A therapeutic community for HIV-infected persons: an approach for prevention and control of AIDS.

The Therapeutic Community (TC) for HIV-infected persons is neither a hospital nor a confinement for people with HIV. Actually it is a family of friends who are in the same boat and a place of education to teach them about AIDS and relevant subjects necessary for their life in the future, and to help establish new values and life styles for them using TC tools which are adapted from the DAYTOP TC in U.S.A. The purpose was not only to build the residents, but also to encourage them to do their part in contributing to build up a better and safer society that will help to save the lives of others

Controlled clinical trial of acyclovir in chronic hepatitis B virus infection.

A randomised, controlled trial comparing acyclovir, 45 mg/kg/day as a continuous IV infusion for 28 days, with no other therapy, was carried out in 30 stable HBsAg carriers seropositive for HBeAg for more than 6 months. Twenty-eight had hepatitis B virus DNA-polymerase activity and/or hepatitis B virus DNA in serum at entry into the study. There were no significant adverse effects of therapy. At 12 months, seroconversion from HBeAg to anti-HBe had occurred in four of 15 treated patients, one of whom had also developed anti-HBs, compared with only one of 15 in the untreated group (95% confidenc

Infections and related risk factors of arthritis in children. A case-control study.

We used a parent-completed questionnaire to record the clinical signs of infection preceding the onset of joint symptoms by 1 month or less in 334 children with arthritis or other joint disease. An upper respiratory tract infection (URTI) often preceded the onset of arthritis (51%) but also arthralgia (56%) and even trauma or orthopedic disease (43%). The results of a case-control analysis of 165 patients and community controls matched for age and sex were in favour of a preceding URTI having an etiologic or triggering role in acute transient arthritis of other joints than the hip only (odds r

Controlled study of therapeutic granulocyte transfusions in granulocytopenic patients with severe infections.

The effectiveness of therapeutic granulocyte transfusions was studied in a controlled trial involving 75 granulocytopenic patients with severe infections. Patients who had granulocyte counts of less than 200/mm3 and no response to antibiotic therapy were assigned to receive antibiotic therapy alone or granulocyte transfusions plus antibiotic therapy. Granulocytes were collected by filtration leukapheresis (FL), intermittent flow centrifuge leukapheresis (IFCL) or continuous flow centrifuge leukapheresis (CFCL). Therapeutic effects of granulocyte transfusions were evaluated on the basis of 20-d

[Varicella-zoster virus infections in the immunosuppressed child. Treatment with Acyclovir and controls].

Sixty minutes intravenous infusions of Acyclovir were given at 5 to 10 mg/kg 3 times daily for 6 to 11 days in 20 immunodeficient children with varicella (9 cases) and herpes zoster (11 cases) in a controlled open study. Twelve patients had a life-threatening illness. All patients who received therapy before the first 4 days had a more rapid cessation of new vesicles formation and more rapid scarring. Fourteen controlled children had accelerated clearance of viral antigens from vesicles. In 19 cases, virus was no longer isolated after the 3rd day. Eighteen patients recovered within 6 to 10 day

A bundle of infection control measures reduces postoperative sternal wound infection due to Staphylococcus aureus but not Gram-negative bacteria: a retrospective analysis of 6903 patient episodes.

BACKGROUND: Prevention of cardiac surgical site infection has largely focused on reducing infection due to Staphylococcus aureus, although other bacteria also play an important role in this complication. AIM: To assess the impact of an evolving infection control programme on the incidence of sternal wound infection (SWI), and the changing incidence of non-staphylococcal infections. METHODS: A retrospective cohort study of all patients who underwent primary sternotomy at a single UK centre between September 2010 and May 2018 was undertaken. Data were collated from the 2 years preceding the st…

Novel insights into immune checkpoints in HIV/SHIV infection: from SHIVSF162P3-infected elite controllers to therapeutic strategy.

During chronic HIV-1 infection, the upregulation of immune checkpoints (ICs) on lymphocytes contributes to persistent immune dysfunction, T-cell exhaustion, and inadequate virological control, highlighting the potential role of ICs in HIV functional cure strategy. Despite extensive research on ICs, the expression dynamics and immunological impact of ICs in natural HIV/SHIV infection, particularly among antiretroviral therapy (ART)-free elite controllers (ECs), remain poorly understood. In this study, we monitored a cohort of SHIVSF162P3-infected rhesus macaques (RMs), identifying four ECs and

Association between the duration of diabetes and gram-negative bacterial infection in diabetic foot infections: a case-control study.

This retrospective case-control study was designed to explore the association between the duration of diabetes and gram-negative bacterial infection in diabetic foot infections (DFIs). All DFI patients hospitalized in the Department of Endocrinology in the Sixth Affiliated Hospital of Sun Yat-sen University between 2013 and 2019 with positive microbial culture results were included. Cases were defined as DFI patients whose microbial cultures grew gram-negative bacteria (including polymicrobial flora). Controls were defined as DFI patients whose positive microbial cultures did not grow gram-neg

Can Control Infections Slow Down the Progression of Alzheimer's Disease? Talking About the Role of Infections in Alzheimer's Disease.

Alzheimer's disease as the most common age-related dementia affects more than 40 million people in the world, representing a global public health priority. However, the pathogenesis of Alzheimer's disease (AD) is complex, and it remains unclear. Over the past decades, all efforts made in the treatments of AD, with targeting the pathogenic amyloid β (Aβ), neurofibrillary tangles, and misfolded tau protein, were failed. Recently, many studies have hinted that infection, and chronic inflammation that caused by infection are crucial risk factors for AD development and progress. In the review, we a

Comparison between antimicrobial stewardship program and intervention by infection control team for managing antibiotic use in neurogenic bladder-related urinary tract infection patients: A retrospective chart audit.

BACKGROUND: Antimicrobial prescriptions are relatively common in urologic outpatients. Therefore, it is necessary to investigate the impact of antimicrobial stewardship program (ASP) interventions. METHODS: In urology outpatients, antimicrobial use density (AUD), antimicrobial agent costs, isolation of urinary tract infection (UTI)-causing organisms and their antimicrobial susceptibilities were compared between intervention by infection control team (ICT) era (pre-2014) and ASP era (post-2014) in 2739 patients with lower urinary tract symptoms, including neurogenic bladder patients with UTI o…

Prior hip or knee prosthetic joint infection in another joint increases risk three-fold of prosthetic joint infection after primary total knee arthroplasty: a matched control study.

AIMS: There is little information regarding the risk of a patient developing prosthetic joint infection (PJI) after primary total knee arthroplasty (TKA) when the patient has previously experienced PJI of a TKA or total hip arthroplasty (THA) in another joint. The goal of this study was to compare the risk of PJI of primary TKA in this patient population against matched controls. PATIENTS AND METHODS: We retrospectively reviewed 95 patients (102 primary TKAs) treated between 2000 and 2014 with a history of PJI in another TKA or THA. A total of 50 patients (53%) were female. Mean age was 69 ye

[THE PROGRAM OF PREVENTION AND CONTROL OF NOSOCOMIAL INFECTIONS: WORLD EXPERIENCE AND RECOMMENDATIONS FOR IMPLEMENTATION].

The nosocomial infections that are related to provision of medical care continue to be one of the key problems of health care system conditioned by high morbidity, mortality and economic losses. According to the WHO, prevalence of such infections ranges from 7% to 15% among hospitalized patients. In countries with low-income this indicator can reach 20%. The article presents a review of modern data on prevalence, clinical characteristics and risk factors of nosocomial infections. The types of these infections, their economic consequences and issues related to resistance to of antimicrobial pre

Role of antimicrobial peptide-based biomaterials in respiratory tract infections control.

In experimental models, such as those of bacterial pneumonia and tuberculosis, AMP-containing systems promote bacterial clearance and reduce inflammatory responses more effectively compared to free AMPs. The use of AMP-containing biomaterials for treating infections offers several benefits over traditional antibiotics. Isolated AMPs are less likely to develop resistance due to their multiple mechanisms of action. Additionally, AMPs kill pathogens quickly and are likely more effective when combined with the body's defense systems. Commonly used antibiotics often lead to the development of resis

Source control in intra-abdominal infections: What you need to know.

Providing optimal source control (SC) for intra-abdominal sepsis (IAS) is a critically important surgical principle, yet one that remains nebulous in terms of strict definitions and required conduct. The entire concept of SC has evolved in the last decades. Contemporary SC is not only surgical but also embraces minimally invasive percutaneous and medical therapies. We propose that adequate SC has evolved from the mere anatomical control of enteric leakage, cleansing of obvious contaminants and necrosis, to a more comprehensive anatomo-phyiological-biochemical model. While any breaches in the i
