Early-Life Gut Health Indicators and Reported Prevalence of Infant Functional Constipation by Healthcare Professionals.

A healthy gut during early childhood is important. However, it seems that there are no standard indicators used to assess it. Healthcare professionals (HCPs) were asked via an electronic survey question about gut health indicators (GHIs) for infants and toddlers, in addition to an estimated prevalence of infant's functional constipation (FC) and its management. HCPs from eight countries participated in the survey (Russia (66.0%, 1449), Indonesia (11.0%, 242), Malaysia (6.0%, 132), Mexico (5.7%, 125), KSA (5.1%, 113), Turkey (3.0%, 66), Hong Kong (2.2%, 49), and Singapore (1.0%, 23)). The 2199

Exclusive human milk feeding and prevalence of early adiposity rebound in ELBW infants: a retrospective cohort study.

The purpose of the present study was to evaluate the prevalence of early adiposity rebound (EAR) and factors associated with its occurrence in a cohort of extremely low birth weight infants (ELBW). We conducted a retrospective longitudinal study including ELBW infants followed-up for up to 10 years after discharge. EAR was defined as occurring before 5 years of age. A multivariate binary logistic regression analysis was performed to evaluate maternal and perinatal variables independently associated with EAR. Out of 212 ELBW infants included in the analysis, 40.6% developed EAR and 21.5% showed

Beyond Necrotizing Enterocolitis Prevention: Improving Outcomes with an Exclusive Human Milk-Based Diet.

OBJECTIVE: The aim of this study was to compare outcomes of infants pre and post initiation of a feeding protocol providing an exclusive human milk-based diet (HUM). MATERIALS AND METHODS: In a multicenter retrospective cohort study, infants with a birth weight <1,250 g who received a bovine-based diet (BOV) of mother's own milk fortified with bovine fortifier and/or preterm formula were compared to infants who received a newly introduced HUM feeding protocol. Infants were excluded if they had major congenital anomalies or died in the first 12 hours of life. Data were collected 2-3 years prio

Feasibility and safety of setting up a donor breastmilk bank in a neonatal prem unit in a resource limited setting: An observational, longitudinal cohort study.

BACKGROUND: The beneficial effects of human milk on decreasing rates of paediatric infections such as necrotizing enterocolitis (NEC) and sepsis have been clearly demonstrated. Donor breastmilk has been encouraged as the milk of choice when a mother's own breastmilk is not available. The objectives of this study were to assess feasibility of providing donor breastmilk to infants in a resource limited Neonatal Prem Unit (NPU). In addition we sought to determine whether donor breastmilk could be safely pasteurized and administered to infants without any adverse events. METHODS: Low birth weight

Effects of probiotics on enteric flora and feeding tolerance in preterm infants.

BACKGROUND: Probiotics are live microbes that colonize the gastrointestinal tract and benefit the host. Preterm infants develop abnormal patterns of bowel colonization, and only a few clinical trials have reported the outcomes of preterm infants treated with probiotics. PURPOSE: We investigated the rate of colonization of Lactobacillus and the clinical variables affecting the colonization in preterm infants. METHODS: Infants with gestational age less than 37 weeks treated at Ewha Womans University Hospital between March 2003 and July 2004 were eligible. Lactobacillus acidophilus (containing

Carbohydrate malabsorption in infants with diarrhea: diagnostic and evolutive aspects.

Twenty three infants with acute or protracted diarrhea were investigated for carbohydrate (CHO) malabsorption during their normal feeding schedules. All infants were fed a chicken-meat formula which contained rice flour and maltodextrin. End-tidal respiratory H2 concentrations were sequentially evaluated in different clinical settings and compared to fecal pH and reducing substances. Fecal pH below 6.0 and/or breath H2 greater than 25 ppm were considered evidence of CHO malabsorption, as these values disappeared while infants were submitted to a trial of CHO withdrawal per os, reappearing afte

Genomic insights into antimicrobial resistance and isolation source classification of Cronobacter sakazakii from infant and toddler food in the United States.

Cronobacter sakazakii is an opportunistic foodborne pathogen associated with severe infections in infants, linked to powdered infant formula (PIF) and related products. We conducted genomic profiling of C. sakazakii (n=209) from infant and toddler food in the United States, comprising all publicly available genomes for this source, through the integration of antimicrobial resistance (AMR) gene (ARG), plasmid replicon, virulence gene, phylogenetic, and pan-genome analyses. We further applied a machine learning (ML)-driven isolation source classification approach based on pan-genome features to …

Influence of pasteurization and spray drying on the fat digestion behavior of human milk fat analog emulsion: a simulated in vitro infant digestion study.

BACKGROUND: Human milk fat analog emulsion (HMFAE) is an emulsion that mimics the composition and structure of human milk (HM) fat globules. The application of HMFAE in infant formula requires a series of milk powder processing steps, such as pasteurization and spray drying. However, the effect of milk powder processing on fat digestion of HMFAE is still unclear. In this study, the influence of pasteurization and spray drying on the lipolysis behavior of HMFAE was studied and compared with HM using a simulated infant in vitro digestion model. RESULTS: Pasteurization and spray drying increased…

The "Breastfeeding Paradox" as a Guide for the Assessment of Premature Infants Growth: It Is More Than Just Weigh-Ins.

One of the neonatologist's greatest challenges is ensuring that the premature infant has adequate growth and is provided with a correct supply of nutrients. Thanks to the establishment of the INTERGROWTH-21st Preterm Postnatal Growth Standards, created longitudinally and prospectively on healthy premature babies, it now appears evident that preterms' growth follows a pattern different from that of a fetus of the same gestational age. In addition to growth, defined solely as weight gain, further significance must be given to the quality of growth, that is, lean mass apposition. This should be e

Chylous ascites following repair of total anomalous pulmonary venous connection coexisting with a persistent left superior vena cava in a neonate: a case report.

Chylous ascites refers to the accumulation of lymphatic fluid in the peritoneal cavity. The causes of chylous ascites are various, and commonly include traumatic injury and obstruction, which disrupt the lymphatic system. In addition, cardiothoracic surgery may injure the thoracic duct and lead to chylothorax. However, there are very few reported cases of isolated chylous ascites developing following cardiothoracic surgery. In this paper, we report a case of postoperative chylous ascites in a full-term neonate. The infant underwent cardiothoracic surgery via thoracotomy to repair total anomalo

The effect of subclinical infantile thiamine deficiency on motor function in preschool children.

We investigated the long-term implications of infantile thiamine (vitamin B1) deficiency on motor function in preschoolers who had been fed during the first 2 years of life with a faulty milk substitute. In this retrospective cohort study, 39 children aged 5-6 years who had been exposed to a thiamine-deficient formula during infancy were compared with 30 age-matched healthy children with unremarkable infant nutritional history. The motor function of the participants was evaluated with The Movement Assessment Battery for Children (M-ABC) and the Zuk Assessment. Both evaluation tools revealed st

Use of Gastrostomy Tube to Prevent Maternal PKU Syndrome.

Maternal Phenylketonuria Syndrome (MPKU) can occur in infants born to mothers with PKU with poor metabolic control during pregnancy. Elevated phenylalanine (phe) acts as a teratogen to the developing fetus with consequences including intellectual disability, microcephaly, facial dysmorphism, growth retardation, and congenital heart disease. MPKU can be prevented if metabolic control is achieved by 8-10 weeks gestation. If control is not achieved, there is a significant risk for MPKU. Therefore, in women with poor metabolic control at time of pregnancy, establishing metabolic control quickly is

Service uptake and performance of the prevention of mother-to-child transmission (PMTCT) programme in Ibadan, Nigeria.

The Prevention of Mother to Child Transmission (PMTCT) programme in the University College Hospital (UCH), Ibadan has been in existence for more than five years and has scaled up to other sites. The study evaluated the service uptake and performance of the programme using national key indicators. Antenatal and delivery records of women enrolled between July 2002 and June 2007 were reviewed. A total of 51952 women attended first antenatal visits and received HIV pre-test counselling. Of these, 51614 (99.5%) accepted HIV test and 49134 (95.2%) returned for their results. Out of the tested patien

Culture Optimization of the IPEC-J2 Piglet Jejunum Cell Line for Applications in Infant Nutrition Research.

The intestinal porcine epithelial cell line (IPEC-J2) is a nontransformed jejunal model derived from unsuckled piglets. It is increasingly used in gut physiology research, yet its application to nutrient absorption remains underexplored. Four media types were assessed for their ability to support IPEC-J2 barrier maturation and permeability: Dulbecco's modified Eagle medium/Ham's F-12 with 10% fetal bovine serum (FBS10), 10% porcine serum (PS10), 5% porcine serum (PS5) supplemented with Wnt3a-R-spondin-Noggin conditioned medium (PS5+L-WRN), and PS5 with epidermal growth factor and insulin-trans

[The introduction of gluten into the infant diet. Expert group recommendations].

At present there is a degree of uncertainty regarding when, how and in what form gluten should be introduced into the infant diet. For years the recommendations of the ESPGHAN Committee on Nutrition have prevailed, which include avoiding early introduction, before 4 months, and late, after 7 months, and gradually introducing gluten into the diet while the infant is being breastfed, with the aim of reducing the risk of celiac disease, diabetes and gluten allergy. However, 2 independent studies published in The New England Journal of Medicine in October 2014 reached the conclusion that the age o

Diversity of risk of mother-to-child HIV-1 transmission according to feeding practices, CD4 cell count, and haemoglobin concentration in a South African cohort.

OBJECTIVE: To estimate the probabilities of intrapartum and postpartum HIV-1 mother-to-child-transmission according to various feeding practices (formula feeding, exclusive breastfeeding, or mixed-feeding) and to other mother and infant covariates. METHODS: We used the promotion time model extended to multiple exposures to study the probability of infection attributable to each transmission occasion. Blood samples from 551 infants from Durban (South Africa) born to HIV-1 positive untreated mothers between 1995 and 1998 were sequentially tested until 15 months. RESULTS: The probability of inf

Iron-Fortified Foods Are Needed To Meet the Estimated Average Requirement for Iron in Australian Infants Aged 6 to 12 Months.

BACKGROUND: Meeting iron intake recommendations is challenging for infants 6-12 mo, especially breastfed infants. Three-quarters of Australian infants 6-12 mo have iron intakes below the estimated average requirement (7 mg), placing them at risk of iron deficiency. After 6 mo, breastmilk is no longer sufficient to meet the increased demand for iron, and iron-rich complementary foods are recommended. Iron-fortified foods may be a means of improving iron intake in infants, particularly those that are breastfed. OBJECTIVES: The aims of the study were as follows: 1) to examine the effect of milk-
