Appendiceal Mucinous Neoplasm: Nurse Education About a Rare and Complex Disease.

BACKGROUND: Appendiceal mucinous neoplasms (AMNs) are a rare and complex spectrum of disease involving a mucinous tumor within the appendix, which can range from benign to malignant. If not limited to the appendix, the mucinous tumor can spread diffusely throughout the peritoneum. OBJECTIVES: Because of the low incidence of AMN, this study describes the diagnosis and treatment process for this disease, which is not well known to most care providers. METHODS: An extensive analysis of the current literature, including incidence, diagnosis, and surgical treatment, was performed. A review of the

Induced hypothermia for neonatal hypoxic-ischemic encephalopathy: pathophysiology, current treatment, and nursing considerations.

Hypoxic-ischemic encephalopathy (HIE) can lead to devastating neurodevelopmental consequences such as cerebral palsy, seizure disorders, and significant developmental delays. HIE in the newborn is often the result of a hypoxic event, such as uterine rupture, placental abruption, or cord prolapse. Biphasic brain injury occurs in HIE. The first phase involves activation of the sympathetic nervous system as a compensatory mechanism. The second phase, known as reperfusion brain injury, occurs hours later. Induced hypothermia, a neuroprotective strategy for treating HIE, targets the second phase to

Creating environments of care with transgender communities.

Partnerships between transgender individuals and community health nurses have been a primary source of monitoring and responding to the impact of the HIV epidemic on transgender communities, specifically transgender women. This article provides two perspectives: first, from a transgender service provider, and second, from a public health nurse, on forming partnerships that brought consumers and providers together to create environments of care in which many transgender persons living with and at high risk of HIV were able to engage with medical providers who believed in their right to self-det

Case management services for injured workers. Providers' perspectives.

The perceptions of service providers involved in case management services are described here. The service providers included claims managers, occupational nurse consultants, attending physicians, and nurse case managers. The purposes of this phase of the study were to describe these providers' perceptions about the case management program; to organize findings according to the quality assessment model that guided this study (including structure, function, and outcomes); and to identify barriers and facilitators to satisfaction with case management services. Structural factors that affected ser

Management of retained placenta using intraumbilical oxytocin injection.

Manual removal of the placenta carries significant risk of hemorrhage and infection plus the risks associated with general anesthesia, if used. Transporting the patient from home or birthing center to hospital or from birthing room to delivery room or operating room is also disruptive to the patient and the initial parent-infant attachment process. The injection of oxytocin into the umbilical vein is a safe procedure that can cause placental separation and delivery, thus preventing the need for manual removal for some women. This technique can be useful in a nurse-midwifery practice in the man

Seasonal affective disorder, the depression of winter: a literature review and description from a nursing perspective.

This article presents a literature review and description of seasonal affective disorder (SAD). SAD is a newly recognized disorder, differentiated from other affective illnesses by its seasonality (with midwinter dysthymia and spring/summer remission), and its atypical symptoms of hypersomnia and hyperphagia. SAD symptoms remit with travel towards the equator or with bright light treatments, possibly indicating a chronobiological circadian etiology. This article introduces aspects of SAD that are amenable to the nursing process, including assessment of the seasonality of dysthymic symptoms; di…

Elimination concerns with acute spinal cord trauma. Assessment and nursing interventions.

Elimination for the spinal cord-injured patient presents a challenge to nurses in both the acute and rehabilitation phases. The most frequent GI and urinary complications associated with spinal shock are gastric dilatation and paralytic ileus, stress ulcers, metabolic disturbances, and neurogenic bowel and bladder. Associated clinical findings are often altered or absent from the lack of sensory, motor, and reflex functions. Nonspecific signs and symptoms such as anorexia, nausea, and vomiting, create the need for complex differential diagnosis. Critical care nurses have a vital role in this d

Nursing care of the patient with spinal cord injury.

The rehabilitation process must begin at the onset of illness and include not only a cure for the immediate disorders but methods of preventing further disability, the general management of convalescence, and the ultimate return of the individual to his maximum health and efficiency. As a member of the rehabilitation team the nurse must recognize the patient as a human being and emotionally accept him, maintain good interpersonal relations with other team members, know what resources are available, and help to provide continuity of care when the patient is discharged. She must counsel and guid

Views on Everyday Life and Reflections on Future Death and Dying Among Older Persons Residing in Nursing Homes: A Meta-Ethnographic Synthesis.

INTRODUCTION: Life issues relating to identity, meaning and dignity are inherent to human existence and remain significant throughout the entire lifespan. To meet the needs of older persons residing in nursing homes, it is essential to understand their views on everyday life, particularly regarding the proximity of death, reflections on future death and dying, and the potential for meaning in everyday life. However, these views have received limited attention in research. The aim of this study was therefore to explore older persons' views regarding everyday life and reflections on their future
