Trauma-induced coagulopathy.

Uncontrolled haemorrhage is a major preventable cause of death in patients with traumatic injury. Trauma-induced coagulopathy (TIC) describes abnormal coagulation processes that are attributable to trauma. In the early hours of TIC development, hypocoagulability is typically present, resulting in bleeding, whereas later TIC is characterized by a hypercoagulable state associated with venous thromboembolism and multiple organ failure. Several pathophysiological mechanisms underlie TIC; tissue injury and shock synergistically provoke endothelial, immune system, platelet and clotting activation, w

[Epidemiology of post-partum haemorrhage].

OBJECTIVES: To synthesize the available evidence regarding the incidence, causes, risk factors of post-partum haemorrhage (PPH) and the associated maternal morbidity. METHODS: Consultation of the Medline database and of national reports on maternal mortality. RESULTS: PPH is defined as a post-partum blood loss≥500mL, and severe PPH as a post-partum blood loss≥1000mL, whatever the delivery route. In population-based studies, the incidence of PPH is around 5% of deliveries when blood loss is not precisely assessed and around 10% when it is. The incidence of severe PPH is around 2%. Prion protein and developments in its detection.

The theoretical risk of transmission of variant Creutzfeldt-Jakob disease (vCJD) via blood transfusions has led to replacement of UK-derived plasma for fractionation by plasma sourced outwith the UK and the introduction of leucodepletion of donated blood and its components. Prion protein in an abnormal conformation (PrPsc) has been identified as inextricably linked with the infectivity of transmissible spongiform encephalopathies such as vCJD and in this review some of its properties relevant to its detection are considered. In particular its insolubility in nonionic detergents and its partial

Continued Relevance of Initial Temperature Measurement in Trauma Patients.

OBJECTIVES: Hypothermia occurs in 30-50% of severely injured trauma patients and is associated with multiple metabolic derangements and worsened outcomes. However, hypothermia continues to be under-diagnosed which leads to inadequate triage and treatment in trauma patients. Our study set out to determine if hypothermia is an independent predictor of mortality in trauma patients. METHODS: We retrospectively reviewed data of all trauma activation patients over a 5-year period. Data were collected on patient demographics, initial core temperature, Glasgow Coma Scale (GCS) on presentation, and in

[Evidence-based therapy for tendinopathy of the knee joint : Which forms of therapy are scientifically proven?].

Tendinopathy in the region of the knee joint is a common pathological disorder. People active in sports, in particular, have a high probability of suffering from tendinopathy. Despite its high clinical relevance, the level of evidence of therapy options for tendinopathy in the knee region differs greatly. This review gives an overview of current evidence levels for therapy options in tendinopathy of the quadriceps, patellar and pes anserinus insertion tendons as well as of the distal iliotibial tract tendon. The treatment with platelet-rich plasma showed a significantly better outcome when use

Gemcitabine-associated thrombotic thrombocytopenic purpura and hemolytic uremic syndrome.

A patient being treated for metastatic adenocarcinoma of the pancreas presents to the clinic for a routine appointment. A complete blood count reveals hemoglobin of 6.5 g/dl and a platelet count of 30,000 K/mm3 thought to be from the last of many doses of gemcitabine. On assessment, the only complaint was fatigue with no evidence of bleeding or other abnormal physical findings other than pallor. Past medical history includes hypertension managed with three antihypertensive agents. Additional laboratory tests reveal elevated blood urea nitrogen (69 mg/dl), creatinine (2.76 mg/dl), and lactic de

[Adult respiratory distress syndrome after open heart surgery].

Cardiopulmonary bypass used in open heart surgery is responsible for nearly 15% of adult respiratory distress syndrome. The condition has a high mortality rate and still today we do not have a specific therapy for it. The aim of this study was to reveal the factors that are responsible for the adult respiratory distress syndrome developing after open heart operations. A retrospective statistical analysis of the database of 837 consecutive open heart operations was performed. Those patients in whom the adult respiratory distress syndrome has not developed have served as control group. chi 2 pro

Initial management of haemorrhagic war casualties: tactical priorities and innovative approaches in modern and future warfare.

BACKGROUND: Haemorrhage remains the leading cause of preventable death in modern armed conflict, affecting both combatants and civilians. Recent conflicts-particularly the ongoing conflict in Ukraine- have highlighted the increasing complexity of battlefield injuries, characterised by hybrid warfare, disrupted evacuation chains, and delayed access to definitive surgical care. These realities challenge traditional trauma paradigms, such as the "Golden Hour" and demand adaptation of haemorrhage control and resuscitation strategies to austere environment. CONTENT: This narrative review synthesiz

Role of transfused red blood cells for shock and coagulopathy within remote damage control resuscitation.

The philosophy of damage control resuscitation (DCR) and remote damage control resuscitation (RDCR) can be summarized by stating that the goal is to prevent death from hemorrhagic shock by "staying out of trouble instead of getting out of trouble." In other words, it is preferred to arrest the progression of shock, rather than also having to reverse this condition after significant tissue damage and organ injury cascades are established. Moreover, to prevent death from exsanguination, a balanced approach to the treatment of both shock and coagulopathy is required. This was military doctrine du

Temperature Safeguards and Peri-Procedural Strategies for Stem Cell Collection in Cold Agglutinin Disease: A Case Report and Literature Review.

Cold agglutinins can result in red blood cell (RBC) agglutination and lysis at low temperatures. Temperature sensitivity during stem cell mobilization, processing, and infusion poses unique procedural challenges in patients with active cold agglutinins. A 49-year-old man presented at an outside hospital with severe hemolysis and was found to have an anti-I cold agglutinin and a component of warm autoimmune hemolytic anemia, together with a positive COVID and Rhinovirus test. He was treated with RBC transfusions, steroids, and Rituximab. Bone marrow evaluation several months later at our instit

Documents to procure gamma irradiation chamber and license for operation in blood center.

Graft-versus-host disease is a critical posttransplant complication, particularly affecting individuals with compromised immune systems. Patients undergoing bone marrow transplants, neonates receiving intrauterine transfusions, and those recipients of blood from first-degree relatives are vulnerable to this condition. The significance of efficient lymphocyte inactivation has driven the widespread adoption of Gamma Irradiation Chambers (GICs) utilizing radiation sources such as Cesium-137 (Cs-137) or Cobalt-60 (Co-60). However, the effective operation of these chambers is contingent on navigati

Sarcomatoid renal cell carcinoma masquerading as xanthogranulomatous pyelonephritis: a case report.

BACKGROUND: Sarcomatoid renal cell carcinoma (sRCC) is a rare, highly aggressive malignancy that can closely mimic xanthogranulomatous pyelonephritis (XGP) on clinical, laboratory, and radiological grounds. While sRCC masquerading as XGP has been previously reported, existing literature lacks a structured analysis of the specific features that discriminate these entities and a practical framework for clinical decision-making. CASE PRESENTATION: A 65-year-old female presented with a four-month history of anorexia and one week of postprandial abdominal distension. Contrast-enhanced CT demonstra

Naphthalene or Mothball Poisoning Manifesting as Acute Intravascular Hemolysis and Acquired Methemoglobinemia.

Naphthalene is a major component of mothballs. Domestically, people use mothballs as an insect repellent. Its deliberate or accidental ingestion leading to toxicity has rarely been reported in the medical literature, despite its widespread use in Southeast Asia. Naphthalene, or mothball poisoning, is a rare but serious condition that can have detrimental effects on human health. This case report presents the clinical course of a 22-year-old male who ingested six naphthalene balls, resulting in severe symptoms including fever, abdominal pain, vomiting, jaundice, and dark-colored urine. Traumatic laceration of the portal vein.

Despite its relatively protected position, the liver is the most frequently injured solid intra-abdominal organ. 1 Most liver injuries can be managed conservatively, but about 5% to 10% require urgent laparotomy, usually when the mechanism of injury involves a vehicle accident and hemodynamic instability persists, in spite of 40 mL/kg of blood transfusion. 2 , 3 In particular, grades IV and V liver injuries may pose a challenge to the surgeon trying to control hemorrhage, the leading cause of mortality. 4 Traumatic injuries to the portal vein are rare but devastating. The mortality rate for po

Active Exsanguination Behind a Urinary Tract Infection (UTI) Diagnostic Label: A Quadruple-Hit Cardiorenal Collapse Following Ambulatory Urological Care.

We report the case of a 95-year-old man with a previous history of myocardial infarction, pulmonary oedema, and hypertension who developed recurrent collapse while undergoing ambulatory urological follow-up for a six-week history of macroscopic haematuria. Following a recent outpatient review, he had received oral antibiotics for a presumed urinary tract infection (UTI). Throughout this six-week ambulatory period, no full blood count or other blood tests were performed, leaving the extent of blood loss and its haematological consequences clinically occult. The patient subsequently experienced

Freeze dried plasma and fresh red blood cells for civilian prehospital hemorrhagic shock resuscitation.

BACKGROUND: The last decade of military trauma care has emphasized the role of blood products in the resuscitation of hemorrhaging patients. Damage-control resuscitation advocates decreased crystalloid use and reintroduces blood components as primary resuscitative fluids. The systematic use of blood products have been described in military settings, but reports describing the use of freeze dried plasma (FDP) or red blood cells (RBCs) in civilian prehospital care are few. We describe our preliminary results after implementing RBCs and FDP into our Helicopter Emergency Medical Service (HEMS). M

[Treatment of thrombotic microangiopathies].

Thrombotic thrombocytopenic purpura and hemolytic uremic syndrome appear as the same expression of thrombotic microangiopathy (TMA), which is a single pathological entity affecting small blood vessels leading to hemolytic anemia, circulatory changes with renal (hemolytic uremic syndrome, HUS) or nervous (thrombotic thrombocytopenic purpura, TTP) involvement. Because of his low incidence, prospective randomized clinical trials are difficult to conduct and apart from plasma exchanges (PE) which appear superior to plasma infusions (PI), other therapeutic recommendations are based on retrospective

Liver transplantation in Jehovah's Witnesses patients in a center of northeastern Brazil.

CONTEXT: Liver transplantation has been accepted as a therapeutic option for patients with end-stage liver disease and acute liver failure. Currently, Brazil has a well-established public organ transplant program, performing 7,425 solid organs transplants in 2012 alone, among which 1,595 were liver transplants. Jehovah's Witnesses report 7,6 million members worldwide. For religious reasons they refuse transfusion of whole blood or its primary components (red cells, fresh frozen plasma, platelets). OBJECTIVE: This study aims to present the results obtained with Jehovah's Witnesses patients by

Therapeutic Plasma Exchange: Core Curriculum 2023.

From producing individual blood components for transfusion to the removal of pathogenic substances, apheresis is a cornerstone of modern medical therapies. The use of therapeutic plasma exchange (TPE), in which plasma and its soluble constituents are removed from the body in exchange for a replacement fluid, can be organ- and life-saving in many diseases. Given the notable similarities between TPE and hemodialysis, the nephrologist is often responsible for managing TPE. As such, one must be familiar with the technologies, approach to therapy, indications for use, and complications. TPE uses ce
