Fracture-dissociation of ceramic liner.

The use of BIOLOX delta ceramic (CeramTec AG, Plochingen, Germany) has been increasing. This ceramic prevents cracking by restraining the phase transformation due to the insertion of nano-sized, yttria-stabilized tetragonal zirconia into the alumina matrix. This restrains the progress of cracking through the formation of platelet-like crystal or whiskers due to the addition of an oxide additive. We observed a case of BIOLOX delta ceramic liner (CeramTec AG) rim fracture 4 months postoperatively. Radiographs showed that the ceramic liner was subluxated from the acetabular cup. Scratches on the

[Simple diagnostics of patellofemoral instability point to tailored treatment].

Three patients presented with patellar instability. On physical examination, the first patient, a 20-year-old male soccer player, reported pain at the medial insertion ofthe medial patellofemoral ligament (MPFL) after a traumatic patella luxation. He was successfully treated by MPFL reconstruction. The second patient, an 18-year-old woman, presented with patellofemoral pain. On physical examination, the patella could be luxated laterally over approximately half of its width. Conventional X-rays showed a crossing sign, indicating trochlear dysplasia. She was treated by trochleaplasty. The last

[Fractures of tibial shaft and tibial head in winter sports].

BACKGROUND: Injuries in alpine skiing often affect the lower extremities, in particular the knee joint and lower leg. In addition to conventional radiological diagnostics, CT and, in the case of fractures of the knee joint area, MRI also play an important role. In the case of tibial head fractures, especially if there has been a dislocation mechanism, there is an increased risk of vascular and nerve injuries. Lower leg fractures are predestined for the development of a compartment syndrome. For these reasons, an exact survey of the vascular-nerve status and a monitoring of the soft tissues sho

Calcinosis Cutis Circumscripta Of Knee-A Rare Presentation.

INTRODUCTION: Congenital insensitivity to pain (CIP) is a rare disorder, and often presents to an orthopaedic surgeon as recurrent fractures, dislocations, pseudoarthrosis, osteomyelitis etc. Here, we report a case of congenital insensitivity to pain presenting with distal femoral physeal separation in a child. CASE REPORT: A 12-year-old girl child came with complaints of limp while walking and swelling in the left knee for past 5 weeks. Mother gave a history that the girl is a known case of congenital insensitivity to pain with clear history of no pain on intramuscular injection since birth.

The docking technique for medial patellofemoral ligament reconstruction: surgical technique and clinical outcome.

BACKGROUND: Current techniques of medial patellofemoral ligament (MPFL) reconstruction vary with respect to methods of fixation on the femur and the patella. This article presents the outcomes of a surgical technique for reconstruction of the MPFL that uses a soft tissue graft with interference screw fixation on the femur and a docking technique for fixation on the patella. HYPOTHESIS: Patients with patellar instability who are treated with the docking technique for MPFL reconstruction will have improvements in knee symptoms and function, with a high percentage achieving good to excellent res

[Post-traumatic hemarthrosis of the knee joint--an indication for arthroscopy].

Diagnostic arthroscopy including the use of a probe should be performed if the cause for a haemarthrosis of the knee cannot be established by radiological means. The clinical examination of the recently injured knee must be considered to be unreliable due to pain. In approximately one-third of knee injuries an incorrect or incomplete diagnosis is made. Even if ligamentous injuries can be established, arthroscopy is mandatory, because this is the only sure way in which acute and degenerative meniscal or cartilaginous lesions can be diagnosed. Arthroscopy should be performed under general or spi

Case report. Microcephalic osteodysplastic primordial dwarfism type II: a child with unusual symptoms and clinical course.

UNLABELLED: We report on a 13-month old boy with microcephalic osteodysplastic primordial dwarfism (MOPD), whose radiographic signs correspond with type II of this entity. Some of his clinical signs, such as the anomalies of the external genitalia and the urinary tract, are common to this subgroup of MOPD, but he also shows unusual clinical signs including bilateral knee dislocation and hypoplasia of the anterior corpus callosum. His clinical course was unusual with several episodes of breathing difficulties and increased intracranial pressure secondary to craniosynostosis at the age of 16 mon…

Ambulation in patients with myelomeningocele: a multivariate statistical analysis.

Factors determining ambulation in 163 patients with myelomeningocele were studied by a multivariate statistical method. Neurological dysfunction unrelated to the plaque was analyzed by magnetic resonance imaging. There were no ambulators at the thoracic or L1-L2 level. At the L3 level, 54% ambulated, and at the L4 level, 67% ambulated. Eighty percent were ambulators at L5 and all at the sacral level. Below L1-L2, one-half of the nonambulators had neurological deficiencies caused by syringohydromyelia or Chiari malformations preventing ambulation. Severe scoliosis was closely, age moderately, a

Patellofemoral pain--a prospective study.

This prospective study confirms that patellofemoral pain arises predominantly in a young population and is more common in females. Patellar malalignment and its progression to patellar subluxation can be managed conservatively with improvement in the majority of cases. Arthroscopic lateral release may be necessary for selected patients with patellar malalignment and subluxation, while more aggressive surgical therapy should be considered in patients with patellar compression syndrome and recurrent dislocation. Resection of symptomatic plicae gave satisfactory results in all patients. Chondroma

Osteochondral Fragment from the Lateral Femoral Condyle in an Adult after Patellar Dislocation: A Surgical Challenge.

INTRODUCTION: Osteochondral lesions of the lateral femoral condyle (LFC) following patellar dislocation are relatively rare and typically observed in the adolescent population. In adults, such injuries-especially involving a large osteochondral fragment from the posterolateral aspect of the condyle-are exceedingly uncommon and pose both diagnostic and therapeutic challenges. CASE REPORT: We present the case of a 20-year-old male who sustained a traumatic right knee injury while playing basketball. Clinical examination and imaging revealed a large osteochondral fragment (2.5 × 2.5 cm) originat

Trochleaplasty for patellar instability due to trochlear dysplasia: A minimum 2-year clinical and radiological follow-up of 19 knees.

BACKGROUND: Recurrent patellar dislocation may be associated with trochlear dysplasia. Trochleaplasty is a surgical procedure which strives to deepen the trochlear groove. We evaluated the clinical and radiological effect of trochleaplasty after a minimum follow-up of 2 years. PATIENTS AND METHODS: We examined 19 knees in 16 patients at a mean of 3 years after trochleaplasty. Postoperatively, a subjective questionnaire, a Kujala score, and tests for potential patellar redislocation and apprehension were evaluated. On radiographs we evaluated the preoperative and postoperative crossing sign, t

[Total hip and knee arthroplasties in hemophilia. Apropos of 27 cases].

Thirteen total hip and 14 total knee prostheses were performed in 21 haemophilic patients. Nineteen had severe factor deficiency. Their average age was 43 years. Pain was the only indication for hip prostheses. All the hip arthropathies were of Arnold stage IV or V. For the knees, the indication was permanent pain with patello-femoral involvement in nine cases. In other cases, stiffness and instability was the indication. Seven knees, one out of two, had a flexion deformity of more than 30 degrees and only in four knees, one in three, was the range of flexion greater than 90 degrees. The quadr
