Eligibility and Implementation of Rivaroxaban for Secondary Prevention of Atherothrombosis in Clinical Practice-Insights From the CANHEART Study.

Background The COMPASS (Cardiovascular Outcomes for People Using Anticoagulation Strategies) trial decreased major adverse cardiovascular events with very low-dose rivaroxaban and aspirin in patients with coronary artery disease and peripheral artery disease. We examined the eligibility and potential real-world impact of this strategy on the COMPASS-eligible population. Methods and Results COMPASS eligibility criteria were applied to the CANHEART (Cardiovascular Health in Ambulatory Care Research Team) registry, a population-based cohort of Ontario adults. We compared 5-year major adverse card

Early Apixaban Use Following Stroke in Patients With Atrial Fibrillation: Results of the AREST Trial.

BACKGROUND AND PURPOSE: It is unknown when to start anticoagulation after acute ischemic stroke (AIS) from atrial fibrillation (AF). Early anticoagulation may prevent recurrent infarctions but may provoke hemorrhagic transformation as AF strokes are typically larger and hemorrhagic transformation-prone. Later anticoagulation may prevent hemorrhagic transformation but increases risk of secondary stroke in this time frame. Our aim was to compare early anticoagulation with apixaban in AF patients with stroke or transient ischemic attack (TIA) versus warfarin administration at later intervals. ME

4-Year Outcomes After Left Atrial Appendage Closure Versus Nonwarfarin Oral Anticoagulation for Atrial Fibrillation.

BACKGROUND: The PRAGUE-17 (Left Atrial Appendage Closure vs Novel Anticoagulation Agents in Atrial Fibrillation) trial demonstrated that left atrial appendage closure (LAAC) was noninferior to nonwarfarin direct oral anticoagulants (DOACs) for preventing major neurological, cardiovascular, or bleeding events in patients with atrial fibrillation (AF) who were at high risk. OBJECTIVES: This study sought to assess the prespecified long-term (4-year) outcomes in PRAGUE-17. METHODS: PRAGUE-17 was a randomized noninferiority trial comparing percutaneous LAAC (Watchman or Amulet) with DOACs (95% ap

Carbamazepine interaction with direct oral anticoagulants: help from the laboratory for the personalized management of oral anticoagulant therapy.

Current guidelines recommend caution in prescribing concomitant use of direct-acting oral anticoagulants (DOACs) and antiepileptic drugs due to drug-drug interactions leading to potential risk of DOACs subtherapeutic concentration and treatment failure. Herein we report a significant interaction between carbamazepine (CZP) and apixaban, causing subtherapeutic concentration of the drug in a patient with atrial fibrillation who had a transient ischemic attack (TIA) episode. Another anti-Xa DOAC, edoxaban, administered to the patient after TIA occurrence did not show significant interaction with

Effect of direct oral anticoagulant for acute major cerebral artery occlusion in cardioembolic stroke/transient ischemic attack patients with non-valvular atrial fibrillation.

BACKGROUND: Direct oral anticoagulants (DOACs) can reduce the frequency of cardioembolic stroke with non-valvular atrial fibrillation as well as or better compared to vitamin K antagonists (VKAs). However, whether taking DOACs prior to stroke can prevent acute major cerebral artery occlusion (MCAO) has not been fully elucidated. METHODS: We enrolled patients who underwent cardioembolic stroke or transient ischemic attack with non-valvular atrial fibrillation who were admitted to our hospital between April 2011 and February 2017. The patients were classified into four groups based on anticoagu

Rivaroxaban-Induced Acute Interstitial Nephritis: A Case Report.

BACKGROUND Direct oral anticoagulant agents (DOACs) have become increasingly more popular in recent years and have largely replaced warfarin in the treatment of certain conditions, such as atrial fibrillation, and in the prevention of thromboembolic events. Rivaroxaban is one of the most commonly used direct anticoagulant drugs for conditions such as atrial fibrillation and thromboprophylaxis. CASE REPORT We present a case of a 70-year-old male who developed acute interstitial nephritis after starting rivaroxaban, and who responded to medical treatment, which included corticosteroid therapy. Recurrent acute portal vein thrombosis in liver cirrhosis treated by rivaroxaban.

Cirrhosis can occur with the development of portal vein thrombosis (PVT). PVT may aggravate portal hypertension, and it can lead to hepatic decompensation. The international guideline recommends for anticoagulation treatment to be maintained for at least 3 months in all patients with acute PVT. Low-molecular-weight-heparin and changing to warfarin is the usual anticoagulation treatment. However, warfarin therapy is problematic due to a narrow therapeutic window and the requirement for frequent dose adjustment, which has prompted the development of novel oral anticoagulants for overcoming these

Anticoagulant treatment: the end of the old agents?

New oral anticoagulants used as single target inhibitors of coagulation enzymes have been developed and tested in extensive trial programmes. Results of most of these trials showed non-inferior and/or superior efficacy and safety compared to standard treatment with LMWH or VKA. These results led to registration of these agents for the prophylaxis or treatment of thrombosis, as well as stroke prophylaxis in atrial fibrillation. In addition to good efficacy and safety these agents are more convenient in their use and promise advantages in quality of life. Caution is needed, though, since drug-in

Albumin and Bleeding Risk in Hospitalized Patients Receiving Apixaban.

Highly protein-bound anticoagulants such as warfarin and rivaroxaban have increased therapeutic effects in the setting of hypoalbuminemia due to increased free concentrations in the drug which result in increased bleed risk. Apixaban is also heavily protein bound at 87% but bleeding in hypoalbuminemic patients receiving apixaban has largely been unevaluated. Therefore, this retrospective, cohort study of hospitalized adults who received apixaban and had a serum albumin level measured was conducted. Included patients were evaluated for bleeding events in accordance with International Society on

Development and Validation of Dynamic Prediction Model for Thromboembolic and Bleeding Risk in Patients With Atrial Fibrillation Initiating Direct Oral Anticoagulants.

BACKGROUND: Current tools for assessing ischemic stroke and bleeding risks in patients with atrial fibrillation focus on warfarin users and initial therapy decisions; however, dynamic models for reassessing these risks in direct oral anticoagulant (DOAC) users are lacking. We aimed to develop a tool that enables the dynamic evaluation of thromboembolic and bleeding risks in patients with atrial fibrillation receiving DOAC therapy. METHODS: Using a national claims database, we included 42 450 patients with atrial fibrillation who were newly prescribed DOACs and divided them into development (2

Relative performance evaluation of four bleeding risk scores in atrial fibrillation patients. What does the new DOAC score provide?

BACKGROUND: Recently, the direct oral anticoagulant (DOAC) score was developed and better predicted major bleeding in DOAC-treated patients with atrial fibrillation (AF) than HASBLED did. Little is known on the new score's performance regarding other bleeding risk in AF. METHODS: We studied 14,672 patients diagnosed with AF between 2014 and 2018. During follow-up, we assessed the performance of DOAC score compared with the HASBLED, ORBIT and SWISS scores at predicting major bleeding in DOACs and non-DOACs users. Discrimination, calibration and decision curve analysis (DCA) were used to assess

Reversing Rivaroxaban Anticoagulation as Part of a Multimodal Hemostatic Intervention in a Polytrauma Animal Model.

BACKGROUND: Life-threatening bleeding requires prompt reversal of the anticoagulant effects of factor Xa inhibitors. This study investigated the effectiveness of four-factor prothrombin complex concentrate in treating trauma-related hemorrhage with rivaroxaban-anticoagulation in a pig polytrauma model. This study also tested the hypothesis that the combined use of a low dose of prothrombin complex concentrate plus tranexamic acid and fibrinogen concentrate could improve its subtherapeutic effects. METHODS: Trauma (blunt liver injury and bilateral femur fractures) was induced in 48 anesthetize…

Inhibition of tissue factor-dependent and -independent coagulation by cell surface expression of novel anticoagulant fusion proteins.

BACKGROUND: Thrombotic vascular occlusion occurs in disorders of diverse etiology, including atherosclerosis, vasculitis, and disseminated intravascular coagulation. The same process results in hyperacute rejection of renal allografts transplanted into sensitized patients and remains a major problem in experimental xenotransplantation. METHODS: We have previously described the design and expression of several genetic constructs encoding novel fusion proteins with anticoagulant properties. They are based on two naturally occurring soluble anticoagulant proteins, human tissue factor pathway inh

Monitoring the new antithrombotic drugs.

Today there is a diverse group of anticoagulant and antithrombotic drugs available that includes warfarin derivatives, heparin, low-molecular-weight heparins, thrombin inhibitors, factor Xa inhibitors, and various antiplatelet agents. Many of these new drugs do not alter measurable blood coagulation parameters, yet they are effective antithrombotic agents through their actions on vascular endothelial cells and proteins. Thus, these new agents do not affect the traditional clot-based prothrombin time/International Normalized Ratio (PT/INR) and activated partial thromboplastin time (aPTT) tests,…

External Validation and Bayesian Forecasting of Rivaroxaban Population Pharmacokinetic Models in Older Chinese Patients with Nonvalvular Atrial Fibrillation.

BACKGROUND: Population pharmacokinetic (PopPK) models have been developed to characterize rivaroxaban exposure; however, their transferability across different clinical populations remains uncertain. This study aimed to externally evaluate published rivaroxaban PopPK models in older Chinese patients with nonvalvular atrial fibrillation (NVAF) and assess whether Bayesian forecasting could improve individual concentration prediction. METHODS: Six published rivaroxaban PopPK models were evaluated using an independent cohort from the RIVA-GAP study. Predictive performance was assessed using predi

Direct oral anticoagulant monotherapy vs combination therapy with antiplatelets in chronic coronary syndrome patients with prior percutaneous coronary intervention requiring anticoagulation: A meta-analysis.

It remains uncertain if direct oral anticoagulant monotherapy improves outcomes beyond bleeding risk compared with combination therapy with antiplatelets in chronic coronary syndrome patients with prior percutaneous coronary intervention requiring anticoagulation. In this meta-analysis of randomized trials, direct oral anticoagulant monotherapy significantly reduced major adverse cardiac and cerebrovascular events and major bleeding without increasing ischemic events. The benefit in major adverse cardiac and cerebrovascular events was driven primarily by reductions in all-cause and cardiovascu

Direct Oral Anticoagulants in Primary and Secondary Prevention of Thrombotic Events: 2026 ACC Scientific Statement: A Report of the American College of Cardiology.

Direct oral anticoagulants (DOACs) have transformed the prevention and treatment of thromboembolic disease, offering predictable pharmacokinetics, fewer interactions, and improved safety compared with vitamin K antagonists. Over the past decade, DOACs have become the preferred oral anticoagulants for most patients with atrial fibrillation and venous thromboembolism; yet, important gaps remain in their optimal implementation across diverse clinical scenarios. Despite strong evidence supporting their efficacy and safety, DOACs continue to be underutilized and sometimes inappropriately dosed, par
