Partial pharyngolaryngectomy with infrahyoid flap: Our experience.

AIM: We evaluated a cohort of advanced hypopharyngeal squamous cell carcinoma, treated with conservative surgery, reconstruction with infrahyoid flap and radio-chemotherapy. METHODS: We used partial pharyngo-laryngectomy and radio-chemotherapy to treat fifty-seven patients with stage III-IV hypopharyngeal SCC from November 1994 to December 2011. Clinical examination and speech therapy evaluation were used for estimation of laryngeal function. RESULTS: All patients received a partial pharyngo-laryngectomy. All patients underwent neck dissection; 56 patients received bilateral neck dissection.…

Pathologic features of occult lymphatic metastasis in supraglottic carcinoma.

OBJECTIVE: To study the pathologic features of occult lymphatic metastasis in supraglottic carcinoma. METHODS: Serial sections of 153 neck dissection specimens in 100 patients with supraglottic carcinoma were evaluated under the microscope. RESULTS: In 100 patients, 38 had occult metastatic lymph nodes. 51 metastatic lymph nodes were found in pathology, and their sizes ranged from 0.5 cm to 2.6 cm (average 1.1 cm). The distribution of 51 lymph nodes was 1 in level I (2%), 37 in level II (73%), 12 in level III (24%), and 1 in level IV (2%). Among the 51 nodes, 21 (41%) were early stage, 18 (3…

[Histopathological study of local invasion and cervical lymph node metastasis in transglottic carcinoma].

To study the histopathological characteristics of local invasion and cervical lymph node metastasis in transglottic carcinoma and provide theoretical guide to clinical practice, 30 laryngectomy and 35 neck dissection specimens were studied by serial section, HE staining and observation under light microscope. The results showed that transglottic carcinomas originated from the ventricle invaded supraglottic and glottic areas equally. The metastatic rate of cervical lymph nodes was 43.3%; the latent metastatic rate was 30%. The metastatic and extracapsular perforation rates as well as the number…

[Cervical artery dissection - retrospective analysis of 19 cases].

Cervical artery dissection is a common cause of stroke in young adults. It might occur shortly after a forceful neck trauma or a minor injury. However, spontaneous dissection is also common, which is associated with genetic, anatomical or environmental risk factors. Cervical artery dissection can produce a broad spectrum of clinical presentation varying from local symptoms to focal neurological deficits determined by the arterial territory involved. Early recognition is important since immediate initiation of treatment can significantly improve patient outcomes. While clinical features may rai

Balloon Assisted Extraperitoneal Laparoscopic Approaches.

Laparoscopic techniques may be applied to structures with an extraperitoneal locale, without the use of generalized intraabdominal gas insufflation. Pelvic lymphadenectomy, bladder neck suspension, inguinal hernia repair, and renal access may be accomplished outside the abdominal cavity. This approach allows local or regional anesthesia to be applied in a variety of laparoscopic procedures. Formation of a working cavity via blunt dissection of the peritoneum away from the organ of interest permits instrument insertion and manipulation in the extraperitoneal space. Peritoneal displacement is pe

[Traumatic vertebral artery dissection causing basilar artery occlusion].

Acute basilar artery occlusion is a potentially fatal disease that can cause brainstem and cerebellar strokes. Traumatic vertebral artery dissection is one of the most common causes of basilar artery occlusion in young patients. We present the case of a 23-year-old woman with vertebral artery dissection complicated by basilar artery occlusion 4 days after a traffic accident that caused severe neurological deficits, cortical blindness. Computed tomographic angiography showed total occlusion of the basilar artery, and right vertebral artery dissection from the third to the fourth segments. Crani

Ischaemic stroke from dissection of the craniocervical arteries in childhood: report of 12 patients.

Dissection of craniocervical arteries is the most common non-atherosclerotic cause of stroke in young adults. During childhood, it is described primarily as isolated reports. Among 59 patients with arterial ischaemic stroke seen consecutively in the same institution, 12 had a dissection of a cervical or cerebral artery. The diagnosis was established through imaging features. The dissection involved the cervical arteries in five patients and intracranial arteries in seven. A cervical or facial trauma preceded the onset of cerebral ischaemic symptoms in four patients with extracranial dissection

Vertebral Artery Dissection.

A 40-year-old woman was referred to physical therapy with complaints of headaches. Examination raised suspicion of a "thunderclap headache," a condition characterized by sudden, intense headaches correlated with bleeding in and around the brain. The patient was referred to a neurologist, who ordered magnetic resonance angiography of the head and neck, which identified a partial dissection of the right vertebral artery. A subsequent computed tomography angiogram confirmed the dissection. J Orthop Sports Phys Ther 2020;50(6):344. doi:10.2519/jospt.2020.8858.

Cerebral hyperperfusion syndrome: a novel presentation of internal carotid artery dissection.

Cervical artery dissection (CeAD) occurs preferentially in the middle-aged, and its annual incidence rate is 2.6 to 3.0 per 100,000.(1) Manifestations of internal carotid artery dissection (ICAD) include ischemic stroke and TIA (>70% of patients), headache, neck pain, Horner syndrome, cranial nerve palsy, pulsatile tinnitus, and, rarely, subarachnoid hemorrhage.(2) Cerebral hyperperfusion syndrome is known to occur after carotid artery revascularization procedures and it is thought to result from the combination of several factors that impair cerebral vascular autoregulatory mechanisms.(3.)

Photophobia as the Presenting Symptom of Internal Carotid Artery Dissection.

A 53-year-old man developed right eye photophobia after a hypertensive crisis, followed by right-sided headache and retro-orbital pain. On examination, a right Horner syndrome was evident and an urgent brain and neck Computed Tomography confirmed the suspicion of an internal carotid artery (ICA) dissection. While photophobia may occur in many neurological disorders, to our knowledge, it has never been described when the ICA is involved. Possible hypotheses of photophobia caused by carotid artery dissection are discussed.

Preauricular transparotid approach to mandibular condylar fractures without dissecting facial nerves.

Preauricular transparotid approach without dissecting the facial nerve was used for surgical treatment of 15 condylar fractures in 14 patients. The parotid fascia was opened just above the fracture site, and by dissecting the parotid gland and masseter muscle, the fracture was directly exposed. The facial nerve itself was not dissected expressly. All fractures could be reduced accurately and fixed firmly with miniplates. A direct approach just above the fracture site provided good vision of the fracture, avoiding facial nerve palsy caused by strong retraction. Moreover, by not dissecting the f

Vertebral artery dissection as the initial manifestation of primary antiphospholipid syndrome in a man.

Antiphospholipid syndrome (APS) is a prothrombotic autoimmune disorder that is associated with arterial and venous thrombosis. Cervical artery dissection is an uncommon cause of stroke and has rarely been reported with APS. We report the case of a 46-year-old man who presented with acute dizziness, vertigo, neck pain, and headache after jujitsu training. Neuroimaging showed right vertebral artery dissection from V1 to V4, and magnetic resonance imaging revealed a right cerebellar infarction. Evaluation revealed persistently elevated lupus anticoagulant titers on repeat testing at 12-week inter

Extracranial internal carotid artery dissection caused by compression from a giant osteophyte due to atlantoaxial osteoarthritis: case report.

Deformed osseous structures have been reported as rare causes of extracranial internal carotid artery (ICA) dissection, including the styloid process and the hyoid bone. Here, the authors describe the first known case of symptomatic ICA dissection caused by a giant osteophyte due to atlantoaxial osteoarthritis. The left ICA was fixed at the skull base and at the ICA portion compressed by the osteophyte, and it was highly stretched and injured between the two portions during neck rotation. The patient was successfully treated with ligation of the affected ICA following balloon test occlusion. A

Pulsatile tinnitus and carotid artery dissection.

Carotid artery dissection is an uncommon entity associated with head and neck pain, partial Horner's syndrome, amaurosis fugax, and brain ischemia, which may all occur in isolation or in combination. Herein, we report a rare case of cervical artery dissection in which pulsatile tinnitus was the only reported symptom. A 38-years-old man attended our hospital with a 4-days history of left side pulsatile tinnitus which began after stumbling. He had no other symptom. MRA showed luminal stenosis with pseudo lumen of the internal carotid artery. The patient was diagnosed with left internal carotid a

Lymph node metastasis in level IIb in oropharyngeal squamous cell carcinoma: a multicentric, longitudinal, retrospective analysis.

INTRODUCTION: Nowadays, 70% of patients in Europe and the USA are affected by a p16 + , potentially HPV driven oropharyngeal squamous cell carcinoma. However, despite the improved survival rate in this group, the quality-of-life remains low in cases which neck dissection took place. In this vein, in recent years, some surgeons have considered to avoid dissection of level IIB, proposing a supra-selective non-IIb neck dissection. MATERIALS AND METHODS: A retrospective, longitudinal, multicentric study was conducted, including patients with pathologically confirmed primary HPV + or HPV - OPSCC w…

[Differentiated thyroid gland carcinoma p-T2/T3--extent of lymphadenectomy].

Systematic lymphadenectomy, which is compartment-orientated, from central node dissection to (modified) radical neck dissection, is not controversial in cases with intra-operative macroscopic node involvement. General "prophylactic" dissection, at least of the ipsilateral central compartment, is advocated due to a high incidence of "occult", microscopic positive nodes, and the elevated risk regarding recurrency and survival which is connected to node-positivity, and lowered recurrence rates with systematic lymphadenectomy. Nevertheless, the biological impact of occult positive nodes, as an ind…
