The Role of Acupuncture in Treating a Patient with a Gambling Disorder.

BACKGROUND: Addiction is a chronic/relapsing disorder characterized by compulsive seeking and continuous involvement despite dangerous consequences. It causes long-term changes in the brain. Gambling disorder is a nonsubstance behavioral addiction. An important feature of gambling disorder is maladaptive gambling behavior that is persistent and repetitive, and interferes with patients' personal lives, families, and/or activities. Acupuncture is a nonpharmacologic therapeutic modality for managing addiction, with good results. CASE: A 32-year-old man with a gambling disorder was referred from

Examination of the utility of the Beck Anxiety Inventory and its factors as a screener for anxiety disorders.

The Beck Anxiety Inventory (BAI) and the Anxiety Disorders Interview Schedule (ADIS-IV) were administered to 193 adults at a major Midwestern university recruited from an anxiety research and treatment center. The BAI and its four factor scores were compared from individuals with a primary diagnosis of generalized anxiety disorder (GAD), specific or social phobia, panic disorder with or without agoraphobia, obsessive-compulsive disorder (OCD), and no psychiatric diagnosis. The cut scores on the BAI and its factors, their sensitivity, specificity, as well as positive and negative predictive val

Musical hallucinations: prevalence in psychotic and nonpsychotic outpatients.

BACKGROUND: Musical hallucinations have been considered a rare manifestation of psychotic states or brain and hearing abnormalities. However, an obsessive-compulsive disorder (OCD) assessment tool refers to musical hallucinations and our preliminary study showed that about one third of OCD patients experienced musical hallucinations. AIMS: To elucidate the lifetime prevalence of musical hallucinations among psychotic and nonpsychotic psychiatric outpatients. METHODS: Lifetime experience of musical hallucinations was examined with a specially designed structured interview in 190 consecutive o

(N-3) Fatty acids: molecular role and clinical uses in psychiatric disorders.

New discoveries in the field of neurophysiology and neuropharmacology have revealed the role of (n-3) fatty acids in controlling inflammation and protecting neuron cells from oxidative damage, preserving their function. It has also been thought that their psychoactive properties could be beneficial in certain psychiatric illnesses. This article discusses the newest discoveries of the affected activities by these fats in the cerebral cortex and the efforts that have been made to put them in practice in clinical trials in humans. In general, we were able to detect certain discord in the scientif

Impulse control disorders in Parkinson's: Sleep disorders and nondopaminergic associations.

OBJECTIVES: Impulse control disorders (ICDs) are common among patients with Parkinson's disease (PD). Risk factors identified for developing ICDs include young age, family history, and impulsive personality traits. However, the association of these potentially disabling disorders with nondopaminergic drugs and sleep disorders has been understudied. Our objective was to examine the association between ICDs and nondopaminergic medications and sleep disorders. METHODS: We conducted an observational study of 53 patients with PD from the National Institute of Neurology and Neurosurgery. ICDs were

Repetitive transcranial magnetic stimulation: advances for psychiatric disorders.

Repetitive transcranial magnetic stimulation (rTMS) has accumulated substantial exploratory evidence in the treatment of major Depressive Disorders, bipolar disorder, schizophrenia, obsessive-compulsive disorder, and generalized anxiety disorder; however, its clinical efficacy is highly dependent on the precise matching of stimulation parameters (such as frequency, intensity, total number of pulses, and treatment course) with target regions (such as the dorsolateral prefrontal cortex, supplementary motor area, temporal-parietal cortex). Key findings: In major Depressive Disorders, both high-fr

From Exposure to Insight: Lessons From Five Contemporary OCD Cases and Where Treatment Should Go Next.

Exposure and response prevention (ERP) remains the gold-standard psychotherapy for obsessive-compulsive disorder (OCD), yet real-world care is limited by dropout, partial response, relapse, and phenotypes that strain habituation-centric protocols. This commentary synthesizes five case reports that upgrade ERP not by replacing it, but by refining how inhibitory learning is taught and enacted. In the first case study Capel and Twohig 2025, Acceptance and Commitment Therapy (ACT) with values-based exposure shifted the goal from distress reduction to values-consistent action under aversive private

From Pharmacological Treatment to Neuromodulation: A Comprehensive Approach to Managing Gilles de la Tourette Syndrome.

Gilles de la Tourette syndrome (GTS) is a neurodevelopmental disorder characterized by motor and phonic tics, often including attention deficit, hyperactivity, and obsessive-compulsive behaviours. The pathophysiology involves the dysfunction of cortico-striato-thalamo-cortical circuits, primarily implicating dopaminergic hyperactivity, but also involving multiple different neurotransmitter systems. Treatment of GTS is complex, highly individualized, and influenced by considerable variability in symptom presentation. Behavioural approaches, such as Habit Reversal Therapy (HRT), play a key role,

Identifying subgroups of urge suppression in Obsessive-Compulsive Disorder using machine learning.

Obsessive-compulsive disorder (OCD) is phenomenologically heterogeneous. While predominant models suggest fear and harm prevention drive compulsions, many patients also experience uncomfortable sensory-based urges ("sensory phenomena") that may be associated with heightened interoceptive sensitivity. Using an urge-to-blink eyeblink suppression paradigm to model sensory-based urges, we previously found that OCD patients as a group had more eyeblink suppression failures and greater activation of sensorimotor-interoceptive regions than controls. However, conventional approaches assuming OCD homog

Group schema therapy for cluster-C personality disorders: A multicentre open pilot study.

BACKGROUND: Group schema therapy (GST) is increasingly popular as a treatment for personality disorders (PDs), including Cluster-C PDs. Individual ST has proven to be effective for Cluster-C PD patients, while the evidence for GST is limited. This study aimed to investigate the effectiveness of GST for Cluster-C PD. Moreover, differences between the specific Cluster-C PDs (avoidant PD, dependent PD and obsessive-compulsive PD) were explored. METHODS: A multicentre open trial was conducted, including 137 patients with a Cluster-C PD (avoidant PD: n = 107, dependent PD: n = 11 and obsessive-com

Use of differential stimulation of the nucleus accumbens and anterior limb of the internal capsule to improve outcomes of obsessive-compulsive disorder.

OBJECTIVE: Personalized stimulation is key to optimizing the outcomes of deep brain stimulation (DBS) for refractory obsessive-compulsive disorder (OCD). However, the contacts in a single conventional electrode cannot be programmed independently, which may affect the therapeutic efficacy of DBS for OCD. Therefore, a novel designed electrode and implantable pulse generator (IPG) that could achieve differential stimulation parameters for different contacts was implanted into the nucleus accumbens (NAc) and anterior limb of the internal capsule (ALIC) of a cohort of patients with OCD. METHODS: T

Psychometric evaluation of the thought-action fusion scale in a large clinical sample.

This study examined the psychometric properties of the 19-item Thought-Action Fusion (TAF) Scale, a measure of maladaptive cognitive intrusions, in a large clinical sample (N = 700). An exploratory factor analysis (n = 300) yielded two interpretable factors: TAF Moral (TAF-M) and TAF Likelihood (TAF-L). A confirmatory bifactor analysis was conducted on the second portion of the sample (n = 400) to account for possible sources of item covariance using a general TAF factor (subsuming TAF-M) alongside the TAF-L domain-specific factor. The bifactor model provided an acceptable fit to the sample da

The Spectrum of Psychiatric Comorbidity in Individuals With Inflammatory Bowel Disease.

BACKGROUND: Research on psychiatric comorbidity in inflammatory bowel disease (IBD) has focused mostly on anxiety and depression. This study aimed to describe the spectrum of psychiatric disorders experienced by individuals with IBD and their overlap. METHODS: Participants were enrolled in a prospective 3-year longitudinal study that assessed psychiatric comorbidity in immune-mediated inflammatory disease. Lifetime prevalence of psychiatric comorbidity was assessed using the Structured Clinical Interview for DSM-IV Disorders (SCID-IV), as the DSM-IV was the prevailing classification at the ti

Pharmacological treatment of tic disorders and Tourette Syndrome.

The present review gives an overview of current pharmacological treatment options of tic disorders and Tourette Syndrome (TS). After a short summary on phenomenology, clinical course and comorbid conditions we review indications for pharmacological treatment in detail. Unfortunately, standardized and large enough drug trials in TS patients fulfilling evidence based medicine standards are still scarce. Treatment decisions are often guided by individual needs and personal experience of treating clinicians. The present recommendations for pharmacological tic treatment are therefore based on both
