Effects of topical corticosteroids on the sciatic nerve: an experimental study to adduce the safety in treating carpal tunnel syndrome.

Despite known detrimental effects on the blood flow and histology of nerves after intraneural corticosteroid injection, the neurotoxic effec … ted and control nerves were not significant after 2 weeks. Topical dexamethasone adversely affected neural conduction in a dose-dependent manner. Our results suggest that caution is required when using large doses of corticosteroid for injection of the carpal tunnel.

Sox10 is required for Schwann cell identity and progression beyond the immature Schwann cell stage.

Mutations in the transcription factor SOX10 cause neurocristopathies, including Waardenburg-Hirschsprung syndrome and peripheral neuropathies in humans. This is partly attributed to a requirement for Sox10 in early neural crest for survival, maintenance of pluripotency, and specification to several cell lineages, including peripheral glia. As a consequence, peripheral glia are absent in Sox10-deficient mice. Intriguingly, Sox10 continues to be expressed in these cells after specification. To analyze glial functions after specification, we specifically deleted Sox10 in immature Schwann cells by

BSER abnormalities in a multiple sclerosis patient with normal peripheral hearing acuity.

The measurement of brain stem evoked potentials has proven clinically useful in determining the presence of multiple sclerosis plaques along … aring acuity (documented over the last six years), BSER test results yielded significantly abnormal neural conduction times when the left ear was ipsilaterally stimulated. A histopathologic examination of the patient's brain stem verified involvement of the auditory tracts. Thus physical corroboration of the abnormal BSER results was obtained in the presence of nor …

Central obesity and fat-free mass are associated with a larger spleen volume in the general population.

UNLABELLED: Transcutaneous auricular vagus nerve stimulation (taVNS) has emerged as a promising neuromodulation technique for enhancing cognitive functions such as working memory (WM), yet its neural mechanisms remain unclear. In this study, fifty healthy young adults were randomly assigned to an active taVNS group or a sham group in a single-blind, sham-controlled design. Participants performed semantic and spatial 2-back WM tasks before and after stimulation. Hemodynamic activity was measured by functional near-infrared spectroscopy (fNIRS), autonomic activity was assessed by heart rate vari

[Development and regeneration of oligodendrocytes: therapeutic perspectives in demyelinating diseases].

The function of the central nervous system (CNS) is in great part depending on glial cells as, for instance, radial glial cells give rise to cortical neurons, and oligodendrocytes synthesize an immense specialized membrane that enwraps axons to make myelin internodes. Myelin allows fast saltatory conduction of action potentials along myelinated nerve tracts and assures the survival of axons. Oligodendrocytes precursors (OP) emerge during development, first in the spinal cord and later in the telencephalon from multipotential neural precursors in germinative zones around the cerebral ventricles

Effects of 4-aminopyridine as an Adjuvant Therapy Following Peripheral Nerve Repair in an Animal Model of Nerve Transection Injury.

Peripheral nerve repair is considered the gold standard treatment for complete nerve transection injuries, yet achieving satisfactory functional recovery remains challenging due to muscle atrophy during the time required for axonal regeneration. This study investigated the beneficial effects of 4-aminopyridine (4-AP), a potassium channel blocker, on neural and muscular recovery.Following complete transection of the right sciatic nerve, 40 mice underwent end-to-end nerve repair using microscopic epineural sutures and were randomly assigned to either the control or 4-AP groups immediately after

Effect of Nimodipine and Botulinum Toxin A on Peripheral Nerve Regeneration in Rats: A Pilot Study.

BACKGROUND: Peripheral nerve damage is a frequent problem, with an estimated 2.8%-5.0% of trauma admissions involving peripheral nerve injury. End-to-end, tension-free microsurgical repair (neurorrhaphy) is the current gold standard treatment for complete transection (neurotmesis). While neurorrhaphy reapproximates the nerve, it does not address the complex molecular regenerative process. Evidence suggests that botulinum toxin A (BTX) and nimodipine (NDP) may improve functional recovery, but mechanisms of action remain unknown. METHODS: This research investigates BTX and NDP for their novel c

Pridopidine in the treatment of Huntington's disease.

Huntington's disease (HD) is a highly common inherited monogenic neurodegenerative disease, and the gene responsible for its development is located in the 4p16.3 chromosome. The product of that gene mutation is an abnormal huntingtin (Htt) protein that disrupts the neural conduction, thus leading to motor and cognitive disorders. The disease progresses to irreversible changes in the central nervous system (CNS). Although only a few drugs are available to symptomatic treatment, 'dopamine stabilizers' (as represented by the pridopidine) may be the new treatment options. The underlying causes of …

Subclinical median nerve alterations after distal radius fractures: a comparative analysis of volar plate fixation and cast immobilization using dynamic ultrasonography and electrophysiology.

BACKGROUND: Distal radius fractures may affect the median nerve through trauma-related and treatment-related mechanisms. While overt neurological deficits are uncommon, subclinical median nerve alterations may occur following both conservative and surgical treatment. PURPOSE: To determine whether treatment modality influences subclinical median nerve behavior using combined dynamic ultrasonographic and electrophysiological assessment. METHODS: This retrospective observational study included 42 patients with unilateral distal radius fractures. 18 patients were treated conservatively with circ

Immunomodulatory effects of a multi-component pharmacological intervention on diabetic peripheral neuropathy in type 2 diabetic rats: An exploratory study.

BACKGROUND: Diabetic peripheral neuropathy (DPN) is a common complication of type 2 diabetes mellitus (T2DM) and is closely linked to immune and inflammatory dysregulation. Multi-component pharmacological interventions have been explored as complementary approaches for metabolic and immune modulation; however, their effects on DPN and related mechanisms remain incompletely understood. METHODS: A rat model of T2DM-associated peripheral neuropathy was established, and a multi-component pharmacological intervention (MPCI) was administered for 8 weeks. Peripheral nerve dysfunction was evaluated b

The Diagnostic and Prognostic Role of Ultrasonography in Cubital Tunnel Syndrome: Results on a Consecutive Series of 100 Patients.

BACKGROUND AND OBJECTIVES: High-resolution ultrasonography (US) has become increasingly popular in cubital tunnel syndrome (CubTS). In this article, we confirmed the diagnostic role of US in a large and homogeneous cohort of patients. Our primary aim was to analyze the predictive role of US findings. By systematically re-evaluating neurosonographic abnormalities over time, we also sought to assess the role of US during follow-up. METHODS: Patients diagnosed with CubTS based on clinical, electrophysiological, and sonography findings and operated on for in situ decompression of the ulnar nerve

Biomarkers, Neurophysiological and Clinical Correlates Following Botulinum Toxin Treatment in Various Clinical Presentations of Cervical Dystonia: A Controlled Study Across Peak and Waning Response Phases.

Cervical dystonia (CD) is a focal dystonia typically treated with botulinum toxin type A (BoNT-A), but objective biomarkers of disease state and treatment response are limited. In a prospective controlled observational study with within-subject phase comparison, we evaluated clinical, neurophysiological, and biochemical markers at two time points (waning and peak response phases) within the BoNT-A injection interval in 30 CD individuals (with caput, collis and mixed patterns) and 25 healthy controls. We measured clinical status (TWSTRS and VAS pain), neurophysiology (F-wave minimal latency (F-

Quantitative sensory testing-based sensory phenotypes align with measures of peripheral nerve damage in patients with painful polyneuropathies.

Quantitative sensory testing (QST) is increasingly used to stratify patients with peripheral neuropathic pain into sensory phenotypes that may reflect distinct underlying pain mechanisms. Yet, their correspondence with objective measures of peripheral nerve damage remains unclear. This study investigated whether QST-based phenotypes are associated with objective measures of peripheral nerve damage in patients with painful polyneuropathy. We retrospectively analyzed data from 130 patients with painful polyneuropathy, who underwent nerve conduction studies, distal leg skin biopsy, and QST at the

Evolution of Nerve Conduction Demyelinating Parameters Between Baseline and Treatment Cessation May Not Be Predictive of Relapse in IVIg-Responsive CIDP Patients.

INTRODUCTION/AIMS: Identifying factors associated with short-term relapse is essential for tailoring maintenance treatment on a case-by-case basis in chronic inflammatory demyelinating polyradiculoneuropathy (CIDP). We sought to determine whether changes in demyelinating electrophysiological features during treatment could predict relapse after treatment discontinuation in a cohort of CIDP patients. METHODS: We conducted a retrospective study of patients with CIDP treated with intravenous immunoglobulin (IVIg), all of whom had a first nerve conduction study (NCS) at treatment initiation or du

Characterization of sensory nerve conduction abnormalities at the time of ALS diagnosis.

OBJECTIVES: Although sensory nerve abnormalities have been reported in amyotrophic lateral sclerosis (ALS), their distribution at diagnosis, relative to motor involvement and clinical onset phenotype remains incompletely characterized. We aimed to systematically compare sensory and motor nerve conduction abnormalities at ALS diagnosis and determine whether sensory involvement follows an onset-dependent pattern similar to motor dysfunction. METHODS: In this prospective cross-sectional study, 40 newly diagnosed ALS patients enrolled in the Iran University ALS Registry (March 2022-March 2023) un

The Enigma of the Motor Nerve Conduction Study.

BACKGROUND: In motor nerve conduction studies (MNCS), proximal stimulation should give a longer duration and lower amplitude compound muscle action potential (CMAP) due to higher temporal dispersion. Yet the CMAP waveforms at the distal and proximal stimulation sites appear remarkably similar. The objective of this study was to confirm this anomaly and investigate its possible cause by studying the median and ulnar nerves. METHODS: Recordings from 50 subjects with normal electrodiagnostic studies were reviewed. The conduction velocity (CV) was measured using different points on the negative p

Spatial heterogeneity of restitution properties and the onset of alternans.

Traditionally, it was believed that cardiac rhythm stability was governed by the slope of the restitution curve (RC), which relates the duration of an action potential to the preceding diastolic interval. However, a single RC does not exist; rate-dependence leads to multiple distinct RCs. We measure spatial differences in the steady-state action potential duration (APD), as well as in three different RCs: the S1-S2 (SRC), constant-basic-cycle-length (BRC), and dynamic (DRC), and correlate these differences with the tissue's propensity to develop alternans. The results show that spatial differe
