Diagnostic imaging and unforeseen associated lesions in astragalo-scaphoid dislocation: a case report.

A case of an isolated astragalo-scaphoid dislocation following a fall from a motorbike is reported. Due to the diagnostic obscurity of the full extent of the injury, computed tomography (CT) with 3D reconstruction was utilized to precisely visualize the articular condition and revealed the presence of small intra-articular bone fragments and calcaneo-cuboid subluxation. Furthermore, the detailed imaging offered valuable information for preoperative planning. Patient has healed without sequelae. In light of the good outcome in this case, we suggest utilizing CT with 3D reconstruction when there

Both Trevor and Ollier disease limited to one upper extremity.

A case is presented of a young boy in whom features of Trevor's dysplasia epiphysealis hemimelica and Ollier's enchondromatosis coexisted in a single extremity, the right upper. As Trevor disease consists of osteochondromas of epiphyses and their equivalents, such as carpal and tarsal bones, it is of interest that exostosis-like centers of the neck of radius and perhaps the proximal third metacarpal are present as well. Advanced maturation of selected centers was most marked at the right scaphoid. The child shows alignment abnormalities as a consequence of the varied lesions, including a varus

[Scapholunate instability. Diagnosis--classification--treatment].

Scapholunate instability is more or less caused by injury. The ligament instability between lunate and scaphoid is not easily to be identified even there is only a little widening of this space between both bones. The instability may be proved by physical examination, by x-ray and by arthroscopy. The scapholunate instability may occur in a different manner. It is divided in three stages. In stage I a none operative treatment is sufficient. For the other stages an operative treatment is necessary: The operative treatment begins with suture of the ligament, continues with ligamentoplasty and end

[Results of surgically treated scaphoid pseudarthrosis using the Matti-Russe method].

For the treatment of scaphoid pseudarthrosis which is recognized sufficiently soon after an accident and where are no arthritic changes, the best method is that described by Matti-Russe; in our series we found healing in 95%. By grafting an autologous cortico-cancellous bone from the ilium we had no failures when we diagnosed and operated on the pseudarthrosis in the first five years after injury. We, therefore, now operate also on fractures with delayed union. The resection of the radial styloid process in combination with the Matti-Russe method has shown no advantages, and we have abandoned

A Rare Case of Carpal Osteomyelitis in a Spinal Cord Injury Patient: A Case Report.

Osteomyelitis of the carpal bones is rare and usually occurs in the setting of penetrating trauma. Here, to our knowledge, we report the first known documented case of carpal osteomyelitis in a spinal cord injury (SCI) patient and discuss the medical management of this patient. A 62-year-old male with a remote history of traumatic SCI at T5 American Spinal Injury Association (ASIA) Impairment Scale (AIS) A and a history of IV polysubstance abuse presented to an acute care hospital for acute non-traumatic right dorsal wrist pain. Initial hand and wrist X-rays were negative for acute findings. A

Management of clinically suspected scaphoid fractures: a survey of current practice in Israel.

BACKGROUND: Fracture of the scaphoid is the most common fracture of a carpal bone. Nevertheless, the diagnosis of SF might be challenging. Plain X-rays that fail to demonstrate a fracture line while clinical findings suggest the existence of such a fracture is not uncommon. Currently there is no consensus in the literature as to how a clinically suspected SF should be diagnosed, immobilized and treated. OBJECTIVES: To assess the current status of diagnosis and treatment of clinically suspected scaphoid fractures in Israeli emergency departments. METHODS: We conducted a telephonic survey amon

Observer variability in measurements of carpal bone angles on lateral wrist radiographs.

Determinations of carpal bone angles are used in the clinical evaluation of carpal malalignment. Eleven frequently referred radiological measures in lateral projection of the wrists in 23 wrists were assessed using different definitions of axes. Interobserver- and intraobserver variations were calculated. The standard deviation of the interobserver variation ranged from 2.60 degrees to 18.15 degrees, and the intraobserver variation from 1.89 degrees to 4.66 degrees depending on the angles measured. The use of three angles for the least observer variability in assessment of carpal alignment is

Anatomical front and back reconstruction sustains radiographic alignment: a comparison to three-ligament tenodesis.

Multiple techniques exist to reconstruct the scapholunate interosseous ligament, though none have demonstrated superiority. This study compares 1-year radiographic outcomes of the three-ligament tenodesis and the anatomical front and back reconstruction. All patients who underwent reconstruction of their scapholunate interosseous ligament at one institution with either anatomical front and back reconstruction or three-ligament tenodesis between 2011 and 2020 were retrospectively reviewed. At 52-week follow-up, anatomical front and back reconstruction maintained a statistically significant impr

Radiographic evaluation of radial flexion osteotomy effect on static scapholunate instability: a preliminary cadaveric study.

INTRODUCTION: The optimal treatment of chronic scapholunate instability has yet to be established. Scapholunate ligament grafts are still far from being the ideal solution. We conducted an experimental study to evaluate whether flexion-opening wedge osteotomy of the distal radius improves misalignment and avoids rotatory subluxation of the scaphoid in a cadaveric model of static scapholunate dissociative instability. MATERIALS AND METHODS: Radiographic studies were performed on 15 cryopreserved specimens after recreating a model of scapholunate instability by division of the scapholunate inte

Utility of preoperative MRI for assessing proximal fragment vascularity in scaphoid nonunion.

AIMS: The benefit of MRI in the preoperative assessment of scaphoid proximal fragment vascularity remains controversial. The purpose of this study is to compare preoperative MRI findings to intraoperative bleeding of the proximal scaphoid. METHODS: A retrospective review of 102 patients who underwent surgery for scaphoid nonunion between January 2000 and December 2020 at a single institution were identified. Inclusion criteria were: isolated scaphoid nonunion; preoperative MRI assessing the proximal fragment vascularity; and operative details of the vascularity of the proximal fragment with t

Late consolidation of the scaphoid. Case report and literature review.

Scaphoid fractures are a common wrist injury accounting for 2-7% of all adult fractures. Nonunion is described in 5-12% of cases leading to osteoarthritis. Several classifications have been developed focused on this pathology and its complication. We present a case of a 28 years old male patient with a scaphoid fracture and nonunion who spontaneously consolidates without treatment. We performed a literature review to recognize this pathology, its common evolution and possible treatment options.

Artificial intelligence vs. radiologist: accuracy of wrist fracture detection on radiographs.

OBJECTIVE: To compare the performances of artificial intelligence (AI) to those of radiologists in wrist fracture detection on radiographs. METHODS: This retrospective study included 637 patients (1917 radiographs) with wrist trauma between January 2017 and December 2019. The AI software used was a deep neuronal network algorithm. Ground truth was established by three senior musculoskeletal radiologists who compared the initial radiology reports (IRR) made by non-specialized radiologists, the results of AI, and the combination of AI and IRR (IR+AI) RESULTS: A total of 318 fractures were repor

Radiographic characterization of acute scaphoid fractures in children under 11 years of age.

BACKGROUND: Delayed diagnosis of scaphoid fractures can lead to long-term morbidity. While radiography is the preferred screening examination, there is a relative paucity of literature that examines fracture visibility in younger children, who have smaller ossification centers, an abundance of unossified cartilage and fractures that preferentially involve the distal scaphoid. OBJECTIVE: To characterize acute scaphoid fractures in younger children on radiographs with observer agreement and with respect to fracture location. MATERIALS AND METHODS: This institutional review board (IRB)-approved

Association of the Interfacet Angle and the Lunate Facet Inclination Angle With Kienböck Disease.

PURPOSE: The etiology of Kienböck disease remains unclear, although mechanical, vascular, and metabolic risk factors have been suggested. We aimed to investigate the association of the angle between the curvatures of the distal radius and the development of Kienböck disease. METHODS: The lunate facet inclination (LFI), scaphoid facet inclination, and interfacet angle (IFA) values were measured using posteroanterior plain radiographs of 82 patients diagnosed with Kienböck disease. The results were compared with normative angular reference values published based on an analysis of 400 wrists of

Age-dependent changes in pediatric scaphoid fracture pattern on radiographs.

OBJECTIVE: To systematically investigate age-dependent changes in scaphoid fracture prevalence and fracture patterns on radiographs in children under 15 years of age. METHODS AND MATERIALS: This retrospective study included children with scaphoid fractures, who underwent radiographic examinations between May 1, 2009, and August 31, 2019. Blinded to outcome, all radiographs were reviewed to determine fracture visibility on initial radiographs; to characterize fracture location (distal corner, distal, mid, and proximal body) and orientation (horizontal oblique, transverse, and vertical oblique)

Intercarpal ligamentoplasty for scapholunate dissociation: comparison of two techniques.

We modified our original surgical technique of scapholunate intercarpal ligamentoplasty for treating chronic scapholunate dissociation. The aim of this study was to compare the outcomes in patients treated by the same surgical team with the original method and the modified method over two different time periods. Nineteen patients with a mean age of 40 years were treated with the original method (mean follow-up of 34 months, range 12-54), and 21 patients with a mean age of 38 years were treated with the modified method (mean follow-up of 27 months, range 13-40). In both groups, we found a signi

[Advances in diagnosis and treatment of acute scaphoid fractures].

OBJECTIVE: To review the advances in diagnosis and treatment of acute scaphoid fractures. METHODS: The characteristic, classification, diagnosis, and treatment of acute scaphoid fractures were reviewed and summarized. RESULTS: As one of the common fracture in hand, scaphoid fractures are generally classified as either undisplaced and stable or displaced and unstable. CT and MRI has best diagnostic specificity and sensitivity respectively. Most undisplaced and stable fractures can be treated successfully by plaster immobilization, whereas the displaced and unstable fractures have great progno
