Expression profile of microRNA-200 family in cholangiocarcinoma arising from choledochal cyst.

BACKGROUND AND AIM: The risk of cholangiocarcinoma (cCC) arising from choledochal cyst (CC-CC) is imminent, if the latter not treated appropriately in time. Epithelial-to-mesenchymal transition (EMT) is considered a critical step for various solid cancers, which is regulated by the microRNA-200 (miR-200) family. The aim of this study was to assess the role of miR-200 family in the pathogenesis of CC-CC. METHODS: Sixteen patients with CC-CC were enrolled and 254 patients with conventional cCC served as clinicopathologic controls. Fifty-four cCC were selected to compare the miR-200 family expre

[Minimally invasive surgery in pediatric patients within a general urology department].

OBJECTIVES: The advances of minimally invasive surgery in urology over the last years have enabled a progressive and constant implementation of endourology and laparoscopy in pediatric patients. We perform a review of our experience, as a general hospital, with minimally invasive surgery performed in pediatric patients over the last ten years. METHODS: We retrospectively analyzed the endourological and laparoscopic operations performed between 1997 and 2007 in children up to the age of 16 years, collecting data about patient's age and gender, type of disease, techniques, anesthesia, and perio

Late choledochal pathology after cholecystectomy for cholelithiasis.

UNLABELLED: After "simple" cholecystectomy for lithiasis, biliary disorders can appear, with the onset more than 3 years postoperative, like cholangitis or transitory jaundice. Meantime, a whole range of congenital abnormalities initially ignored can become manifest: biliary tract congenital dilatations, duodenal para-Vater diverticulum, Oddi stenosis. AIM: to establish the pathological circumstances that determine late choledochal syndrome, including an analysis concerning the therapeutical approach in these cases. Patients with cholecystectomy complains of late biliary disorders (least 3 ye

Diagnostic criteria in predicting a biliary origin of acute pancreatitis in the era of endoscopic ultrasound: multicentre prospective evaluation of 213 patients.

BACKGROUND: No study on bioclinical criteria predicting a biliary origin for acute pancreatitis has included endosonography as a reference examination. Re-examination of bioclinical parameters deserves consideration in the era where other causes are known (e.g. hereditary, autoimmune). AIM AND METHODS: To determine the performance of bioclinical markers in predicting a biliary origin of acute pancreatitis where the diagnosis of biliary lithiasis was established or ruled out using endosonography. Only patients with a first acute episode of pancreatitis were included. RESULTS: 213 patients (ma

Asymptomatic nonspecific serum hyperamylasemia and hyperlipasemia: spectrum of MRCP findings and clinical implications.

We assessed the magnetic resonance cholangiopancreatographic (MRCP) findings in patients with asymptomatic, mild elevations of serum amylase and lipase levels to determine whether there might be a pathoanatomic cause for these laboratory abnormalities. MRCP was performed in 633 consecutive patients. Of these, 54 (8.5%) images were obtained in patients with asymptomatic serum hyperamylasemia and hyperlipasemia. MRCP was performed on a 1.0-T MR system; breath-hold gradient-recall, half-Fourier acquisition, and rapid acquisition with relaxation enhancement sequences were obtained. Findings were v

Recurrent Nephrolithiasis in Congenital Cystinuria With a Complete Duplex Collecting System: A Case Report.

Cystinuria is a rare autosomal recessive genetic disorder that affects the reabsorption of cystine in the renal tubules. Although cysteine lithiasis is a rare form of nephrolithiasis, it typically presents early in life and is associated with a highly recurrent course and frequent need for repeated surgical interventions. The presence of a duplicated collecting system adds complexity, as it can hinder drainage and make endoscopic access more difficult. We report the case of a 16-year-old male patient with congenital cystinuria and a complete bilateral duplex collecting system. He presented wit

Procalcitonin as a Predictor of Mortality in Patients With Severe Acute Pancreatitis.

BACKGROUND: Acute pancreatitis (AP) is a severe inflammatory disorder that begins with the inappropriate activation of pancreatic enzymes within acinar cells due to biliary reflux, alcohol abuse, gallstones, and autoimmune disease. Several biomarkers have been studied that may aid in the early detection of pancreatic necrosis. The aim of this project was to evaluate the usefulness of procalcitonin (PCT) in predicting mortality in patients with severe AP in Mexican population. METHODS: An observational study, including 59 patients diagnosed with AP from 2018 to 2023, was conducted in a tertiar

Gallstone Ileus. What therapeutic options are there?

UNLABELLED: Gallstone ileus is a rare disorder in emergency surgical practice with diagnosis usually difficult and only achieved at surgery. The current approaches are: enterolithotomy, cholecystectomy and fistula repair (one-stage surgery), enterolithotomy with cholecystectomy performed later (two-stage surgery) and only enterolithotomy (most reported surgical procedure). METHODS: The clinical, operative and follow-up data on 14 consecutive patients treated in our clinic for gallstone ileus was retrospectively reviewed. RESULTS: Gallstone ileus was recorded in 0.06% of all operations for bi

Hemorrhagic cholecystitis: ultrasound and CT imaging findings-a retrospective case review series.

PURPOSE: Gallbladder pathology is diverse, and imaging tests are essential tools for its diagnosis. Acute cholecystitis has multiple manifestations or complications, one of which is hemorrhagic cholecystitis (HC). In the current literature, this pathology has been described only in the spectrum of acute cholecystitis complications, case reports, or series with a maximum of 2 to 3 cases. After a retrospective review, we present 11 cases of hemorrhagic cholecystitis and discuss its various causes, clinical presentations, and findings on ultrasound (US) and computed tomography (CT), considering d

Ureterocystoplasty in pediatric patients with unilateral nonfunctioning kidney.

OBJECTIVE: Bladder augmentation with uroepithelium lined material yields an absence of mucus production, with reduced possibility of urinary infection and lithiasis. The utilization of the ureter in augmentation cystoplasty results in a uroepithelium-lined neobladder with all of the appropriate histologic layers. The purpose of this study was to assess the efficacy and safety of ureterocystoplasty in children with a small bladder capacity and low bladder compliance. MATERIAL AND METHODS: Between January 1992 and August 2011, six females and eight males who were 3 to 13 years old (median age 6

Gallbladder carcinoma: intraoperative imprint cytology, a helpful and valuable screening procedure.

BACKGROUND/PURPOSE: A major problem arises when a tumor is not readily recognized at the time of cholecystectomy. A carcinoma at early stages or one hidden by acute or chronic inflammation can be overlooked during surgery, and the diagnosis would then be made only after microscopic examination of paraffin-embedded tissue. The purpose of the present report is to communicate the results of 10 years' experience with the use of imprint cytology for the intraoperative diagnosis of gallbladder carcinoma. METHODS: During the period June 1994 to June 2004, 525 imprint cytologies of gallbladder mucosa

Similar performance but higher cholecystitis rate with covered biliary stents: results from a prospective multicenter evaluation.

BACKGROUND AND STUDY AIMS: Endoscopic biliary stenting is now a well-established treatment method in patients with unresectable malignant biliary obstruction. Despite advances with metal stents, the problem of stent occlusion has not yet been resolved. Covered metal stents could reduce the occlusion rate by preventing tumor ingrowth, but have not been well evaluated. A prospective multicenter study was therefore conducted to evaluate the efficacy and disadvantages of covered Wallstents. PATIENTS AND METHODS: Covered Wallstents were implanted endoscopically in 62 patients with inoperable dista

Standard surgical approaches to primary choledocholithiasis--definitive versus temporary decompression.

The occurrence of retained/recurrent calculi after primary CBDE followed by temporary T-tube decompression, have remained at rates varying from 5.4% to 20.9% over the last 10 years in spite of sophisticated pre and intraoperative imaging techniques. It is postulated that a functional obstruction, due to dysmotility of the SO, lies behind most stone-containing ducts. Thus it seems logical to us that a permanent "fenestration" should be the management of most such ducts. We prospectively followed-up, for one to 10 years, two groups of patients submitted to primary CBDE aiming to assess the short
