Inferior alveolar nerve function after open reduction and internal fixation of mandibular fractures.

PURPOSE: Mandibular fractures are amongst the most common facial fractures and are usually treated by open reduction and internal fixation (ORIF). Inferior alveolar nerve (IAN) injuries are seen frequently in mandibular fractures as well as after ORIF of these fractures due to the exposition and the close proximity of the nerve during fracture reduction. Therefore the continuity of the IAN can be disrupted. Permanent injury to the IAN can result in diminished quality of life. This retrospective study was designed to objectively analyse the incidence and the outcome of pre- and postoperative me

Reduced circ_lrrc49 in trigeminal ganglion contributes to neuropathic pain in mice by downregulating Ist1 and impairing autophagy.

Orofacial neuropathic pain is a common symptom induced by orofacial nerve injury caused by a range of trauma or dental and maxillofacial procedures but lacks effective treatment. Circular RNAs (circRNAs) participate in the regulatory processes of neuropathic pain. Nevertheless, the biological roles of circRNAs in orofacial neuropathic pain remain unexplored. In this study, circRNA sequencing and Real-time quantitative polymerase chain reaction (RT-qPCR) were carried out. Notably, a novel circRNA named circ_lrrc49 was identified to be downregulated following chronic constriction injury of the i

Comparison Between the Bone Lid Technique and the Traditional Technique in Surgical Treatment of the Posterior Mandibular Lesions: A Randomized Controlled Trial.

Introduction Cystic lesions of the jaws and the impacted teeth are two of the most common cases that require surgical intervention in oral and maxillofacial surgery; however, surgeons also frequently use a traditional technique that involves the removal of the buccal bone plate. This study was conducted to compare the clinical and radiologic outcomes of the bone lid technique and the traditional technique. Methods This randomized controlled trial included 20 patients who were randomly divided into two groups (n = 10 each): the T group, in which the lesions were accessed using the traditional t

Mitochondrial Bioenergetic, Photobiomodulation and Trigeminal Branches Nerve Damage, What's the Connection? A Review.

BACKGROUND: Injury of the trigeminal nerve in oral and maxillofacial surgery can occur. Schwann cell mitochondria are regulators in the development, maintenance and regeneration of peripheral nerve axons. Evidence shows that after the nerve injury, mitochondrial bioenergetic dysfunction occurs and is associated with pain, neuropathy and nerve regeneration deficit. A challenge for research is to individuate new therapies able to normalise mitochondrial and energetic metabolism to aid nerve recovery after damage. Photobiomodulation therapy can be an interesting candidate, because it is a techniq

Morphological assessment of the posterior superior alveolar artery using cone-beam computed tomography.

BACKGROUND: In dentistry and maxillofacial surgery, detailed knowledge of maxillary sinus anatomy and its relationship to adjacent structures is essential to reduce the risk of intraoperative complications, particularly during implant placement and sinus floor elevation procedures. This study aimed to assess the prevalence, diameter, and anatomical course of the posterior superior alveolar artery (PSAA), as well as its spatial relationship to the alveolar crest and the maxillary sinus floor, using conebeam computed tomography (CBCT) to support preoperative surgical planning. Endoscopic Removal of a Displaced Dental Implant From the Tail of the Inferior Nasal Concha.

Accidental displacement of dental implants into adjacent anatomical cavities is a rare but clinically significant complication of implant surgery. Migration into the nasal cavity, particularly into posterior regions such as the tail of the inferior nasal concha, is exceedingly uncommon and sparsely documented in the literature. Prompt radiographic evaluation and coordinated interdisciplinary management are essential in preventing secondary complications and determining the most appropriate treatment approach. We present the case of a 65-year-old woman who was referred after an accidental displ

Cone beam computed tomographic evaluation of temporal crest canal in the Turkish population.

BACKGROUND: The temporal crest, a prominent ridge descending from the coronoid process to the third molar region, defines the medial boundary of the retromolar triangle and plays a pivotal role in evaluating the temporal crest canal (TCC). The TCC, an uncommon variant of the mandibular canal, often originates posterior to the temporal crest and extends anteriorly, conveying the buccal nerve. Using cone-beam computed tomography (CBCT), clinicians can trace the TCC's path relative to the temporal crest. This relationship is critical in the retromolar triangle, where surgical interventions such a

Radiological analysis of maxillary artery relationships to key bony landmarks in maxillofacial surgery.

The maxillary artery (MA) is a key structure at risk of injury in numerous oral and maxillofacial surgical (OMS) procedures. Knowledge of safe distances from this vessel to surgically familiar bony landmarks could improve patient safety and prevent catastrophic haemorrhage. Distances between the MA and bony landmarks on the maxilla and mandible were measured using CT angiograms on 100 patients (200 facial halves). The vertical height of the pterygomaxillary junction (PMJ) was mean (SD) measurement of 16 (3) mm. The MA enters the pterygomaxillary fissure (PMF) a mean (SD) distance of 29 (3) mm

A 39-Year-Old Woman with Cervicofacial and Mediastinal Emphysema 10 Hours After an Elective Lower Molar Dental Extraction.

BACKGROUND Mediastinal emphysema, also called pneumomediastinum, is a very infrequent condition in patients reporting to an Emergency Department. Mediastinal emphysema is the pathologic presence of free air enclosing the mediastinal structures. In the case of chest pain in a young healthy adult, pneumomediastinum as a differential diagnosis will undoubtedly be omitted. This would be especially likely if the symptoms are not preceded by an injury, intense vomiting, or an infection in this area. CASE REPORT We present a case in which a previously healthy 39-year-old woman experienced painless pr

The benefit of surgical management in post-traumatic trigeminal neuropathy: a retrospective analysis.

Post-traumatic trigeminal neuropathy (PTTN) is a known complication of common oral and maxillofacial procedures. The burden on the patient and society is often underestimated. This retrospective study included 29 patients with PTTN who underwent surgical treatment. Symptoms were differentiated, pre- and postoperatively, into neuropathic discomfort and loss of perceptive function. Clinical and patient-reported outcomes were recorded. The Brief Pain Inventory questionnaire was completed at the last follow-up. The effect of different variables was evaluated through subgroup analysis. The mean tim

The nasofrontal beak: A consistent landmark for superior septectomy during Draf III drill out.

INTRODUCTION: Cerebrospinal fluid (CSF) leak occurs in 1-11% of endoscopic Draf III, or endoscopic modified Lothrop, procedures. CSF leak can occur during surgery during a superior nasal septectomy. This study investigated whether the posterior edge of the nasofrontal beak (NFB) at the level of the internal frontal ostium is a safe landmark to use to avoid skull base injury when beginning the superior septectomy. METHODS: Preoperative computed tomography maxillofacial scans were reviewed from 100 patients from the University of Pennsylvania sinus surgery data base. The narrowest anteroposteri

Antral bony wall erosion, trigeminal nerve injury, and enophthalmos after root canal surgery.

INTRODUCTION: The frequently used irrigant in dental surgery, sodium hypochlorite, is occasionally the cause of minor, usually circumscribed, adverse effects. Severe, extensive complications, with lasting sequelae, however, also can occur, as in the case we report herein. CASE REPORT: A 55-year-old woman underwent an endodontic procedure on a maxillary molar, whose roots, unknown to the surgeon, were protruding into the maxillary sinus. After sodium hypochlorite root canal irrigation, the patient immediately developed intense facial pain, facial edema, and periorbital cellulitis. An emergency

Review of unusual intraoperative and postoperative complications associated with endosseous implant placement.

OBJECTIVES: Common complications associated with dental implant surgery are well recognized and are usually explained to patients during the process of informed consent. For the general dental practitioner, the periodontist, and the oral and maxillofacial surgeon, it is relevant to also be familiar with less frequent complications. This review gathers unusual complications of this surgical procedure and presents unique complications from single case reports. METHOD AND MATERIALS: Studies were located using systematic searches in Medline and the Cochrane Library electronic databases. Key words

Traumatic epistaxis: Skull base defects, intracranial complications and neurosurgical considerations.

INTRODUCTION: Endonasal procedures may be necessary during management of craniofacial trauma. When a skull base fracture is present, these procedures carry a high risk of violating the cranial vault and causing brain injury or central nervous system infection. PRESENTATION OF CASE: A 52-year-old bicyclist was hit by an automobile at high speed. He sustained extensive maxillofacial fractures, including frontal and sphenoid sinus fractures (Fig. 1). He presented to the emergency room with brisk nasopharyngeal hemorrhage, and was intubated for airway protection. He underwent emergent stabilizati
