Late onset of serologic positive titers in a patient with Parinaud's oculoglandular syndrome.

PURPOSE: To report a case of Parinaud's oculoglandular syndrome (POS) in which, despite the initially negative, a late onset of serologic positive titers was observed. METHODS: Case report. RESULTS: A 65-year-old man was examined for a foreign body in the left eye associated with granulomatous nodule on the palpebral conjuctiva, chemosis and a swollen preauricular lymph node. Despite the initially negative serologic titers to Bartonella henselae, a clinical diagnosis of POS was made and treatment was instituted initially with oral ciprofloxacin (500 mg twice a day). Three weeks later, serolo

[Pathogenesis of main objective neurological symptoms in Chiari malformation type I].

To study the influence of Chiari malformation (CM) size, patient's age, hydrocephaly and intracranial hypertension on the disease manifestations, 263 patients with CM type I without concomitant syringomyelia have been examined. All the patients underwent clinical neurological examination, craniography and magnetic resonance imaging of the brain and spinal cord. Five main groups of objective neurological signs were identified: oculomotor (17.1%); cochlear vestibulocerebellar (84.8%); bulbar (35.4%); conduction motor (25.9%); segmental motor sensory disturbances (9.5%). The data obtained suggest

Contribution of cerebellum and brainstem in the control of eye movement: evidence from a functional study in a clinical model.

The idiopathic cerebellar ataxias (IDCA) comprise a wide spectrum of neurodegenerative diseases with heterogeneous neuropathology, characterized by the negativity of search for any known genetic mutation. On the basis of both their clinical presentation and their magnetic resonance imaging pattern, patients with IDCA can be subdivided into patients with a purely cerebellar syndrome and atrophy of the cerebellum (IDCA-C) and patients with additional noncerebellar symptoms and atrophy of both cerebellum and brainstem (IDCA-P). The aim of the present study was to evaluate the disaggregated contri

Ophthalmologic complications after intraoral local anesthesia with articaine.

A series of 14 cases of ophthalmologic complications after intraoral anesthesia of the posterior superior alveolar nerve is presented. The most commonly encountered symptoms were diplopia, mydriasis, palpebral ptosis, and abduction difficulties of the affected eye. In all cases, these effects occurred a few minutes after injection of the anesthetic, followed by complete resolution without sequelae on cessation of the anesthetic effect. The pathogenic mechanism underlying such ophthalmologic disorders is discussed in terms of a possible diffusion of the anesthetic solution toward the orbital re

Benign aqueductal cyst causing bilateral internuclear ophthalmoplegia after external ventricular drainage. Case report.

The introduction of magnetic resonance (MR) imaging to the field of neuroimaging has allowed detection of various lesions that cause aqueductal stenosis. The authors report the case of a 3-year-old boy in whom a benign ventricular cyst developed in the aqueduct. The patient became drowsy after having complained of headache and vomiting; MR imaging revealed mild triventricular dilation and a normal-sized fourth ventricle. Repeated MR imaging performed 1 week later revealed an aqueductal cyst that had markedly enlarged during the intervening period. An external ventricular drainage system was in

A sensitive and semi-quantitative pediatric myoclonus evaluation scale.

One of the difficulties in evaluating myoclonus in childhood is the lack of a standardized scale which addresses the constraints of pediatric scoring. The scale needs to be simple and rapid as well as sensitive and semi-quantitative and applicable across different ages. We videotaped children and young adults with myoclonus of various etiologies, such as progressive myoclonus epilepsy, opsoclonus-myoclonus and other acquired etiologies, and scored the videotapes using a new scale. Each clinical type of myoclonus (spontaneous, action and sensory-evoked myoclonus) was graded separately on a five

Motor neuron disease with slow eye movements and vertical gaze palsy.

Two middle-aged patients with motor neuron disease showed common eye movement disorders and intellectual impairment in the later stage of the illness. Eye movement disorders were characterized by slow saccades and vertical glaze palsy, which seemed to be supranuclear ophthalmoplegia. Neuropathological examination of one patient revealed degeneration of the substantia nigra without pathological changes in the ocular motor nuclei, in addition to findings compatible with motor neuron disease. These cases appear to raise a possibility of a distinct group of multiple system degenerations.

Ocular motor abnormalities from head trauma.

Head injuries cause the hospitalization of 200-300 persons per 100,000 population per year. Ophthalmologists provide diagnostic and therapeutic care to those trauma victims with damage to the globe, optic nerve, orbit, and ocular motor system. Eye movements can be affected by damage at any level of the central nervous system or peripheral motor unit. Comprehensive ocular motor assessment of the trauma patient can substantially contribute to the understanding of the patient's injury, recovery, and rehabilitation. This review examines all aspects of head and face trauma that can lead to ocular m

Exploring eye movement abnormalities as objective biomarkers for Parkinson's disease utilizing virtual reality-based eye tracking.

BACKGROUND: Oculomotor abnormalities are recognized as potential biomarkers in Parkinson’s disease (PD). However, their broader clinical adoption can be hindered by the accessibility and practicality of assessment tools. OBJECTIVE: This study evaluates a virtual reality (VR)-integrated eye-tracking system for characterizing oculomotor function in PD. We aimed to comprehensively assess oculomotor performance in PD patients compared to healthy controls (HC), investigate its relationship with clinical measures of disease severity, and develop a diagnostic model based on multi-paradigm eye movemen

Comparison of subjective cyclovertical deviation and objective ocular torsion in sagging eye syndrome and superior oblique palsy.

Purpose: To compare the differentiating clinical features of subjective cyclovertical deviation and objective ocular torsion in elderly patients with cyclovertical sagging eye syndrome (CSES) versus superior oblique palsy (SOP). Study design: Prospectivestudy. Methods: Patients with CSES (n = 22) and unilateral SOP (n = 20) aged ≥50 years were included. Subjective cyclovertical deviation was measured in the primary, secondary, and third-gaze positions using a synoptophore. The disc-fovea angle (DFA), which evaluates objective ocular torsion, was measured using fundus photographs. Results: The

Postinfectious SARS-CoV-2 Opsoclonus-Myoclonus-Ataxia Syndrome.

BACKGROUND: The opsoclonus-myoclonus-ataxia syndrome (OMAS) represents a pathophysiology and diagnostic challenge. Although the diverse etiologies likely share a common mechanism to generate ocular, trunk, and limb movements, the underlying cause may be a paraneoplastic syndrome, as the first sign of cancer, or may be a postinfectious complication, and thus, the outcome depends on identifying the trigger mechanism. A recent hypothesis suggests increased GABAA receptor sensitivity in the olivary-oculomotor vermis-fastigial nucleus-premotor saccade burst neuron circuit in the brainstem. Therefor

MRI topography of lesions related to internuclear ophthalmoplegia in patients with multiple sclerosis or ischemic stroke.

BACKGROUND AND PURPOSE: Internuclear ophthalmoplegia is a dysfunction of conjugate eye movements, caused by lesions affecting the medial longitudinal fasciculus (MLF). Multiple sclerosis (MS) and ischemic stroke represent the most common pathophysiologies. While magnetic resonance imaging (MRI) allows for localizing lesions affecting the MLF, comprehensive comparative studies exploring potential different spatial characteristics of lesions affecting the MLF are missing until now. METHODS: We retrospectively investigated MRI examinations of 82 patients (40 patients with MS and 42 patients with

Diplopia in Parkinson's disease: visual illusion or oculomotor impairment?

INTRODUCTION: Approximately 20% of patients with Parkinson's disease (PD) experience diplopia; however, the cause of the diplopia is unclear. We aimed to explore the association of diplopia, and its subtypes, with oculomotor abnormalities, impaired vision, and visual hallucinations, in patients with PD. METHODS: This exploratory study included 41 PD patients, recruited from two general hospitals, of whom 25 had diplopia and 16 did not have diplopia, as well as 23 healthy controls (HCs). We defined subtypes of diplopia as selective diplopia, i.e., diplopia of single objects, and complete diplo

Strabismus and Micro-Opsoclonus in Machado-Joseph Disease.

We describe novel deficits of gaze holding and ocular alignment in patients with spinocerebellar ataxia type 3, also known as Machado-Joseph disease (MJD). Twelve MJD patients were studied. Clinical assessments and quantitative ocular alignment measures were performed. Eye movements were quantitatively assessed with corneal curvature tracker and video-oculography. Strabismus was seen in ten MJD patients. Four patients had mild to moderate intermittent exotropia, three had esotropia, one had skew deviation, one had hypotropia, and one patient had moderate exophoria. Three strabismic patients ha

Surgical Excision of Orbital Cysticercosis Lodged in Superior Oblique Muscle: Clinical Case Report.

Ocular cysticercosis refers to parasitic infections in humans. Most cases were treated by medicine. The case we reviewed was rarely reported with successful surgical intervention treatment. This case report describes a patient with cysticercosis existing in superior oblique tendon. The main symptom of the patient was recurring history of painless orbital swelling and double vision in upgaze. Ocular motility examination revealed a restriction of the right eye in levoelevation. A contrast-enhanced computed tomographic scan of the orbit revealed the presence of a well-defined hypodense cystic lesi

Iatrogenic inferior oblique palsy: intentional disinsertion during transcaruncular approach to orbital fracture repair.

Hypotropia following orbital fracture repair is traditionally attributed to residual tissue entrapment, scarring, direct muscle injury, or damage to the branches of the oculomotor nerve serving the inferior oblique or inferior rectus muscles. We present a case of acquired hypotropia and incyclotropia that occurred following repair of an orbital fracture involving the floor and medial wall. In order to enable adequate visualization and treatment of the combined fractures, access via a transcaruncular approach and disinsertion of the inferior oblique muscle at its origin was necessary. Whereas t

Rectus muscle flap tear as an independent cause of restricted motility.

Most published cases of rectus muscle flap tear have been associated with orbital trauma of various degrees of severity. When they accompany an orbital fracture, however, it is difficult to determine whether the flap tear is merely an incidental additional finding or a major contributing cause of the resulting restriction. How to treat the flap itself remains an open question. We report a 24-year-old man with an inferior rectus muscle flap tear caused by direct laceration of the muscle. The major finding was a "reverse leash" vertical restriction. Discarding the flap instead of reattaching it
