Predicting treatment failure in regular care Internet-Delivered Cognitive Behavior Therapy for depression and anxiety using only weekly symptom measures.

OBJECTIVE: Therapist guided Internet-Delivered Cognitive Behavior Therapy (ICBT) is effective, but as in traditional CBT, not all patients improve, and clinicians generally fail to identify them early enough. We predict treatment failure in 12-week regular care ICBT for Depression, Panic disorder and Social anxiety disorder, using only patients' weekly symptom ratings to identify when the accuracy of predictions exceed 2 benchmarks: (a) chance, and (b) empirically derived clinician preferences for actionable predictions. METHOD: Screening, pretreatment and weekly symptom ratings from 4310 reg

PTSD treatment response and quality of life in women with childhood trauma histories.

OBJECTIVE: Childhood trauma is associated with greater psychological problems and poorer quality of life (QOL). This study evaluates the effect of multiple types of childhood trauma on posttraumatic stress disorder (PTSD) and QOL following cognitive processing therapy (CPT). Understanding how the breadth of traumatic experiences in childhood can influence psychosocial treatment outcomes may help to identify the unique needs of this population. METHOD: This secondary data analysis of a randomized controlled trial delivering CPT to civilian (n = 105) and veteran women (n = 21) with PTSD used li

The role of neurocognitive tests in the assessment of adult attention-deficit/hyperactivity disorder.

Despite widespread recognition that attention-deficit/hyperactivity disorder (ADHD) is a lifelong neurodevelopmental disorder, optimal methods of diagnosis among adults remain elusive. Substantial overlap between ADHD symptoms and cognitive symptoms of other mental health conditions, such as depression and anxiety, and concerns about validity in symptom reporting have made the use of neuropsychological tests in ADHD diagnostic assessment appealing. However, past work exploring the potential diagnostic utility of neuropsychological tests among adults has often relied on a relatively small subse

A bifactor model of obsessive beliefs: Specificity in the prediction of obsessive-compulsive disorder symptoms.

Although current cognitive-behavioral models suggest that maladaptive "obsessive beliefs" about threat, responsibility, uncertainty, perfectionism, importance, and control of thoughts contributes to the development of obsessive-compulsive disorder (OCD), the structure of such beliefs is unclear. The current study evaluated a bifactor model of obsessive beliefs, as assessed by the Obsessive Beliefs Questionnaire (OBQ-44), and its association with OCD symptoms in clinical (Study 1; N = 264) and nonclinical (Study 2; N = 437) samples. The goals were as follows: (a) to evaluate whether obsessive b

Competing indirect effects in a comparative psychotherapy trial for generalized anxiety disorder.

In a randomized trial for generalized anxiety disorder, cognitive-behavioral therapy (CBT) and CBT integrated with motivational interviewing (MI) promoted comparable worry reduction at posttreatment, whereas MI-CBT outperformed CBT over 12-month follow-up (Westra, Constantino, & Antony, 2016). Secondary analyses revealed competing mediators of the long-term treatment effect: MI-CBT related to lower patient resistance to the treatment, which promoted lower follow-up worry, whereas CBT related to greater increases in patient friendly submissiveness (FS), or compliance, which also promoted lower

Highly active antiretroviral therapy as the sole treatment for AIDS-related primary central nervous system lymphoma: a case report with implications for treatment.

A 40-year-old male presented to medical attention with Pneumocystis jiroveci pneumonia and HIV infection. His CD4+ count was 18 cells per microliter and his HIV viral load (VL) was more than 400,000 copies milliliter. After 3 weeks of antibiotic therapy, he continued to have global cognitive deficits. A brain imaging study showed a right temporal mass, which on biopsy proved to be primary central nervous system lymphoma (PCNSL). He began highly active antiretroviral therapy (HAART) but declined palliative whole-brain radiotherapy (WBRT). Four months later, his CD4+ count had improved to 153 ce

Neurodegeneration from mitochondrial insufficiency: nutrients, stem cells, growth factors, and prospects for brain rebuilding using integrative management.

Degenerative brain disorders (neurodegeneration) can be frustrating for both conventional and alternative practitioners. A more comprehensive, integrative approach is urgently needed. One emerging focus for intervention is brain energetics. Specifically, mitochondrial insufficiency contributes to the etiopathology of many such disorders. Electron leakages inherent to mitochondrial energetics generate reactive oxygen free radical species that may place the ultimate limit on lifespan. Exogenous toxins, such as mercury and other environmental contaminants, exacerbate mitochondrial electron leakag

(Re-)activation of neurons in aging and dementia: lessons from the hypothalamus.

Our hypothesis is that there is 'wear and tear' in the brain, which is the basis of the process of aging, but that stimulation of brain function may slow down brain aging and diminish the risk for neurodegenerative diseases such as Alzheimer's disease (AD), possibly by activating repair mechanisms. Evidence supporting this hypothesis is presented in this review. During normal aging and in AD, cell loss is not as prominent a phenomenon as is often presumed. In fact, unaltered neuronal numbers have been reported in many brain areas in AD, e.g. in the nucleus basalis of Meynert (NBM) where the nu

The effect of neuroticism on mobile phone addiction among undergraduate nursing students: a moderated mediation model.

BACKGROUND: Mobile phone addiction (MPA) has numerous deleterious effects on college students, including depression, anxiety, cognitive impairments, and sleep disorders. Undergraduate nursing students play a crucial role as a significant reserve workforce in clinical nursing practice, and their compromised mental health status significantly influences the quality of nursing work and nurse-patient relationships in the future. OBJECTIVES: This study aims to investigate the associations between neuroticism and MPA among undergraduate nursing students, the mediating effect of self-control and the

Neurobehavioral outcomes of infants exposed to MDMA (Ecstasy) and other recreational drugs during pregnancy.

3,4-methylenedioxymethamphetamine (MDMA) or "Ecstasy" is one of the most widely used illicit recreational drugs among young adults. MDMA is an indirect monoaminergic agonist and reuptake inhibitor that primarily affects the serotonin system. Preclinical studies in animals have found prenatal exposure related to neonatal tremors and long-term learning and memory impairments. To date, there are no prospective studies of the sequelae of prenatal exposure to MDMA in humans, despite concerns about its potential for harmful effects to the fetus. The present study is the first to prospectively identi

Frailty for Perioperative Clinicians: A Narrative Review.

Frailty is a multidimensional syndrome characterized by decreased reserve and diminished resistance to stressors. People with frailty are vulnerable to stressors, and exposure to the stress of surgery is associated with increased risk of adverse outcomes and higher levels of resource use. As Western populations age rapidly, older people with frailty are presenting for surgery with increasing frequency. This means that anesthesiologists and other perioperative clinicians need to be familiar with frailty, its assessment, manifestations, and strategies for optimization. We present a narrative rev

Alcohol cue reactivity and inhibitory control in young adults at risk for alcohol use disorder.

OBJECTIVE: This study examined whether automatic and reflective neurocognitive processes differ as a function of alcohol use risk, sex, and cue type among young adult binge drinkers. METHOD: Eighty healthy young adults (71% female; Mage = 20.8 years) who reported at least two binge drinking episodes in the past month completed alcohol-cued Oddball and Go/NoGo tasks while electroencephalography was recorded. Event-related potentials indexing alcohol cue reactivity (P3) and inhibitory control (NoGo N2) were analyzed. Participants were stratified into higher and lower risk groups using k-means c

Unmet needs poststroke and associated factors: A systematic review and meta-analysis.

PURPOSE/OBJECTIVE: Data on commonly reported unmet needs poststroke are crucial to inform interventions and ongoing care of survivors. We aimed to synthesize contemporary data on the prevalence of unmet needs among survivors in the community, and associated factors. RESEARCH METHOD/DESIGN: We searched MEDLINE, Embase, and CINAHL for articles with prevalence estimates on unmet needs poststroke. Only articles comprising data collected from 2013 to June 2025 from survivors living in the community or their proxies were included. A random-effects model was used to meta-analyze prevalence, whereas

Cognitive behavioral telehealth treatment for adolescents with loss-of-control eating: A randomized controlled feasibility study.

This study aimed to develop and test the feasibility and acceptability of a telehealth treatment for adolescent loss-of-control (LOC) eating, including the feasibility of attaining clinically meaningful outcomes. LOC-eating episodes occur when experiencing a sense of LOC while eating a subjectively or objectively large amount of food. Participants were 55 adolescents (aged 12-17) with recurrent LOC-eating episodes and elevated weight (based on age and sex-normed percentiles) who enrolled in a feasibility trial of cognitive behavioral therapy (CBT). Five participants enrolled in the treatment d

Research on the dynamic evolution law of four-dimensional in-situ stress during the production process in deep shale gas reservoir.

Deep shale gas is a key area for increasing reserves and production of shale gas in China, and its efficient development is of great strategic significance to national energy security. As the core demonstration area of deep shale gas exploration and development, the complex structure and heterogeneous reservoir conditions in southern Sichuan pose a serious challenge to long-term production safety. The pore pressure drop induced by deep shale gas production drives the dynamic evolution of in-situ stress field through fluid-solid coupling, which is the key geomechanical mechanism to control frac

Single-item patient-rated helpfulness and improvement as an alternative to standardized questionnaires for establishing anxiety and depression treatment efficacy.

Evidence-based psychological treatments for anxiety and depression are widely used, yet roughly half of those treated do not respond. Treatment response prediction could help to optimize patient outcomes and use of clinical resources. However, existing longitudinal studies with potentially valuable predictors are unlikely to include comprehensive, prospective measures of symptoms throughout therapy. Single-item patient ratings of helpfulness and improvement are a potentially cost-effective and efficient alternative, but their relationship with typically used change score measures is unknown. D

Multimodal prehabilitation combined with perioperative enhanced recovery after surgery care for older patients undergoing spinal fusion surgery in China: protocol for a multicentre randomised controlled trial (PRACTICE trial).

INTRODUCTION: Besides the comorbid risk factors shared by older and younger patients, older individuals may also experience malnutrition, as well as cognitive or functional impairments. The accumulation of frailty and various geriatric syndromes in older individuals results in decreased physiological reserves, which makes the recovery process after spine surgery particularly challenging. Theoretically, combining the presurgery optimisation provided by a multimodal prehabilitation programme with the reduction of surgical stress provided by an enhanced recovery after surgery (ERAS) programme cou
