Two-Part Intertrochanteric Femur Fractures with Bisection of the Lesser Trochanter: An Irreducible Fracture Pattern.

OBJECTIVE: To introduce the clinical features of and surgical techniques for a rare type of irreducible pertrochanteric femur fracture pattern with bisection of the lesser trochanter. METHODS: From January 2017 to December 2019, 357 patients with per/intertrochanteric femur fractures underwent surgery by closed reduction and internal fixation, of whom 12 patients were identified with rare preoperative imaging features, the lesser trochanter was almost equally bisected. The main fracture pathoanatomy of these cases included: The anterior fracture line passed along the intertrochanteric line to

The incidence of rotational displacement following distal radial fractures in adults: a biomechanical-clinical study.

The aims of this study were to establish the radiographic features of distal radial fracture malrotation on bone models and then assess the prevalence of distal radial malalignment in a series of extra-articular fractures in adults. We cut distal radial bone models and simulated pronation and supination malrotation in different positions of dorsal angulation. We also reviewed 160 displaced distal radial fractures in 158 adults assessing any malrotation immediately following fracture and after closed reduction. Malrotation was more obvious on lateral than posteroanterior radiographic images. Fo

Trimalleolar fractures of the ankle. An end result study.

Fifty-one trimalleolar ankle fractures in 50 patients were evaluated subjectively, objectively, and roentgenographically at an average of 42 months following injury. Anatomic reduction of the posterior marginal fragment was achieved in 6 of 7 ankles treated by open reduction and internal fixation of the fragment. Anatomic reduction of the posterior marginal fragment was achieved in three of 44 ankles treated by closed reduction and in none of the eight with large fragments. End results rated as fair or poor occurred more frequently among those patients with large posterior lip fragments and th

Outcome of Percutaneous Fixation of Calcaneal Fractures: A Prospective Analysis in an Indian Population.

Displaced intra-articular calcaneal fractures can be difficult to treat. Open surgical techniques are associated with wound complications, whereas nonoperative management leads to arthrosis. In the present study, 23 displaced intra-articular calcaneal fractures in 19 patients were treated with closed reduction and percutaneous Kirschner wire fixation. Sanders and Essex-Lopresti classification systems were used. We studied anatomical (Gissane and Bohler angles and width of calcaneus) and functional (Maryland Foot Score and American Orthopaedic Foot and Ankle Society Ankle-Hindfoot Score) outcom

Alterations in the intrauterine environment by glucocorticoids modifies the developmental programme of the auditory system.

BACKGROUND: Mandibular fractures constitute 57% of all maxillofacial trauma cases, particularly in developing countries such as India. The treatment of mandibular fractures involves either open reduction and internal fixation (ORIF), or closed reduction which is intermaxillary fixation (IMF). These fractures diminish the mandible's load-bearing capacity and compromise the functional ability of the patient. Masticatory efficiency, which measures the effectiveness of chewing, is a key indicator of the functionality of a remodeled mandible. AIM: This study evaluates the masticatory efficiency an

Irreducible lateral patellar dislocation: the importance of impaction fracture recognition.

Irreducible lateral patellar dislocation may occur in the older patient with a previous history of patellofemoral arthritis. The only subtle finding on physical examination to suggest this diagnosis will be positioning of the knee in less flexion than a typical lateral patellar dislocation, anterolateral position of the patella and internal rotation of the patella from the coronal plane. That is, the patella is dislocated laterally but the lateral border comes to lie in a position of variable degrees of anterior displacement relative to the medial patellar border. Plain x-rays may reveal the r

Ankle fractures in children.

Computed tomography is useful for preoperative planning and postreduction assessment for intra-articular pediatric ankle fractures. Nondisplaced pediatric ankle fractures can be effectively managed with cast immobilization and close radiographic follow-up evaluation. Physeal ankle injuries in younger children with considerable growth remaining should be followed closely for at least one year after injury as growth arrest may result in substantial angular deformity. Open reduction and internal fixation should be strongly considered when an articular step-off of <2 mm cannot be maintained by clo

Radiomorphometric analysis of isolated zygomatic arch fractures: A comparison of classifications and reduction outcomes.

PURPOSE: Although different proposals have been made to categorize isolated zygomatic arch fractures (ZAF), an investigation about fracture type and clinical outcome has not been published. In this study, we analyzed the geometric fracture morphology in isolated ZAF and provide a survey of reduction outcomes in accordance with 4 independent classifications. MATERIALS AND METHODS: Geometric measurements were performed in radiologic images of 94 patients with isolated ZAF, which were consecutively treated by closed reduction over transbuccal approach. Fracture types were allocated to the classi

Dynamic Hip Screw Fixation of a Proximal Femur Fracture in a Patient With Ipsilateral Above-Knee and Contralateral Hindquarter Amputations.

Intertrochanteric hip fractures are usually managed with dynamic hip screw (DHS) fixation, but standard positioning and traction are difficult in patients with major lower-limb amputations. We describe a 77-year-old woman with a right above-knee amputation and no contralateral limb who sustained an intertrochanteric fracture after a wheelchair fall. The main challenge was achieving axial traction and rotational control on a very short femoral stump without damaging it or compromising imaging. We achieved closed reduction by inserting a distal femoral Steinmann pin and attaching a stirrup to ap

The Primary Cost Drivers of Outpatient Distal Radius Fracture Fixation: A Cost-Minimalization Analysis of 15,379 Cases.

Background  Distal radius fractures are the most common fracture of the upper extremity. While some distal radius fractures can be managed with closed reduction and immobilization, operative treatment is the standard of care, with open reduction internal fixation (ORIF) as a predominant operative method. Questions/Purpose  To investigate how patient and surgical characteristics affect the overall costs of internal fixation of distal radius fractures in adults. Patients and Methods  The 2014 State Ambulatory Surgery and Services Databases for six states were used to identify cases and surgical

Factors affecting remanipulation in paediatric forearm fractures.

Children's forearm fractures treated in plaster after closed reduction can redisplace and require remanipulation. A retrospective review of 164 consecutive manipulations of fractures of radius and ulna in theatre over a 3-year period was carried out. After various exclusions, 114 patients entered the study. The overall rate of remanipulation was 14.04% (16 children). Presence of preoperative translation deformity predisposed to loss of position in plaster (P<0.0001). Residual deformity on intraoperative films also had a strong adverse influence on the outcome (P = 0.001). Usage of below-the-el

Operative strategy of acetabular fractures.

Anatomic structure of acetabular fractures are complex and operative exposure and fixation are extremely difficult. For those obviously displaced acetabular fractures, close reduction is doomed to cause deformed healing. Open reduction with internal fixation (ORIF) not only results in anatomic reduction, but also brings complications. No matter which method will be adopted, traumatic arthritis or avascular necrosis of femoral head might occur. In order to treat acetabular fractures more effectively, orthopedic surgeons should be required to fully master the acetabular anatomy, biomechanics, cl

Novel management of distal tibial and fibular fractures with Acumed fibular nail and minimally invasive plating osteosynthesis technique: A case report.

RATIONALE: Anatomical characteristics, such as subcutaneous position and minimal muscle cover, contribute to the complexity of fractures of the distal third of the tibia and fibula. Severe damage to soft tissue and instability ensure high risk of delayed bone union and wound complications such as nonunion, infection, and necrosis. PATIENT CONCERNS: This case report discusses management in a 54-year-old woman who sustained fractures of the distal third of the left tibia and fibula, with damage to overlying soft tissue (swelling and blisters). Plating is accepted as the first choice for this ty

Intrafocal pinning for distal radius metaphyseal fractures in children.

The purpose of this retrospective case control study was to evaluate the results of intrafocal pinning for distal radius metaphyseal fractures in children and to compare these results with conventional pinning. Data were collected from medical records and radiographs from patients who underwent closed reduction and percutaneous pinning for distal radius fracture in a Level I trauma center at the authors' institution between 2008 and 2010. Inclusion criteria included a dorsally angulated metaphyseal fracture without physeal involvement, an open distal radius physis, and a follow-up to radiograp

Fractures of mandible: is spontaneous healing possible? Why? When?

Closed and open reduction of fractures of the mandible constitutes an endless debate, and it is interesting that modern times have not brought a conclusion to this controversy. From 1995 to 1997 in the Plastic Surgery Department of Argerich Hospital in Buenos Aires, Argentina, 23 patients were evaluated with different fractures of the mandible in which neither closed nor open reduction was performed. Seventeen patients (74%) presented associated fractures in different areas of the mandible and 6 (26%) only had one fracture; 2 of them (9%) were edentulous. Three (13%) of the 23 patients had mor
