Biomechanical contributions of posterior deltoid and teres minor in the context of axillary nerve injury: a computational study.

PURPOSE: To determine whether transfer to only the anterior branch of the axillary nerve will restore useful function after axillary nerve injury with persistent posterior deltoid and teres minor paralysis. METHODS: We used a computational musculoskeletal model of the upper limb to determine the relative contributions of posterior deltoid and teres minor to maximum joint moment generated during a simulated static strength assessment and to joint moments during 3 submaximal shoulder movements. Movement simulations were performed with and without simulated posterior deltoid and teres minor para

Characterization of the shoulder net joint moment during manual wheelchair propulsion using four functional axes.

Understanding how individuals distribute mechanical demand imposed on their upper extremity during physically demanding activities provides meaningful insights to preserve function and mitigate detrimental mechanical loading of the shoulder. In this study, we hypothesized that parameterization of the shoulder net joint moment using four functional axes could characterize distribution tendencies about the shoulder during manual wheelchair propulsion and that regardless of demographics, a shoulder flexor dominant NJM distribution would be predominantly used by individuals with paraplegia (n = 13

Injuries of the terminal branches of the infraclavicular brachial plexus: patterns of injury, management and outcome.

We reviewed 101 patients with injuries of the terminal branches of the infraclavicular brachial plexus sustained between 1997 and 2009. Four patterns of injury were identified: 1) anterior glenohumeral dislocation (n = 55), in which the axillary and ulnar nerves were most commonly injured, but the axillary nerve was ruptured in only two patients (3.6%); 2) axillary nerve injury, with or without injury to other nerves, in the absence of dislocation of the shoulder (n = 20): these had a similar pattern of nerve involvement to those with a known dislocation, but the axillary nerve was ruptured in

Sports injuries in team handball. A one-year prospective study of sixteen men's senior teams of a superior nonprofessional level.

One hundred eighty-six players of 16 teams in 2 male team handball senior divisions were observed prospectively for 1 season to study the injury incidence in relation to exposure in games and practices. Ninety-one injuries were recorded. Injury incidence was evaluated at 2.5 injuries per 1000 player-hours, with a significantly higher incidence in game injuries (14.3 injuries per 1000 game-hours) compared with practice injuries (0.6 injuries per 1000 practice-hours). Practice injury incidence was higher in the lower performance level group, and game injury incidence was higher in the high-level

The painful shoulder: part II. Acute and chronic disorders.

Fractures of the humerus, scapula and clavicle usually result from a direct blow or a fall onto an outstretched hand. Most can be treated by immobilization. Dislocation of the humerus, strain or sprain of the acromioclavicular and sternoclavicular joints, and rotator cuff injury often can be managed conservatively. Recurrence is a problem with humerus dislocation, and surgical management may be indicated if conservative treatment fails. Rotator cuff tears are often hard to diagnose because of muscle atrophy that impairs the patient's ability to perform diagnostic maneuvers. Chronic shoulder pr

[Research progress on factors of shoulder stiffness after rotator cuff tear repair].

Rotator cuff tear is a common shoulder injury in daily work and sports activities. Arthroscopic repair is the most widely used method for rotator cuff tears, and the postoperative prognosis is good. However, there are still a series of postoperative complications that affect the therapeutic effect and patient satisfaction, such as postoperative pain, infection, retear, shoulder stiffness, etc. This paper reviews the occurrence and influencing factors of shoulder stiffness after arthroscopic repair of rotator cuff tear, in order to provide reference for the prevention of postoperative shoulder

A bizarre complication of shoulder arthroscopy.

A case is presented of an unusual complication of shoulder arthroscopy, which was caused by incorrect location of the posterior portal, inducing neurapraxia of both the axillary and radial nerves, with significant clinical and functional sequelae. A subsequent open surgical neurolysis was required to restore normal nerve function. This type of lesion has not been reported previously. Level of evidence V.

[Ultrasound diagnosis in shoulder trauma].

In cases of injuries to the shoulder region, ultrasonography is an ideal supplement to clinical and radiologic examination, including dynamic functional evaluation. The advantages of the method in children and adults are mainly the reduction of conventional radiographs and thus a minimum exposure to radiation. Despite the growing experience of clinicians and the development of high-resolution sonographic technology, primary conventional radiology cannot be dispensed with. Moreover, sonographic examination is rather time-consuming. Evaluation of the subacromial space and the glenohumeral joint

The pitcher's mound: a late sequela of posterior type II SLAP lesions.

Three patients with long-standing disabling shoulder pain underwent arthroscopic examination. Two of the 3 had preoperative magnetic resonance imaging scans showing complete rotator cuff tears confirmed at surgery. The third patient was found to have a partial-thickness bursal surface cuff tear. In addition, each patient was found to have a quite prominent posterior superior glenoid osteophyte located beneath an unstable type II SLAP lesion.

Challenges and Opportunities of Early Brachial Plexus Reconstruction in Polytrauma: Case Report and Review of the Literature.

Background: Assessment and treatment of brachial plexus injury in polytrauma patients is often challenging due to concomitant injuries requiring life-saving interventions. Furthermore, the role of immediate nerve exploration in closed postganglionic nerve injuries remains debated. Case presentation: We present the case of a 21-year-old male with an infraclavicular brachial plexus injury, floating shoulder and axillary artery rupture following a motorcycle accident. Early multidisciplinary intervention included vascular repair, bone stabilization and brachial plexus exploration. Nerve reconstru

Clinical findings in C5-C6 and C5-C7 root palsies with brachial plexus traction lesions.

Stretch injuries of the C5-C7 roots of the brachial plexus traditionally have been associated with palsies of shoulder abduction/external rotation, elbow flexion/extension, and wrist, thumb, and finger extension. Based on current myotome maps we hypothesized that, as far as motion is concerned, palsies involving C5-C6 and C5-C7 root injuries should be similar. In 38 patients with upper-type palsies of the brachial plexus, we examined for correlations between clinical findings and root injury level, as documented by CT tomomyeloscan. Contrary to commonly held beliefs, C5-C7 root injuries were n

Complications of epilepsy and a ruptured pyonephrosis: radiology to the rescue in the Brooks murder case.

A woman and her lover were accused of murdering the woman's husband. Five weeks after fracture/dislocations of both shoulders and a central fracture/dislocation of the right hip were diagnosed the woman's husband died of septicaemia consequent on a ruptured infected hydronephrosis. The shoulder and hip injuries and the ruptured hydronephrosis were attributed to a beating with a blunt instrument. A review of the medical records of the deceased, particularly his radiological examinations, led the authors to conclude that his joint injuries and ruptured kidney were not due to the alleged assault.

Superior Capsular Reconstruction of the Shoulder Using the Long Head of the Biceps Tendon: A Systematic Review of Surgical Techniques and Clinical Outcomes.

Background and Objectives: Superior capsular reconstruction (SCR) with the use of a fascia lata autograft or a dermal allograft is an established treatment in treating irreparable rotator cuff (RC) tears. The long head of the biceps tendon (LHBT) has been recently proposed as an alternative graft for SCR. The purpose of this study was to present the surgical techniques and clinical studies utilizing the LHBT for SCR. Material and Methods: Medline, Scopus, and the Cochrane library were searched for relevant studies up to December 2020. The primary outcomes were pain intensity improvement and th

Capsular Injury and Inflammation.

The capsular and ligamentous structures of the glenohumeral joint are important for stability of the shoulder. These structures are best evaluated by MR imaging. Familiarity with normal and abnormal appearance of the capsular structures of the shoulder is important to ensure that important pathology is not overlooked. Injury to the capsular structures can occur in the setting of trauma and most commonly involves the inferior glenohumeral ligament and axillary pouch. Adhesive capsulitis is a common inflammatory condition with characteristic imaging features that should be considered in the abse

Editorial Commentary: Does the Morphology of the Acromion Explain All Rotator Cuff Tears?

Given the high prevalence of rotator cuff tears and their socioeconomic impact, surgeons and researchers have tried to understand their etiology and pathomechanism for almost hundred years. Articles about tendon degeneration with increasing age dominate the literature, and numerous factors contributing to tendon degeneration have been identified. One of them, the lateral extension of the acromion, as quantitated using the acromion index or the critical shoulder angle, has become very popular in the last few years. Other big tendons in the human body, such as the distal biceps tendon, the Achil

Editorial Commentary: Superior Labral Reconstruction for Superior Humeral Migration With Large or Massive Rotator Cuff Tears: Not a Paradigm Shift…Yet!

Large or massive rotator cuff tears are vexing clinical problems. Current treatment algorithms, while effective for many patients, are associated with failure of repair, persistent pain, and weakness including the inability to use the hand above shoulder level for some. Superior humeral migration associated with large or massive rotator cuff tears may lead to arthrosis or shoulder weakness. The biceps tendon has been reported for superior capsular reconstruction, but not often for reconstruction of the superior labrum. However, it is widely recognized the glenoid labrum plays a primary role in

Editorial Commentary: Our Mentors Were Right, New Is Not Always Better: The Posterolateral Shoulder Trans-Rotator Cuff Portal Is Safe for SLAP Repairs.

Shoulder SLAP repair anchors have traditionally been placed through a posterolateral trans-rotator cuff "Port of Wilmington" portal. Alternative anterolateral portals have been proposed to place superior glenoid anchors that seemingly avoid traversing the rotator cuff via the rotator interval but put the suprascapular nerve at risk. A recent cadaveric study demonstrated that the traditional posterolateral portal safely traversed the infraspinatus muscle belly in all specimens, which was in contrast to the anterolateral portal that traversed the supraspinatus tendon in all specimens.
