesia. To minimize additional corneal complications, the patient underwent indirect corneal neurotization by a sural nerve autograft anastomosed to the c

Atypical anti-NMDA receptor encephalitis associated with varicella zoster virus infection.

The triggering effect of herpes simplex virus infection on the development of anti-N-methyl-D-aspartate receptor (NMDAR) encephalitis is now well established. However, there are very few reports that has linked a varicella zoster virus (VZV) reactivation with anti-NMDAR encephalitis. In this report, we describe a case of a 57-year-old man presented with atypical clinical presentation of anti-NMDAR encephalitis with gait ataxia, complete ophtalmoplegia, and abolished reflexes followed by drowsiness and confusion. Initial diagnosis of Bickerstaff's brainstem encephalitis was suspected. Few days

Multiple subluxations and comminuted fracture of the cervical spine in a sheep.

A 5-month-old, 13.5 kg, female Corriedale sheep was referred to the Veterinary Medicine Teaching Hospital, with a history of traumatic injury of the cervical spine followed by non-ambulatoric tetraparesis that occurred 2 weeks before being admitted to the hospital. At admission, malalignment of the cervical spine with the cranial part of the neck deviating to the right was noted. Neurological examinations identified the absence of postural reactions in both forelimbs, mildly decreased spinal reflexes, and normal reaction to pain perception tests. Radiography revealed malalignment of the cervic

Reversible hepatocerebral degeneration-like syndrome due to portovenous shunts.

Ataxia and tremor are rare manifestations of hepatocerebral degeneration due to portovenous shunts. Ammonia is a neurotoxin that plays a significant role in the pathogenesis of hepatic encephalopathy. A 58-year old male patient was assessed with the complaints of gait disturbance, hand tremor, and impairment of speech. His neurological examination revealed dysarthric speech and ataxic gait. Bilateral kinetic tremor was noted, and deep tendon reflexes of the patient were hyperactive. Serum ammonia level was found to be 156.9 microg/dL. Cranial magnetic resonance (MR) imaging revealed increased

Brainstem mediation of the stapedius muscle reflex in hydranencephaly.

Hydranencephaly is a rare condition in which the cerebral hemispheres are absent at birth and are replaced by membranous sacs in a cerebrospinal fluid-filled cranium. Surviving hydranencephalic patients have a functional brainstem and possible remnants of the cerebral cortex. This case report examines hearing function and the integrity of the brainstem mediated stapedius muscle reflex (SMR) in an adult with hydranencephaly, using middle ear impedance change measures. The brainstem mediated ipsilateral and cross-brainstem contralateral SMRs were elicited in the right ear at normal threshold lev

[A clinical study of Bell's palsy and factors influencing its prognosis].

We encountered 185 patients with Bell's palsy at our hospital between January 2003 and December 2005. Of these patients, 60% visited our department within 3 days of the onset, and 90% within 7 days of the onset; the interval from onset to hospital visit showed no relation with the severity of the paralysis. Complete recovery was obtained in 85.0% of the patients with steroid or steroid + antiviral treatment. Preservation of the stapedius reflex was a statistically significant predictor of good prognosis, with a high positive predictive value (95.5%). Several factors influencing the prognosis w
