Single-stapled double-pursestring anastomosis after anterior resection of the rectum.

BACKGROUND: Single-stapled double-pursestring technique for colorectal anastomosis to the mid-rectum or upper rectum is the most commonly used technique in the single institution reported here. The investigators evaluate single-stapled double-pursestring anastomosis after anterior resection of the rectum performed at a single institution. METHODS: Medical records of patients who underwent single-stapled double-pursestring anastomosis between January 2000 and May 2005 were analyzed to identify postoperative anastomotic complications. Patients with previous radiation, diverting stoma, coloanal,

Appendico-ileo-vesical fistula.

Appendico-vesical fistula is a rare condition. In total, 109 cases, most secondary to appendicitis, have been reported in the English-language literature. We report the first case, to our knowledge, of appendico-ileo-vesical fistula secondary to appendiceal diverticulitis. An enterovesical fistula was diagnosed by urine culture, cystoscopy, and computed tomography. The locations of enteric opening sites were demonstrated by barium enema and colonoscopy. Ileocecal resection and fistulectomy with primary reconstruction were performed. We believe that accurate pre- and intra-operative diagnosis i

[Laparoscopy-assisted reoperation after sigmoid resection and Hartman rectal occlusion].

The anastomosis between the descending colon an the rectal stump after sigma-resection and blind closure of the rectum according to Hartmann could be performed as well via an open laparotomy approach as via a laparoscopic procedure. By means of a first group of five laparoscopically reoperated patients the authors are demonstrating the good practicability of their laparoscopic method. Extended adhesions in the pelvic area could be detached in all cases. The advantages of this laparoscopic operation are obvious and in the authors opinion laparoscopic assisted reconnection between colon and rect

Risk factors of early rebleeding in the endoscopic management of colonic diverticular bleeding.

BACKGROUND AND AIM: The risk factors for early rebleeding following the management of colonic diverticular bleeding (CDB) are unclear. This study aimed to determine the risk factors for early rebleeding following initial colonoscopy. METHODS: Overall, 370 patients with CDB were divided as having presumptive (229) or definite CDB with stigmata of recent hemorrhage (141) on the basis of initial colonoscopy. Definite CDB cases were treated by either endoscopic clipping (EC) or endoscopic band ligation (EBL) as a first-line treatment. Time-to-event analysis for early rebleeding was performed by K

Laparoscopic resection for diverticular disease: follow-up of 500 consecutive patients.

OBJECTIVE: To examine morbidity, mortality, conversion rates, and disease recurrence after laparoscopic resection of complicated and uncomplicated diverticular disease in a single center. SUMMARY BACKGROUND DATA: In contrast to colorectal cancer, there are few large studies of laparoscopic or open resection for diverticular disease. METHODS: This study represents a retrospective analysis of a prospectively collected database of all laparoscopic resections for uncomplicated and complicated diverticulitis from a single center. RESULTS: Five hundred patients (305 female) were identified (media

Vacuum-assisted closure for the treatment of parastomal skin necrosis: a novel approach to an unusual complication. Report of a case.

Extensive peristomal skin necrosis is an unusual complication, which is difficult to manage because of frequent contamination from the stoma site. This wound leads to difficulty in applying the stoma appliance. This case report describes the successful use of the vacuum-assisted closure system in the management of a patient who developed peristomal skin necrosis after emergency Hartmann's procedure.

The outcome of staged management of complicated diverticular disease of the sigmoid colon.

In a retrospective study, 83 consecutive patients with indication for a staged treatment of complicated diverticular disease were reviewed. There was a considerable overall morbidity rate following construction of the colostomy, colonic resection, and colostomy closure. However, only 58% underwent a resection of the diseased segment, and even fewer (46%) obtained an eventual restoration of the colon continuity. This was primarily due to an unacceptable high mortality rate when patients with general peritonitis were managed by a proximal colostomy and drainage only. Subsequently, advanced age a

Oral rehydration solution for the management of fluid and electrolyte disturbances in patients with an ileostomy: A scoping review.

Ileostomy creation is common among patients with colorectal cancer, diverticulitis, and inflammatory bowel disease. Surgical removal or diversion of the colon can result in nutrition complications, most notably dehydration-a leading cause of hospital readmission-and increased risk of kidney injury. Existing studies on fluid and electrolyte management after ileostomy largely focus on short bowel syndrome or high-output stomas, overlooking the physiological implications of ileostomy creation, even in patients with normal output. Given the absence of standardized oral rehydration protocols and li

Lactobacillus Paracasei CNCM I 1572 is Better than Placebo in Preventing Acute Diverticulitis Occurrence (Revised Manuscript 661a120f-b910-4133-ab7e-4bd3e1713c96).

Data analysing the impact of probiotics in preventing the occurrence of acute diverticulitis (AD) in patients having symptomatic uncomplicated diverticular disease (SUDD) of the colon are still scarce. We assessed the effectiveness of Lactobacillus paracasei CNCM I 1572 in preventing the first episode of AD in patients with SUDD. A post-hoc analysis of a 12-month, multicentre, double-blind, placebo-controlled study was conducted. One hundred and five patients were randomly enrolled in a double-blind fashion into Group L (Lactobacillus paracasei CNCM I 1572, 24 billion CFU/day) and Group P (pla

Epiploic Appendagitis Unveiled: A Case of Misleading Abdominal Pain With Omental Fat Necrosis.

Epiploic appendagitis is a benign inflammatory condition of the epiploic appendages, small fat-filled structures attached to the colon. Misdiagnosed frequently as more serious conditions like appendicitis or diverticulitis, it usually resolves with minimal treatment. This case report aims to emphasize the importance of recognizing epiploic appendagitis in differential diagnoses, highlighting the role of accurate imaging and surgical intervention in managing unusual presentations. We report a case involving a 27-year-old male who presented with acute, severe pain in the left iliac fossa. Initia

Conservative management of fish bone-induced large bowel perforation: Case report.

INTRODUCTION: Fish bone ingestion is one of the common medical complaint. Most foreign bodies passed safely through gastrointestinal tract (GIT) without any complications. The clinical presentation of foreign body ingestion is similar to other conditions such as diverticulitis. Most literatures focus on the surgical management of complications secondary to fish bone ingestion. In this case we report a case of an elder patient with complain of progressive abdominal pain. PRESENTATION OF CASE: 71-year-old female, admitted to surgical ward with the complain of progressive abdominal pain. Physica

[Diagnosis of vesico-intestinal fistulas by contrast medium enhanced 3-D ultrasound].

AIM: Standard diagnostic tools for vesico-intestinal fistulas are cystoscopy, cystography, colonoscopy, and contrast enema. The aim of our study was to evaluate the efficacy of transrectal 3D-ultrasound with contrast media in these patients. METHOD: From 5/98 to 12/99 we examined 10 patients with symptoms of a vesico-intestinal fistula (pneumaturia, faecaluria). After placement of a transurethral catheter a transabdominal ultrasound examination (Kretz Combison 530) was performed with the bladder half full to evaluate the bladder wall. Then the bladder was filled with diluted ultrasound contra

Gastrointestinal disease following heart transplantation.

With advances in heart transplantation, a growing number of recipients are at risk of developing gastrointestinal disease. We reviewed our experience with gastrointestinal disease in 92 patients undergoing 93 heart transplants. All had follow-up, with the median time 4.8 years (range 0.5-9.6 years). During the period of the study we progressively adopted a policy of low immunosuppression aiming toward monotherapy with cyclosporine. Nineteen patients (20.6%) developed 28 diseases related to the gastrointestinal tract. Thirteen patients required 18 surgical interventions, five as emergencies: cl
