The unravelling of primary myocardial impairment in Marfan syndrome by modern echocardiography.

Use of echocardiography has dramatically changed the way in which patients with Marfan syndrome are diagnosed, monitored and treated. Owing to the lethal nature of aortic complications, priority has been given to the assessment of the aortic root. Echocardiographic studies on patients with Marfan syndrome have also provided data supporting primary myocardial involvement, although this evidence has remained controversial for several years. Use of more sensitive ultrasound techniques has demonstrated mild myocardial impairment in these patients. Biventricular function assessment should be added

[Pulmonary hypertension due to left heart disease].

PULMONARY HYPERTENSION DUE TO LEFT HEART DISEASE. Pulmonary hypertension due to left heart disease (PH-LHD) is the leading cause of PH worldwide and is associated with poor short to long-term survival. Its diagnosis (mean pulmonary artery pressure > 20 mmHg with pulmonary artery wedge pressure > 15 mmHg) requires right heart catheterization at rest and afterwards, isolated post-capillary PH (RVP lower 2 WU) has to be separated from combined pre- and post-capillary PH (RVP ≥ 2 WU). Pulmonary vasodilator therapies are not indicated in PH-LHD (grade III) and the main goal is to optimize left hear

[Diastolic Dysfunction in CKD: Cause or Effect?s].

Left Ventricular Diastolic Dysfunction (LVDD) describes changes in the heart's structure and function, leading to impaired ventricular relaxation and increased filling pressures during the diastolic phase of the cardiac cycle. In the context of isolated LVDD, the left ventricular end-diastolic volume, global contractility, and ejection fraction (EF) are normal, with EF being 50% or higher. However, as filling pressures progressively increase, diastolic dysfunction worsens. LVDD is classified into varying degrees of severity along a continuum that may ultimately lead to heart failure. The diagn

Advances in echocardiography for cardiac amyloidosis and restrictive cardiomyopathies.

PURPOSE OF REVIEW: Cardiac amyloidosis is an infiltrative cardiomyopathy with characteristic echocardiographic findings. Echocardiography is the initial imaging test to identify restrictive cardiomyopathies, and timely diagnosis is important for treatment and prognosis. This review outlines key echocardiographic "red flags," structural and functional findings, and emerging diagnostic tools that enhance early recognition and monitoring of cardiac amyloidosis. RECENT FINDINGS: Classic echocardiographic red flags for cardiac amyloidosis include increased ventricular wall thickness, biatrial enla

[Focused echocardiography in acute medicine].

Focused echocardiography has become increasingly important for bedside diagnostics in acute medicine. Focused echocardiography can detect various cardiac pathologies, such as pericardial effusion, left ventricular dysfunction, right heart strain, relevant heart valve defects and dissection of the ascending aorta. Echocardiographic findings should be interpreted in the clinical context.

[Cardiotoxicity of trastuzumab: mechanisms of development, diagnosis, treatment].

The review discusses mechanisms of trastuzumab cardio- toxicity. There are presented risk factors of development of the left ventricular dysfunction in treatment of a drug, among which the most important are the simultaneous administration of anthracycline antibiotics and the presence of associated cardiovascular diseases. There are discussed the features of cardiac management of patients receiving trastuzumab therapy that focuses on early detection of abnormalities of left ven- tricular function from the normality as well as measures aim- ing at preventing the occurrence of cardiac dysfunctio

[Tachycardia-induced cardiomyopathy].

Cardiomyopathy is a heterogeneous group of diseases of heart muscle accompanied with impaired cardiac function. Tachycardia-induced cardiomyopathy (TIC) is caused by prolonged tachycardia leading to dilatation and systolic dysfunction with clinical manifestation of heart failure. This state is reversible after normalization of heart rate. The diagnosis is usually made retrospectively after normalization of heart rate and recovery of left ventricular function (LVF). More than 100 years after the first documented case (described in 1913 in a young patient with atrial fibrillation and symptoms of

Authors' reply to Finsterer and Aliyev.

BACKGROUND: The fibronectin type 3 and ankyrin repeat domains 1 gene, Fank1, is an ancient, evolutionarily conserved gene present in vertebrates. Short-hairpin RNA (shRNA)-based knockdown transgenic mice have oligospermia caused by an increase in apoptotic germ cells. In this study, we investigated the in vivo function of Fank1. METHODS: In this study, we generated Fank1-knockout mice using the CRISPR/Cas9 system. We then investigated the phenotype and in vivo function of Fank1. Testes and epididymis tissues were analyzed by histological and immunofluorescence staining. Apoptotic cells were a

Relationship between Urinary Norepinephrine, Fibrosis, and Arrhythmias in Chronic Chagas Heart Disease with Preserved or Mildly Reduced Ejection Fraction.

BACKGROUND: In Chronic Chagas Cardiomyopathy (CCC), studies are needed to identify arrhythmogenic risk factors in patients in which moderate to severe ventricular dysfunction is not present. OBJECTIVE: To verify the correlation between frequent ventricular arrhythmias (PVC), left ventricular ejection fraction (LVEF), extension of fibrosis by cardiac magnetic resonance (CMR), and urinary norepinephrine measurement (NOREPI) in CCC with preserved or mildly compromised LVEF. METHODS: The presence of ventricular extrasystoles > 30/h was analyzed on Holter. At CMR, LVEF and quantification of fibro

Relationships between left ventricular geometry and remodeling in dilated cardiomyopathy.

BACKGROUND: Since left ventricular reverse remodeling (LVRR) and sphericity index (SI) are correlated with DCM patients' survival, we attempted to establish the relationship between LVRR, SI and left ventricle (LV) dimensions. METHODS: In 70 DCM patients, we measured EF, LV transverse (sLVd) and longitudinal (lLVd) diameters at hospital admission, then after 3 and 12 months. SI was assessed thus: SI=sLVd/lLVd. RESULTS: LVRR was present in 32 patients (52%). SI measurements were similar in LVRR-present and -absent groups at baseline (0.71 vs. 0.70) and differed after 3 and 12 months (0.61 vs.

Combining tissue Doppler echocardiography and B-type natriuretic peptide in the evaluation of left ventricular filling pressures: review of the literature and clinical recommendations.

Tissue Doppler imaging is an echocardiographic technique that directly measures myocardial velocities. Diastolic tissue Doppler velocities reflect myocardial relaxation, and in combination with conventional Doppler measurements, ratios (transmitral early diastolic velocity/mitral annular early diastolic velocity [E/Ea]) have been developed to noninvasively estimate left ventricular (LV) filling pressure. Consequently, mitral E/Ea can help to establish the presence of clinical congestive heart failure in patients with dyspnea. However, E/Ea has a significant 'gray zone', and is not well validat

Assessment of pit viper associated subclinical myocardial injury by speckle tracking imaging.

In post-snakebite patient care, cardiac function is chiefly monitored through Left Ventricular Ejection Fraction (LVEF) via traditional echocardiography. This approach fails to detect early systolic dysfunction sensitively, which is key for managing envenomation-induced myocardial damage and impacts treatment and prognosis. In a study of 37 snakebite patients, individuals were grouped by severity using the Snakebite Severity Scale (SSS). Correlations between serological markers and two-dimensional speckle tracking echocardiography (2D-STE) strain parameters were analyzed, with ROC curves asses

Association between reduced left ventricular ejection fraction and peritoneal dialysis related peritonitis: a single center retrospective cohort study in Japan.

We present a single-center retrospective analysis of 228 Japanese patients with peritoneal dialysis, in which we examined whether reduced left ventricular ejection fraction (LVEF) is a risk factor for peritonitis development. Time-dependent multivariable-adjusted Cox proportional hazards models revealed that reduced LVEF (LVEF < 50% vs. preserved LVEF ≥ 50%, hazard ratio (HR) 2.10; 95% confidence interval (CI) 1.16-3.82) was associated with peritonitis. Qualitatively, similar associations with reduced LVEF (< 50%) were observed for enteric peritonitis (adjusted HR 7.68; 95% CI 2.51-23.5) but n

Prognostic role of three-dimensional speckle-tracking echocardiography-derived left ventricular global longitudinal strain in cardiac amyloidosis: Insights from the MAGYAR-Path Study.

INTRODUCTION: Systemic amyloidosis is an uncommon disorder in which amyloid fibrils deposit extracellularly. Three-dimensional speckle-tracking echocardiography (3DSTE) is a novel method able to assess left ventricular (LV) global longitudinal strain (GLS). Our aim was to evaluate the prognostic impact of 3DSTE-derived LV-GLS in patients with cardiac amyloidosis (CA). MATERIALS AND METHODS: A total of 35 patients suffering from light-chain (AL) CA or transthyretin (TTR) CA were selected, but 7 patients had to be excluded due to insufficient image quality or were lost for follow-up. With AL-CA

Total 25-Hydroxyvitamin D Is an Independent Marker of Left Ventricular Ejection Fraction in Heart Failure with Reduced and Mildly Reduced Ejection Fraction.

Vitamin D emerged as an important prognostic biomarker in heart failure (HF), with currently highly debated therapeutic implications. Several trials on vitamin D supplementation in HF showed improvements in left ventricular (LV) remodeling and function and health-related quality of life (HRQoL), which did not translate into mid- to long-term beneficial effects regarding physical performance and mortality. We addressed total 25-hydroxyvitamin D (25(OH)D), serum albumin, and uric acid (UA) levels, focusing mainly on vitamin D deficiency, as potential markers of LV systolic dysfunction in HF with
