[Cranial sonography for newborn screening: a 10-year retrospective study in 11,887 newborns].

We retrospectively analyzed the results of a sonographic cranial screening study, performed between 1985 and 1994 to determine the incidence … PIVH and delivery-modes; however, we noted a higher risk of PIVH Grade IV in preterm newborns with breech presentation for vaginal delivery versus caesarean section (38.5 % versus 7.4 %, p = 0.005). The incidence of PIVH over this 10 year time period did not increase despite an increasing number of preterm newborns over time. In conclusion, this study, which represe …

Vaginal birth after Caesarean section: review of antenatal predictors of success.

OBJECTIVE: To determine antenatal factors that may predict successful vaginal birth after Caesarean (VBAC). DATA SOURCES: The MEDLINE datab … easily calculated. RESULTS: A nonrecurrent indication for previous Caesarean section (CS), such as breech presentation or fetal distress, is associated with a much higher successful VBAC rate than recurrent indications, such as cephalopelvic disproportion (CPD). Even with a history of CPD, two-thirds of women will have successful VBAC, though rates decrease with inc …

Forty weeks and beyond: pregnancy outcomes by week of gestation.

OBJECTIVE: To assess pregnancy outcomes at 40, 41, and 42 weeks' gestation when labor induction is done routinely at 42 but not 41 weeks. METHODS: We reviewed all singleton pregnancies delivered at 40 or more weeks' gestation between 1988 and 1998 at Parkland Memorial Hospital, Dallas, Texas. We excluded women with hypertension, prior cesarean, diabetes, malformations, breech presentation, and placenta previa. Labor characteristics and neonatal outcomes of pregnancies at 41 and 42 weeks' gestation were compared with pregnancies that ended at 40 weeks. Women with certain dating criteria had in…

Preeclampsia in multiple pregnancy.

BACKGROUND: Preeclampsia is one of the most common pregnancy-induced complications, and results in a large number of maternal deaths. How pr … e pregnancies, the incidence of preeclampsia was 12.2% compared with 6.2% (p < 0.05) in multiparas. Breech presentation was the most common indication for Cesarean section in all groups. The preeclamptic group had a significantly higher rate of Cesarean section compared with the control group (p < 0.006), with prolonged labor and fetal distress were the two main reas …

Late diagnosis dislocation of the hip joint in children.

Dating from 1953, the hips of newborn have been examined routinely all over Sweden. In 1963 more than 99 per cent of all newborns (about 110 … s during the first weeks of life, especially newborns predisposed to dislocation owing to heredity, breech presentation or different kinds of malformations.--that it should be borne in mind that complete luxation, though rare, may exist already in the neonatal period,--that one should not forget to examine the hips of newborns who, because of prematurity, asphyxia et …

Infantile idiopathic scoliosis. Causative factors, particularly in the first six months of life.

An attempt has been made to determine the aetiological factors in infantile idiopathic scoliosis from a clinical, genetic and epidemiologica … general population frequency for the Edinburgh area. Other positive findings included an excess of breech presentations and of premature, low birthweight males, and a preponderance of curves developing in the winter months. Infants with progressive scoliosis tended to have older mothers and to come from poorer families. Only three children all with resolving scolios …

Anomalous Flexor Carpi Radialis Tendons Giving Rise to a Novel Surgical Technique in the Treatment of Thumb Carpometacarpal Joint Arthritis: A Case Report.

INTRODUCTION: Giant cell tumor of bone (GCTB) is a rare yet locally aggressive neoplasm primarily affecting young adults. Its hallmark features include multinucleated osteoclast-type giant cells and mononuclear tumor cells. Treatment with denosumab, an anti-RANK ligand antibody, has shown efficacy, but it alters histomorphology, posing diagnostic challenges. Co-occurrence with achondroplasia, though rare, warrants consideration in bone lesion evaluations. CASE REPORT: A 30-year-old male with achondroplasia presented with a proximal femur GCT, a rare association. Neoadjuvant denosumab was admi

Developmental dysplasia of the hip: an update on diagnosis and management from birth to 6 months.

PURPOSE OF REVIEW: Our understanding of the epidemiology, diagnosis, and management of developmental dysplasia of the hip (DDH) is evolving. This review focuses on the most up-to-date literature on DDH in patients from birth to six months of age. RECENT FINDINGS: Well known risk factors for DDH include family history, female sex, and breech positioning. Recent evidence suggests higher birth weight is a risk, whereas prematurity may be protective. Screening includes physical examination of all infant hips and imaging when abnormal findings or risk factors are present. Treatment in the first si

Trapezoidal fractures: Overview and introduction of a novel diagnostic classification system.

INTRODUCTION: Trapezoidal fractures account for only 0.4% of all carpal bone fractures. Owing to their rarity, there is paucity in the literature regarding the clinical findings and treatment. This paper aims to summarize the current understanding of trapezoidal fractures and present a novel classification algorithm. METHODS: A diagnostic classification algorithm was created based on the known blood supply and ligamentous attachment of the trapezoid. The proposed treatment algorithm was then applied to trapezoidal fractures in the literature to validate the algorithm and determine whether pat

Vesicouterine fistula in pregnancy: a case report.

BACKGROUND: Vesicouterine fistula is a rare complication of Caesarean delivery. This is the third known report of vesicouterine fistula diagnosed during pregnancy. CASE: Linda (pseudonym), a 28-year-old woman in her second pregnancy, having had a Caesarean delivery in her first pregnancy, was admitted to the delivery unit with possible preterm ruptured membranes at 23 weeks' gestation. She also complained of a fluid-filled sac bulging from her introitus during her admission assessment. Diagnosis of premature rupture of membranes was confirmed by a positive nitrazine paper test and appearance

Meeting the challenge of the rising cesarean birth rate.

The rising cesarean birth rate is a matter of national concern, and the goal of clinical obstetricians is to contain this trend while mainta … indications and strategies for the indicated induction of labor, alternatives in the management of breech presentations and twin deliveries, and identification of patients at risk for genital herpes. The total cesarean birth rate increased to a level of 19.3% in this community in 1986, largely accounted for by the increase in the primary rate to 13.7%. Repeat cesare …

Neonatal morbidity and mortality by mode of delivery in very preterm neonates.

BACKGROUND: The safest mode of delivery to use for very preterm infants is an ongoing topic of debate. There are many reasons to perform a cesarean delivery in cases of extremely preterm and very preterm infants, including indication for labor, fetal distress, maternal indications, and malpresentation. OBJECTIVE: This study aimed to determine whether cesarean delivery is associated with a considerable improvement in neonatal morbidity. STUDY DESIGN: This study is a retrospective cohort study of all singleton pregnancies, delivered from 22 to 29 weeks' gestation between 2010 and 2015, admitte

The Impact of Uterus Didelphys on Fertility and Pregnancy.

Uterus didelphys occurs as a result of abnormal fusion of the paramesonephric ducts and is characterized by complete duplication of uterine horns, cervix, and very often also the vagina or presence of longitudinal vaginal septum. Most women with a uterus didelphys are asymptomatic; some cases may coincide with dyspareunia or dysmenorrhea. The anomaly is associated with a higher risk of miscarriage, preterm labor, breech delivery, and decreased live births. We present the case of a 26-year-old woman (primigravida) who was known to have uterus didelphys. The diagnosis was made when the patient w

Intrapartum asphyxia in term and post term infants.

Many attempts have been made to identify infants at risk of suffering asphyxial brain damage. In a retrospective review of records at the Rotunda Hospital over a five year period all infants who died or suffered seizures, presumed secondary to asphyxia, were compared with the general hospital population. Out of 28,655 deliveries reviewed, there were 13 deaths in infants at or after term associated with perinatal asphyxia, and 32 surviving infants had asphyxial seizures. Seizures were regarded as asphyxial in origin if they occurred in the first forty eight hours of life and were associated wit

Some etiologic factors in Perthes' disease.

The cause of Perthes' disease remains unknown but the nature of the problem and area of research have now become more clearly defined. Genetic and environmental aspects are here reviewed, with the conclusion that there is no good evidence for genetic factors in the great majority of patients. Retarded growth and delayed skeletal maturation have been shown to be already present at the time of diagnosis of the disease. These children are more likely to have been breech births, born late in the family of older-than-average parents. A high proportion of the children come from low income homes. It
