The Female Emergency Medical Services Experience: A Mixed Methods Study.

BACKGROUND: Emergency Medical Services (EMS) is a challenging profession. Little is known if there are gender differences in the experiences among EMS clinicians. Therefore, our aim was to understand and characterize the occupational experiences of female EMS clinicians. METHODS: A mixed methodological study was conducted among currently licensed female EMS clinicians via focus group and self-report survey data. Three focus groups (n = 5, 4, 13, respectively) were conducted with participants purposively recruited from primarily Northeastern EMS agencies. Through ongoing collaborations, a recr

The significance of breastfeeding on sleep patterns during the first 48 hours postpartum for first time mothers.

Postpartum women sleep less during the early days following delivery. Long-term sleep disturbance and fatigue can have significant implications for physical and mental health, relationships, employment and parental competence. Most studies focus on the quality and quantity of maternal sleep between weeks one and six postpartum. We explored sleep patterns and reasons for wakefulness of first-time mothers immediately following delivery. Thirty first-time mothers completed a postnatal sleep questionnaire, detailing their total sleep time (TST), the reasons for being awake and their level of fatig

[The so-called body-mind problem].

THE PROBLEM: Even psychosomatic researchers seem to want to avoid the so-called body-mind problem, which is actually a mind-brain problem. In line with Beckermann (2008), the first the four possible positions on the mind-brain problem are presented. The debate over the past 100 years has revolved around the question of whether mental events are ontologically independent of brain physiology or whether they are in fact entirely determined by it. Such a physicalism approach based on properties (i.e., mental characteristics or phenomena are physical or can be completely reduced to physical charact

Medical psychology in exercise and sport.

Several psychological outcomes that accompany acute and chronic exercise have medical significance. Transient reductions in somatic tension and subjective anxiety appear most reliable. Symptom abatement in moderate depression can occur with chronic exercise in a manner comparable to psychotherapy and may offer a better prognosis in some instances. Other cognitive, behavioral, and perceptual events associated with exercise may assist in managing mental health, and exercise has been successfully used as a therapeutic adjunct in a variety of psychiatric disorders. Regular exercise may also comple

Clinical assessment of decision-making capacity in acquired brain injury with personality change.

Assessment of decision-making capacity (DMC) can be difficult in acquired brain injury (ABI) particularly with the syndrome of organic personality disorder (OPD) (the "frontal lobe syndrome"). Clinical neuroscience may help but there are challenges translating its constructs to the decision-making abilities considered relevant by law and ethics. An in-depth interview study of DMC in OPD was undertaken. Six patients were purposefully sampled and rich interview data were acquired for scrutiny using interpretative phenomenological analysis. Interview data revealed that awareness of deficit and th

Monitoring treatment and managing adherence in schizophrenia.

The majority of patients with schizophrenia will be nonadherent at some point during their treatment, which is why clinicians must assess adherence at each visit and promptly address any barriers. Treatment adherence can be negatively impacted by substance use, cognitive impairment, complex treatment regimens, and antipsychotic-induced adverse effects. Clinicians must monitor these risk factors, particularly metabolic, neurologic, and other side effects, not only to improve adherence but also to protect patients from dangerous health conditions. Interventions that can address some barriers to

Quantifying the burden of emotional ill-health amongst patients referred to a specialist rheumatology service.

OBJECTIVES: (1) To determine the prevalence of emotional disorders (DSM IV depression, anxiety and panic disorders) amongst patients referred to a rheumatology out-patient service and the proportion of these detected by the rheumatologist. (2) To test the hypotheses that emotional disorders are associated with (i) broad categories of rheumatological diagnosis (systemic, inflammatory vs non-systemic, non-inflammatory), (ii) female gender, (iii) greater symptom burden and disability and (iv) markers of socio-economic deprivation. METHODS: A cross-sectional study was made of consecutive newly re

Sexually assaulted older women attending a U.K. sexual assault referral centre for a forensic medical examination.

There is a paucity of literature on sexual violence against older adults, particularly in the U.K. This work aims to identify and describe the characteristics of alleged sexual assault and subsequent forensic medical examination (FME) for older clients presenting to a large sexual assault referral centre (SARC) in Manchester over a ten-year period. A retrospective analysis of forensic case notes was conducted for all clients (n = 39) who had attended for an FME between 2007 and 2017, and who were aged 70 or over at the time of the alleged assault. The average age of clients was 83 years, all w

[Treatment against the patient's will exemplified by electroconvulsive therapy : Clinical, legal and ethical aspects].

BACKGROUND: Severe affective and psychotic disorders may be accompanied by legal incapacity. If in this case the patient refuses treatment and in parallel there is a risk of serious damage to health, treatment can be carried out against the patient's non-autonomous will under defined prerequisites. Due to its good and partly superior effectiveness in the treatment of severe and pharmacotherapy-resistant affective and psychotic disorders, electroconvulsive therapy (ECT) is an important treatment option in such constellations. AIM: Description of the general principles and prerequisites of ther

Neuropsychiatric symptoms in Alzheimer's disease: associations with caregiver burden and treatment outcomes.

BACKGROUND: Caregivers play a major role in providing care for patients with Alzheimer's disease (AD) and are themselves at higher risk of health comorbidities. AIM: To address the impact of neuropsychiatric symptoms of patients in different stages of AD on their caregivers' burden. DESIGN: This prospective study enrolled 260 AD patients with clinical dementia rating (CDR) of 0.5, 1 and 2 at a tertiary medical center. METHODS: All patients were tested using the mini-mental state examination (MMSE), the cognitive abilities screening instrument (CASI), the neuropsychiatric inventory (NPI) and

The association between giant hydrocele and depression in a rural clinic in Nigeria.

One of the dreaded disfiguring disease conditions among the Andoni tribesmen in the Nigerian Niger delta region is hydrocele, especially when its size is large (giant hydrocele) and it cannot be concealed. This case-control study was designed to evaluate the prevalence of depression among patients with giant hydrocele presenting to Bethesda Clinic Ngo, Andoni, Nigeria. A total of 52 patients were recruited into this study: 26 in the giant hydrocele group and 26 in the control group. Their age range was 23 to 78 years, with a mean age of 53.4 ± 15.5 years for the giant hydrocele group and 53.6

[The psychic cross-sectional finding, cardinal point of psychiatric-psychotherapeutic expertise].

Expertise is based on history and clinical findings. Anamnestic data and clinical results have to be differentiated carefully. Clinical findings are of special importance because they are helpful in verifying anamnestic information with regard to extent and impairment quality of complaints, plausibility and thereby their significance for the expertise. The author is of the opinion that the clinical-psychic finding is in danger of being put in the background. With the victory of the descriptive diagnosis and the international diagnostic systems ICD-10 and DSM-IV, some examiners are tempted to r

A cluster-randomised controlled trial evaluating the effects of a blended care intervention in pregnancy and postpartum on weight, health behaviour, and mental health in women with low socio-economic status.

OBJECTIVES: Pregnancy and the early postpartum period are key for maternal health, with excessive weight gain posing long-term risks. Women with low socio-economic status (SES) are at increased risk, but this group is often not reached. Mobile Health (mHealth) solutions, combined with face-to-face counselling, might better reach such a vulnerable population. The I-PREGNO study evaluated a blended care intervention to improve health behaviours and mental health in women with low SES during the perinatal period. STUDY DESIGN: This two-arm, cluster-randomised controlled trial included an interve

Emotional and Behavioral Problems in Children and Adolescents With Asthma and Their Association With Disease Severity.

INTRODUCTION: Asthma is a common chronic disease in childhood and is often associated with psychological comorbidities. This study aimed to assess emotional and behavioral problems in children and adolescents with asthma, comparing findings by disease severity. METHODS: An analytical cross-sectional study was conducted with children and adolescents (6-18 years) followed at a specialized asthma clinic. Patients were classified as having mild (GINA steps 1-2) or moderate/severe asthma (steps 3-5). Caregivers completed the Child Behavior Checklist for ages 6-18 (CBCL/6-18), validated in Portugue

Long-term diagnostic and social outcomes after first psychiatric hospitalization.

BACKGROUND: The early phases of severe mental disorders are often diagnostically challenging, with frequent diagnostic shifts over time. Few studies have combined detailed baseline diagnostic assessment with long-term follow-up to examine both diagnostic and social development. METHODS: We conducted a 20-year register-based follow-up of 150 patients with first-time psychiatric hospitalizations. At baseline, all participants underwent a comprehensive diagnostic assessment. Follow-up data were obtained through linkage to national registers, providing information on psychiatric diagnoses, educat

Beyond the digital panopticon: a mixed-methods examination of teacher burnout, psychological capital, and well-being through the lens of work-life balance.

INTRODUCTION: In the contemporary Chinese educational landscape, characterized by involution (neijuan) and increasing digital encroachment, the maintenance of educator well-being has become a critical challenge. This study investigated how teacher burnout and psychological capital (PsyCap) predict psychological well-being (PWB), with a specific focus on the mediating role of work-life balance (WLB). METHODS: An explanatory sequential mixed-methods design was employed. In the quantitative phase, survey data were collected from 842 primary and secondary school teachers across three Chinese prov

Living alone and risk of dementia, cognitive decline, and institutionalization in the MEMENTO cohort.

INTRODUCTION: Alzheimer's disease and related dementias (ADRD) represent a public health challenge, with prevention strategies focusing on modifiable risk factors such as isolation. Living alone is used as a proxy for social isolation, although its relationship with ADRD outcomes remains unclear, partly due to the distinction between objective isolation and subjective loneliness. This study examined the association between living alone and the risk of dementia, cognitive decline, and institutionalization in the MEMENTO cohort, a French clinic-based study of individuals with cognitive complaint
