BACKGROUND: Many approaches to the anterior skull base have been reported. Frequently used are the pterional, the unilateral or bilateral frontobasal, the supraorbital and the frontolateral approach. Recently, endoscopic transnasal approaches have become more popular. The benefits of each approach has to be weighted against its complications and limitations. The aim of this study was to investigate if the anterior interhemispheric approach (AIA) could be a safe and effective alternative approach to tumorous and non-tumorous lesions of the anterior skull base.
METHODS: We screened the operativ

INTRODUCTION: The past two decades have seen a paradigm shift in the surgical treatment of sinonasal malignancies, moving from traditional open approaches to minimally invasive endoscopic techniques. Technological advancements have enabled the use of the endoscopic endonasal transcribriform approach (EETA) for tumors involving the anterior skull base (ASB). This study evaluates the oncologic outcomes of EETA for locally advanced sinonasal malignancies with ASB involvement.
METHODS: We conducted a retrospective cohort study at a tertiary cancer center, including patients treated with EETA for

BACKGROUND: Approach selection paradigm for tuberculum sellae meningiomas (TSMs) is still a point of discussion. The trans-eyebrow supraorbital keyhole approach (TSKA) is a minimally invasive transcranial approach and has been used for the treatment of TSMs during recent years. In this study, we presented a series of 30 cases of TSMs treated via TSKA with a long-term follow-up.
METHOD: Thirty patients with TSMs underwent surgery via TSKA between December 2006 and August 2018. The medical data and follow-up results were retrospectively analyzed.
RESULTS: A total of 5 males and 25 females were

OBJECTIVE: Dysarthria and tongue swelling may be seen with hypoglossal nerve palsy, and on cross-sectional imaging studies, tongue denervation can be misinterpreted as a primary base-of-tongue mass. Understanding radiological patterns of tongue denervation is important to prevent misinterpretation. Close evaluation of the skull base is critical as hypoglossal palsies resulting from pathology here are often overlooked.
METHODS: Neck and brain magnetic resonance imaging studies obtained in 7 adult patients referred to our institution with clinically and/or radiologically suspected tongue base m

OBJECTIVE: Adenoid cystic carcinoma of the salivary glands has a propensity for perineural invasion, which could favor spread along the major cranial nerves, sometimes to the skull base and through the foramina to the brain parenchyma. This study evaluated the relationship between neural spread and relapse in the skull base.
STUDY DESIGN: During surgery, we performed multiple biopsies with extemporaneous examination of the major nerves close to the tumor to guide the surgical resection.
RESULTS: The percentage of actuarial local control at 5 years for patients with a positive named nerve and

Radiotherapy is advocated for many brain and head and neck tumors close to the pituitary gland. Pituitary hormones govern many vital functions but data of standardized monitoring for deficiencies after irradiation are lacking. We prospectively assessed the latency and frequency of hypothalamo-pituitary radiation dose-effects in patients undergoing proton therapy or mixed photon /proton beams for CNS, skull base and head and neck tumors. Radiation oncologists prospectively were asked to monitor endocrine functions based on a standardized protocol complying with international recommendations at

BACKGROUND: Despite advances in skull base surgery, achieving effective surgical management of jugular foramen schwannomas (JFSs) that avoids postoperative cranial nerve (CN) deficits remains a challenge. Subtotal resection followed by radiosurgery (rather than radical resection) is increasingly being viewed as a better treatment strategy. Here, an institutional database was retrospectively analyzed for outcomes after surgical treatment of JFSs to evaluate the optimal strategy for managing JFSs.
METHODS: Twenty-two patients with JFSs were operated on by either radical resection (n = 13) or co

BACKGROUND: Lesions of infratemporal fossa (ITF) represent a group of pathologically heterogeneous diseases. Endoscopic biopsy has been the primary diagnostic method. To facilitate precise treatment, we introduced ultrasound fusion-guided core needle (UFCN) biopsy, which offers accuracy, minimal invasiveness, and real-time capability. This study aimed to evaluate the efficacy of UFCN biopsy for ITF lesions.
METHODS: This retrospective cohort study compared patients with ITF lesions who underwent UFCN biopsy or not. Using logistic Poisson regression analysis, we evaluated the efficacy of UFCN

Mesenchymal tumors of the head and neck can lead to tumor-induced osteopeni, necessitating a biochemical cure to alleviate associated symptoms. We present a case of a 40-year-old man who presented with diffuse pain and wheelchair dependency secondary to a skull base mesenchymal tumor producing tumor-induced osteopeni. The tumor involved the cavernous sinus, infratemporal fossa, and middle cranial fossa. The patient failed the balloon occlusion test. Additionally, the patient consented to the procedure. Cerebral revascularization was performed using a robotically harvested internal thoracic art
