Chondroblastoma of the temporal bone.

Chondroblastomas are highly destructive tumors that are derived from immature cartilage cells. The occurrence of this tumor in the temporal bone or skull base is uncommon. Approximately 70 cases have previously been reported, several of which have involved the temporomandibular joint (TMJ). We report here the case of a 67-year-old woman who presented with right-sided mixed hearing loss, a right external auditory canal mass, ear fullness, otalgia, blood-stained otorrhea, and pain around the TMJ, associated with difficulty in opening the mouth. CT and MRI revealed a mass involving the TMJ, infra

The resected area of the posterior wall of the external auditory canal during transcanal endoscopic ear surgery for cholesteatoma.

OBJECTIVE: The aim of this study was to evaluate part of the area of the posterior wall of the external auditory canal (EAC) that is resected during transcanal endoscopic ear surgery (TEES) for cholesteatomas that extend to the mastoid cavity, and to determine whether TEES is a minimally invasive surgical procedure for cholesteatoma. METHODS: This was a retrospective study involving 25 patients with cholesteatoma that extended to the mastoid cavity, and who underwent surgery between October 2014 and October 2015. The patients' cholesteatomas were removed using TEES. In this procedure, the sup

Vein versus tragal perichondrium in stapedotomy.

OBJECTIVE: To assess, in otosclerosis surgery, whether the vein or the tragal perichondrium in stapedotomy with interposition yields the better long-term hearing outcome. STUDY DESIGN: A retrospective chart review of prospectively collected audiometric data of 452 ears. SETTING: Academic tertiary otology-neurotology referral center. PATIENTS: Four hundred fifty-two stapedotomies with interposition were performed in 412 patients (bilateral in 40 patients) by the senior author (R.C.) between 1987 and 1998. A tragal perichondrium graft was used in 314 cases and a vein graft was used in 138 cas

A Novel Single Stage Reconstruction of Midline Nasal Dorsum Skin Defects Using an Auricular Composite Graft.

BACKGROUND: Reconstruction of midline nasal dorsum skin defects following oncologic excision is challenging because of the aesthetic prominence of the region and the limited mobility of surrounding soft tissues. Flap-based techniques are commonly recommended; however, they may be associated with donor-site morbidity, visible scarring, and multistage procedures. Auricular composite grafts are well established for select nasal subunits, but their intentional use for midline nasal dorsum skin reconstruction has not been routinely described. METHODS: We describe a novel single-stage reconstructiv

Management of Post-traumatic Ear Reconstruction in a Tertiary Care Center: A Retrospective Study.

Background The external ear, due to its prominent anatomical position and complex three-dimensional (3D) cartilaginous framework, is particularly susceptible to traumatic injuries. There has been a rising incidence of partial and total auricular deformities resulting from road traffic accidents, assaults, human and animal bites, burns, and accidental falls. These injuries are often aesthetically conspicuous and psychologically distressing. Reconstruction of post-traumatic auricular defects poses significant challenges because of the need to restore bilateral symmetry, the limited availability

Relapsing polychondritis: prospective study of 23 patients and a review of the literature.

Relapsing polychondritis (RP) is not a totally rare rheumatic disease. We have seen 23 patients from 1960-1975, and there are now a total of 159 reported cases, which form the basis of this study. RP occurs equally in both sexes, and has a maximum frequency in the fourth decade. 2) Empirically defined diagnostic criteria are proposed, to include the most common clinical features: a) Bilateral auricular chondritis b) Nonerosive sero-negative inflammatory polyarthritis c) nasal chondritis d) Ocular inflammation e) Respiratory tract chondritis f) Audiovestibular damage The diagnosis is based prim

Contemporary Treatment of Microtia-Atresia.

Microtia-atresia is a congenital deformity affecting the external ear and ear canal that can present with varying degrees of severity and morbidity. Treatment occurs along a spectrum and primarily centers on improving aesthetic appearance. Many cases of microtia will not be effectively treated with conservative measures and will require some form of reconstruction. There are several options available, including porous polyethylene implants, autologous rib grafting, and autologous chondrocyte frameworks. Equally significant is the treatment of hearing loss, as many patients with microtia-atresi

Benefit of a new hearing device utilizing cartilage conduction.

OBJECTIVE: Our previous study demonstrated that sound was effectively transmitted by attaching a transducer to the aural cartilage even without fixation pressure. This new method for sound transmission was found by Hosoi in 2004, and was termed cartilage conduction (CC). CC can be utilized even in hearing-impaired patients who cannot use air-conduction hearing aids owing to continuous otorrhea or aural atresia. A prototype hearing aid employing CC was investigated in this study. METHODS: Four patients with conditions such as continuous otorrhea and acquired aural atresia after surgery partici

Super-Thin ALT Flap for Composite Nasal Reconstruction in an Elderly Kidney Transplant Patient.

Nasal reconstruction is one of the most challenging procedures in plastic surgery, requiring restoration of the three-dimensional structure, precise contour, internal lining, and airway function. These challenges are further amplified when traditional donor sites are unavailable, particularly in elderly patients or those undergoing concomitant treatments. We report the case of a 78-year-old man on immunosuppressive therapy following kidney transplantation, presenting with recurrent basal cell carcinoma affecting the middle and lower thirds of the nose. After tumor excision resulting in a 7 × 5

The Effect of External Auditory Canal Packing Duration on Healing After Endoscopic Cartilage Myringoplasty.

INTRODUCTION: The external auditory canal (EAC) packing is widely used in otosurgery and is considered to promote hemostasis and to support tympanic membrane grafts. However, few studies have investigated the effects of packing removal time on healing. OBJECTIVE: We investigated the effect of EAC packing duration on healing after endoscopic cartilage myringoplasty in patients with chronic tympanic membrane perforations (TMPs). MATERIALS AND METHODS: Patients with chronic TMPs who underwent endoscopic "push-through" cartilage myringoplasty were divided into early and late groups based on the

Temporomandibular joint herniation into the external ear canal through foramen of Huschke.

Foramen of Huschke, which is also known as the foramen tympanicum, is an anatomical variation of the tympanic portion of the temporal bone. This foramen is located on the anteroinferior aspect of the external auditory canal, posteromedial to the temporomandibular joint. Herein, a rare case of foramen Huschke with a herniation of the soft tissues around the temporomandibular joint that result in severe otalgia and its treatment are presented.
