Case report: MRI of the brain in metronidazole toxicity.

Metronidazole is a commonly used antimicrobial drug. When used excessively, it can cause encephalopathy. We report the MRI findings in one such case. A 43-year-old male patient was treated with metronidazole for 2 months, for an amebic liver abscess and presented with neurological signs and symptoms. MRI of the brain showed findings consistent with metronidazole toxicity.

Intracerebral sepsis due to intestinal perforation by ventriculo-peritoneal shunts: two cases.

Two cases of ventriculo-peritoneal (V-P) shunt infection attributable to intestinal perforation are reported. One patient developed a brain abscess, the other ventriculitis. Microbiology consisted of faecal flora and the peritoneal catheter was found to be faecally stained in both cases. There were no abdominal symptoms or signs. It is likely that infection developed via the ascending route.

Babesiosis concurrent with multiple abscesses from Staphylococcus aureus infection: A case report.

BACKGROUND: Babesiosis is a tick-borne illness. These patients may have signs of a systemic inflammatory response, but abscess formation is unusual. Multiple abscesses in a patient with confirmed babesiosis is very rare, so concurrent infection by another pathogen should be considered. CASE PRESENTATION: We report a 42-year-old male patient who had fever, chills, joint pain, abdominal pain, and altered mental status after a possible tick bite on his right foot while fishing in a river. The laboratory tests, including a blood smear, suggested babesiosis. Imaging studies showed multiple brain a

Scedosporium cerebral abscesses after extra-corporeal membrane oxygenation.

BACKGROUND: Scedosporial infections are usually encountered in the immunocompromised patients. However, they are now emerging in the immunoc … n appropriate antifungal agent, the patient did not survive this aggressive infection. CONCLUSION: Brain abscesses with Scedosporium apiospermum present a therapeutic challenge. High clinical suspicion leading to early appropriate antifungal therapy and combined surgical interventions might improve the prognosis.

Isolation of Clostridium in human infections: evaluation of 114 cases.

One hundred fifty-two strains of Clostridium were isolated from 144 patients over a 14-month-peroid. These included 23 recognized species and 23 strains that were unclassified. Soft tissues or abscesses yielded 84 strains of Clostridium. Intraabdominal sites predominated, but clostridia were recovered from empyema, carcinoma, frostbite with gas gangrene, muscle abscess, aortic graft, and brain abscess. Blood cultures yielded 65 strains of Clostridium from 49 patients, representing 0.3% of 16,314 blood cultures (or 2.6% of 2,168 positive cultures). Clostridium perfringens was most common in blo…

[Phinocerebral mucormycosis--report of a case].

Rhinocerebral mucormycosis is a rare and fatal deep fungus infection occurring in debilitated patients. Its reports have been recently increased because of the increase of such conditions as uncontrolled diabetics, leukemia, and cancer. A 60-year-old male suffering from continuous headache and fever was diagnosed as meningitis, and repeated culture of cerebrospinal fluid was negative. Antibiotic therapy was ineffective. CT scan revealed brain abscess in the right frontal lobe and bone defect over the right posterior ethmoidal sinuses. Both maxillary and ethmoidal sinuses were operated on 38 ye…

Cross-reactivity of Nocardia spp. in the fungal (1-3)-β-d-glucan assay performed on cerebral spinal fluid.

Cerebral spinal fluid from a patient affected by a brain abscess caused by Nocardia abscessus gave a positive result for (1-3)-β-d-glucan (BG) assay, in absence of any fungal infection. This study aimed to assess whether Nocardia spp. show cross-reactivity with BG assay. All Nocardia spp. analyzed provided positive reactions.

Pseudallescheria boydii infection of the central nervous system in a cardiac transplant recipient.

We have described a cardiac transplant recipient with fatal Pseudallescheria boydii infection of the central nervous system. Unusual features included meningitis without brain abscess and noncommunicating hydrocephalus due to exuberant growth of the organism in the ventricular fluid. Long-term amphotericin B administration may have played a role in the development of this infection.

Respiratory epithelial cells masquerading as agents of primary amebic meningoencephalitis: distinguishing features.

CSF does not normally contain motile cells within flagella or cilia. When motile cells are seen in the CSF, they may be incorrectly assumed to be amebae. We studied a patient with a traumatic mucocele, in whom motile ciliated respiratory epithelial cells were seen in the CSF and within a frontal lobe brain abscess.

Seemingly inconsequential yet catastrophic: the importance of wound washout with minute depressed skull fracture from a dog bite.

Depressed skull fractures from dog bites are common pediatric head injuries which are contaminated with native skin and canine oral flora. Outcomes can potentially be catastrophic. Thus, these injuries require proper initial management to prevent future complications. We present an 18-month-old female who was bitten by a Great Dane dog and resulted in a small left temporal depressed skull fracture with an underlying brain contusion. This was initially treated conservatively with antibiotics and bedside irrigation. Five weeks later, she developed a large multiloculated abscess with mass effect,

Third-generation cephalosporins in the treatment of rare infections.

This article reviews the published clinical experience of the use of the third-generation cephalosporins in the treatment of rare infections. Rare infections are defined as those caused by unusual pathogens or multi-resistant organisms as well as those occurring in unusual or pharmacologically protected body sites. Examples of such infections are uncommon causes of meningitis and ventriculitis, brain abscess, rare causes of bacterial endocarditis, metastatic Salmonella infections, spontaneous bacterial peritonitis and liver abscess, late complications of Lyme borreliosis, uncommon Pseudomonas …

De novo progressive dural arteriovenous fistula with putamen hemorrhage associated with long-term Down syndrome: A case report and literature review.

Dural arteriovenous fistula (DAVF) is considered an acquired change in blood flow related to factors such as craniotomy, trauma, and infection. However, several factors related to its development remain unknown. Here, we present a case of a 48-year-old man with Down syndrome and Eisenmenger syndrome. He had a history of craniotomy for multiple brain abscesses, followed by the occurrence of a de novo straight sinus (StS) DAVF within the last 2 years. The patient presented with right putamen hemorrhage due to venous congestion by a StS DAVF. The shunt flow was occluded by transarterial embolizat…

Central nervous system invasion by community-acquired meticillin-resistant Staphylococcus aureus.

We report a case of community-acquired meticillin-resistant Staphylococcus aureus (CA-MRSA) bacteraemia with cavernous sinus thrombosis, meningitis and brain abscess in a previously healthy American, who was employed in Belgium. We consecutively reviewed all published cases of CA-MRSA with central nervous system (CNS) involvement. A total of 12 similar cases were found, of which 11 were published in the last 4 years. Predominantly, young previously healthy subjects were affected (median age 28 years). The cases involved brain abscesses (5/12), disseminated disease (4/12), cavernous sinus throm…

Cerebral aspergillosis in immunologically competent patients.

Aspergillosis of the central nervous system is a rare disease, especially if the patient's immune system is not compromised. The authors report three cases of cerebral aspergillosis in the immunocompetent state: a rhinocerebral form in a diabetic patient, a direct extension from chronic Aspergillus otitis media, and a postoperative Aspergillus brain abscess after brain tumor surgery. In spite of the poor prognosis of cerebral aspergillosis, two of the patients survived. The pathogenesis, predisposing factors, radiologic findings including magnetic resonance image, and the outcome are presented…

Nocardia asteroides cerebral abscess in immunocompetent hosts: report of three cases and review of surgical recommendations.

BACKGROUND: Nocardia asteroides cerebral abscesses are rare intracranial lesions. They account for only 2% of brain abscesses. Existing literature takes the form of anecdotal reports, small case series, and retrospective studies. An optimal treatment approach has not been established. However, there is evidence that the size of the lesion and clinical and immune status of the patient are relevant to surgical decision making. METHODS: Three recent cases are presented and the existing literature is reviewed. RESULTS: The outcome in all three cases was satisfactory. Although the currently recom…
