Personalized Medicine: Ethical Aspects.

In our time of genome-based personalized medicine, clinical research and clinical medicine are accelerating at a quick pace. Faster and cheaper DNA sequencing and protein profiling, microfluidic devices for capturing blood biomarkers, nanoparticles for precise drug delivery and enhanced imaging, rapid computational analysis of massive data inputs, and other technological wonders coalesce to create a kind of Moore's Law for medicine. Needs are obvious, knowledge grows, capital becomes available, but these factors are not entirely sufficient to make health more achievable. Personalized medicine

Real-time imaging of Huntingtin aggregates diverting target search and gene transcription.

The presumptive altered dynamics of transient molecular interactions in vivo contributing to neurodegenerative diseases have remained elusive. Here, using single-molecule localization microscopy, we show that disease-inducing Huntingtin (mHtt) protein fragments display three distinct dynamic states in living cells - 1) fast diffusion, 2) dynamic clustering and 3) stable aggregation. Large, stable aggregates of mHtt exclude chromatin and form 'sticky' decoy traps that impede target search processes of key regulators involved in neurological disorders. Functional domain mapping based on super-re

Cognitive and behavioral phenotype of a young man with a chromosome 13 deletion del(13)(q21.32q31.1).

Cognitive, emotional, and behavioral characterizations have been reported for patients with a few chromosomal imbalances, but not for patients with a 13q deletion. We report the neuropsychological profile and specific linguistic, visual, spatial, constructional, and behavioral disabilities of a young man with a de novo chromosome 13 deletion (13)(q21.32)(q31.1). Karyotyping at 550 G-band resolution showed that the patient's parents did not share the deletion. According to array-comparative genomic hybridization, the deletion spanned about 14 Mb and included 27 genes. A fluorescence in situ hyb

New treatments for chronic prostatitis/chronic pelvic pain syndrome.

Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) is a common condition among men of a wide age range, with detrimental effects on quality of life. The etiology, pathogenesis, and optimal treatment of CP/CPPS remain unknown, although progress has been made in these domains in recent years. A wide variety of pharmacologic and nonpharmacologic therapies have been studied in clinical trials, but most have shown limited efficacy in symptom alleviation. CP/CPPS is increasingly viewed as a condition that involves variable degrees of neuropathic pain. Medications such as gabapentin, pregabal

Molecular modification of T4 bacteriophage proteins and its potential application - review.

Bacteriophage T4 is a virus with well-known genetics, structure, and biology. Such techniques as X-ray crystallography, cryo-EM, and three-dimensional (3D) image reconstruction allowed describing its structure very precisely. The genome of this bacteriophage was completely sequenced, which opens the way for the use of many molecular techniques, such as site-specific mutagenesis, which was widely applied, e.g., in investigating the functions of some essential T4 proteins. The phage-display method, which is commonly applied in bacteriophage modifications, was successfully used to display antigen

Unregulated actin polymerization by WASp causes defects of mitosis and cytokinesis in X-linked neutropenia.

Specific mutations in the human gene encoding the Wiskott-Aldrich syndrome protein (WASp) that compromise normal auto-inhibition of WASp result in unregulated activation of the actin-related protein 2/3 complex and increased actin polymerizing activity. These activating mutations are associated with an X-linked form of neutropenia with an intrinsic failure of myelopoiesis and an increase in the incidence of cytogenetic abnormalities. To study the underlying mechanisms, active mutant WASp(I294T) was expressed by gene transfer. This caused enhanced and delocalized actin polymerization throughout

ERα36, a variant of estrogen receptor α, is predominantly localized in mitochondria of human uterine smooth muscle and leiomyoma cells.

ERα36 is a naturally occurring, membrane-associated, isoform of estrogen receptor α. The expression of ERα36 is due to alternative splicing and different promoter usage. ERα36 is a dominant-negative effector of ERα66-mediated transactivational activities and has the potential to trigger membrane-initiated mitogenic, nongenomic, estrogen signaling; however, the subcellular localization of ERα36 remains controversial. To determine the cellular localization of ERα36 in estrogen-responsive human uterine smooth muscle (ht-UtSMC) and leiomyoma (fibroid; ht-UtLM) cells, we conducted systematic confoc

Human brain imaging studies of DISC1 in schizophrenia, bipolar disorder and depression: a systematic review.

Disrupted-in-Schizophrenia 1 (DISC1) is a well researched candidate gene for schizophrenia and affective disorders with a range of functions relating to neurodevelopment. Several human brain imaging studies investigating correlations between common and rare variants in DISC1 and brain structure and function have shown conflicting results. A meta-analysis of case/control data showed no association between schizophrenia and any common SNP in DISC1. Therefore it is timely to review the literature to plan the direction of future studies. Twenty-two human brain imaging studies have examined the inf

Complete pathological response in patients with bladder cancer: predictors and outcomes.

Pathologic complete response (pCR) in bladder cancer, most often defined as pT0N0 at radical cystectomy (RC) following transurethral resection of the bladder tumor with/without neoadjuvant therapy, has emerged as a powerful prognostic marker. Predicting pCR before RC and understanding its prognostic nuances are increasingly important as perioperative chemotherapy and immunotherapy evolve. In this study, we reviewed the contemporary evidence regarding definitions and clinical contexts of pCR in bladder cancer; clinical, pathological, imaging, molecular, and liquid-biopsy predictors of pCR; and

Enhancing gastric cancer prognosis prediction via multi-step multi-modality ensemble survival modeling of HE-stained images and mIHC data.

BACKGROUND: Gastric cancer's profound molecular and phenotypic heterogeneity hinders precision therapy. Delineating tumor microenvironment (TME) heterogeneity enables improved prognosis and tailored treatment strategies. In this study, we propose a Multi-step Multi-modality Ensemble Survival (MMES) model for gastric cancer prognosis based on HE-stained histological images and multiplex immunohistochemistry (mIHC) data. METHODS: The MMES model employs a multi-step approach to decompose the complex overall survival (OS) prediction task into smaller, manageable subtasks, enabling the use of ligh

A New Radiotranscriptomic Approach to Analyze Combined Sets of T3b Stage-Specific Genes and Radiomic Features in Prostate Cancer.

BACKGROUND: Current clinical staging of prostate cancer (PCa) using the tumor-node-metastasis (TNM) system and serum biomarkers remains limited in distinguishing locally advanced (T3b) PCa from organ-confined (T2c) disease. AIMS: Building on our previous biomarker discovery in differentiating PCa from that of benign prostatic hyperplasia, this study pioneers a radiotranscriptomic model to distinguish T3b stage PCa from T2c stage PCa by integrating contrast-enhanced ultrasound (CEUS) radiomics with stage-specific transcriptomic signatures, addressing a critical knowledge gap in precision stagi

Genetic evidence for causal relationships between brain functional networks and domain-specific recovery after nondisabling ischemic stroke.

Intrinsic brain networks are crucial for post-stroke recovery, but the causal relationships between specific networks and domain-specific recovery outcomes, as well as the role of lipid metabolism, remain unclear. This study leveraged Mendelian randomization (MR) to evaluate 191 resting-state functional magnetic resonance imaging (rs-fMRI) BOLD-derived phenotypes in relation to post-stroke recovery after nondisabling ischemic stroke. Genetic instruments for rs-fMRI phenotypes were derived from a UK Biobank genome-wide association study (n = 34,691). Outcomes included motor, cognitive, and glob

Dynamic Fusion of Genomics and Functional Network Connectivity in UK Biobank Reveals Schizophrenia-Related SNP Manifolds.

Many mental disorders show strong genetic influence. In parallel, dynamic functional network connectivity (dFNC) has shown high sensitivity to brain changes related to mental disorders. However, previous studies linking dFNC to genetics largely follow a paradigm to identify associations between one set of genetic factors and multiple sets of connectivity features from different dFNC states, ignoring the potential variability in genetic correlates across states. We propose a novel joint ICA (jICA)-based "dynamic fusion" framework to identify dynamically tuned genetic manifolds. A sliding window
