Proximal interphalangeal joint arthroplasty using self-locking type surface replacement implant for symptomatic osteoarthritis: Extended dorsal central splitting approach and intermediate-term outcome.

BACKGROUND: This retrospective study introduced wide exposure and radical osteophytectomy by employing the extended dorsal central splitting approach (EDCSA) to mitigate complications associated with surface implant arthroplasty for osteoarthritis of the proximal interphalangeal (PIP) joint using a self-locking type of implant. METHODS: In total, 171 fingers of 113 patients underwent cementless surface non-constrained PIP implantation using the EDCSA. The surgical approach involved larger implant placement, optional procedures, and complex collateral ligament repair following wide exposure an

Smile incision and reverse shotgun approach in distal interphalangeal joint arthrodesis.

BACKGROUND: Arthrodesis serves as the traditional therapeutic approach for advanced distal interphalangeal joint (DIPJ) arthritis. However, the conventional technique may prove insufficient when the excision of pronounced volar and lateral spurs is required. To address this, we innovated the 'smile incision with reverse shotgun approach'. This method enhances joint exposure and yields superior cosmetic results by extending the transverse skin incision over the DIPJ and cleaving the accessory collateral ligament, thereby improving access to the volar and lateral joint margins. Conservative tibiotalocalcaneal fusion for partial talar avascular necrosis in conjunction with ankle and subtalar joint osteoarthritis in Kashin-Beck disease: A case report.

RATIONALE: Kashin-Beck disease (KBD) is known for some typical characters like finger joint enlargement, shortened fingers, and dwarfism. However, Avascular necrosis (AVN) of the talus in KBD has rarely been reported in the literature. Here, we reported on a KBD patient presented with partial AVN of the talus in conjunction with ankle and subtalar arthritis. PATIENT CONCERNS: A 50-year-old woman presented with severe pain and limited range of motion in her left ankle and subtalar joint while walking for 2 years. She had been walking with the aid of crutches for many years. Conservative treatm…

Postural and Intention Tremors: A Detailed Clinical Study of Essential Tremor vs. Parkinson's Disease.

BACKGROUND: An estimated 30-50% of essential tremor (ET) diagnoses are incorrect, and the true diagnosis in those patients is often Parkinson's disease (PD) or other tremor disorders. There are general statements about the tremor in these ET and PD, but published data on the more subtle characteristics of tremor are surprisingly limited. Postural tremor may occur in both disorders, adding to the difficulty. There are several anecdotal impressions regarding specific features of postural tremor in ET vs. PD, including joint distribution (e.g., phalanges, metacarpal-phalangeal joints, wrist), tre

Acute septic arthritis of the fingers. A clinical study of 87 cases.

The authors analyse a clinical series of 87 operated cases of acute septic arthritis of the fingers. Arthritis followed either a direct neglected punctiform inoculation or an adjacent infection (subcutaneous felon), or a serious open wound having received emergency surgery. During surgery a careful assessment was made of the extent of the infection, indicating the condition of the cartilage and that of the articular and periarticular soft tissues. Treatment was either conservative (surgical drainage, synovectomy and closure) or non-conservative (arthrodesis, articular resection or even amputat

Delayed flexor tendon entrapment in callus after an open comminuted distal radius physeal injury in a child: A case report.

Tendon entrapment within fracture callus is an uncommon complication in children, particularly after forearm fractures. We report an 8-year-old boy with an open comminuted distal radius physeal injury treated with debridement, open reduction, and K-wire fixation, who later developed an isolated flexion contracture of the index finger at the distal interphalangeal joint. Surgical exploration revealed entrapment of the flexor digitorum profundus tendon of the index finger within a bony tunnel formed by exuberant callus. The bony tunnel was excised, and tenolysis was performed. The child regained

[Therapy of severe hand infections with local penicillin-streptomycin administration].

The primary goal of treatment of severe hand infections (finger joint infection, tenosynovitis, infections of the palm and the dorsum of the hand) is nowadays not only a matter of bringing the infection under control, but also trying to get a good functional result. To reach good results the author regards two steps in treating severe hand infections as indispensable: a) the principle of early operation (the average interval between the first examination in the hospital and surgery was only 0,5 days) and b) the local instillation of antibiotics in high concentration (penicillin and streptomyci…

Tenosynovial giant cell tumor in the cervico-thoracic junction.

Tenosynovial giant cell tumor (TGT) rarely arises from the posterior coloumn of the cervical spine. Most lesions of TGT involve the tendon sheath and joint lining of the small joints of the fingers and hands, and consecutively the knee, ankles and feet, and hips. Rate of extra-articular presentation is about 5-15% in all cases. In this report, a case with paraparesis caused by TGT in the cervico-thoracic junction is presented. The clinical manifestations, diagnosis, and treatment of this unusual condition are discussed. In the treatment of TGT of the vertebral column, the main aim should be to

[Arthroplasty of the rheumatoid hand. Pre- and postoperative imaging with special consideration of biomechanical and pathobiomechanical aspects and its radiological evaluation].

Radiology plays a key role when the indications for arthroplasties of the hand and finger joints are determined and for the postoperative follow-up. On the one hand, the degree of inflammatory changes in all affected compartments is to be evaluated and graded; on the other hand, conventional radiograms allow for a first assessment of possible joint instability and impaired biomechanics. Both aspects influence the choice of the proper surgical therapeutic strategy. Osteolysis, deformity, fracture, prosthesis loosening or failure, heterotopic ossification, and foreign body-associated formation o…

Full mosaic monosomy 22 in a child with DiGeorge syndrome facial appearance.

We describe an abnormal premature male infant with mosaic monosomy of chromosome 22. He had a unique facial appearance, similar to those with DiGeorge syndrome, and hypertonicity, limitation of extension at major joints, and flexion contractures of all fingers. This rare chromosomal aberration has been reported previously in 6 cases, three of them being nonmosaic and three mosaic patients. There was a great variability of expression among the anomalies of these patients. However, the most common anomalies were in the face and joints. A correlation between the severity of expression and percent

Computer vision detects inflammatory arthritis in standardized smartphone photographs in an Indian patient cohort.

INTRODUCTION: Computer vision extracts meaning from pixelated images and holds promise in automating various clinical tasks. Convolutional neural networks (CNNs), a deep learning network used therein, have shown promise in analyzing X-ray images and joint photographs. We studied the performance of a CNN on standardized smartphone photographs in detecting inflammation in three hand joints and compared it to a rheumatologist's diagnosis. METHODS: We enrolled 100 consecutive patients with inflammatory arthritis with an onset period of less than 2 years, excluding those with deformities. Each pat

[Hereditary symphalangism: observations on this syndrome in a newborn infant and his mother (author's transl)].

Symphalangism is the partial or complete bony fusion of phalanges in the fingers and toes due to an autosomal dominant gene, with inherited failure of development of one or more interphalangeal joints. Two patients--a newborn baby and his mother--both revealing this rare malformation, are presented in this paper and the clinical and radiological features are described and discussed.

Upper extremity joint tenderness as a practical indicator for assessing presenteeism in rheumatoid arthritis patients: A cross-sectional observational study.

OBJECTIVE: Rheumatoid arthritis (RA) causes chronic polyarthritis and joint dysfunction, reducing work productivity. This reduction is mainly due to presenteeism, characterized by impaired work performance despite being present at work. This study aims to investigate the impact of specific joint involvement, particularly in the upper extremities, on work disability in RA patients. METHODS: Annual surveys assessing work disability were conducted among RA outpatients enrolled in the Nagahama Riumachi Cohort at Nagahama City Hospital, using the Work Productivity and Activity Impairment Questionn

Clinical Outcomes of Retrograde Percutaneous Pinning for Shaft Fracture of Metacarpal Bone: A Case Series.

INTRODUCTION: Surgical treatment should be considered for metacarpal fractures that have severe angulation or shortening. Percutaneous pinning is a useful technique in that it is minimally invasive. Insertion of Kirschner wires using retrograde pinning is easier than that using antegrade or transverse pinning. On the other hand, anterograde pinning is also used for metacarpal shaft and neck fractures, but some complications such as perforation of the metacarpal head or laceration of the extensor tendons can occur. The purpose of this study is to evaluate the functional outcomes of simple retro

The anatomy and function of Cleland's ligaments.

The cutaneous ligaments of the digits have been recognized by anatomists for several centuries, but the best known description is that of John Cleland. Subsequent varying descriptions of their morphology have resulted in the surgical community having an imprecise view of their structure and dynamic function. We micro-dissected 24 fresh frozen fingers to analyze the individual components of Cleland's ligamentous system. Arising from the proximal interphalangeal (PIP) joint, proximal, and sometimes middle phalanx, we found strong ligaments that ran proximally (PIP-P) and distally (PIP-D). On eac
