Use of Joshi External Stabilizing System in postburn contractures of the hand and wrist: a 20-year experience.

Postburn contractures of the hand and wrist can range from a minor cosmetic problem to a crippling condition. The contractures, initially limited to the skin, extend to all the soft-tissue structures over time, often necessitating capsulotomies and tendon lengthening, although they still may not be amenable to total correction. The Joshi External Stabilizing System (JESS) is a versatile, lightweight external fixator consisting of K wires, distractors, and connecting rods (both hinged and nonhinged) along with various link joints. JESS is a dynamic system that allows the lengthening of the cont

Nerve injuries in total hip arthroplasty.

Nerve injury occurs in 1% to 2% of patients who undergo total hip arthroplasty and is more frequent in patients who need acetabular reconstruction for dysplasia and those undergoing revision arthroplasty. Injury to the peroneal division of the sciatic nerve is most common, but the superior gluteal, obturator, and femoral nerves can also be injured. Nerve injury can be classified as neurapraxia, axonotmesis, or neurotmesis. The worst prognosis is seen in patients with complete motor and sensory deficits and in patients with causalgic pain. Prevention is of overriding importance, but use of ankl

Clinical experiences with a convertible thermoplastic knee-ankle-foot orthosis for post-stroke hemiplegic patients.

As rehabilitation for post-stroke hemiplegic patients has become widely accepted practice, there has been an increase in patients who are more difficult to treat. In the prescription rationale of orthoses for hemiplegics, the knee-ankle-foot orthosis (KAFO) for the lower limb has generally been underestimated because of its inhibitory effect on the normal walking pattern and also its interference with gait training. The authors had an experience of 28 hemiplegics with severe physical impairments who were fitted with a convertible plastic KAFO. Among these patients, there were 11 cases in which

Nonoperative management of posterior tibial tendon dysfunction.

Forty-nine patients with posterior tibial tendon dysfunction (4 patients had bilateral involvement) were treated with orthoses. Forty feet were treated with molded ankle-foot orthoses, and 13 feet were treated with University of California Biomechanics Laboratory shoe inserts with medial posting. A total of 37 women and 12 men were included in the study. The mean follow-up period was 20.3 months (range, 8-60 months). The average age of the patients was 66 years (range, 42-89 years). Sixty-seven percent of patients had good to excellent results, according to a functional scoring system based on

Multi-adjustable post-operative orthosis for congenital muscular torticollis.

A multi-adjustable torticollis orthosis is described for the post-operative bracing of patients after surgical correction of congenital muscular torticollis. The orthosis can be put on in the early post-operative period and the head and neck position can be maintained in the corrected, and later over-corrected position by the built-in multi-adjustable joint-mechanism. The details of the manufacturing are described. Twenty-five patients (13 girls and 12 boys) from age 1 to 22 with congenital muscular torticollis were fitted with the orthosis post-operatively for an average duration of 10 weeks.

Osteotomies of the great toe.

Patients are operated on for forefoot problems at an increasingly younger age, according to the author's experience. This has resulted in extra articular osteotomies of the first phalanx of the great toe, usually associated with other forefoot procedures. There exist several indications for these osteotomies. Among them are hallux valgus, hallux rigidus, and other forefoot disorders, such as rheumatoid diseases. Since 1984, the author has performed 2850 great toe osteotomies, allowing him to specify indications, to elaborate osteotomy procedures (varisation, derotation, shortening) and to devi

Fracture of the pars interarticularis in adolescent athletes: a clinical-biomechanical analysis.

Stress fracture of the pars interarticularis is an increasing cause of disability in highly competitive adolescent athletes. We have documented this lesion in 14 adolescent athletes engaged in repetitive training and competition exercises involving flexion/extension of the lumbar spine. An in vivo spinal muscle torque study in 11 normal adolescent girls also revealed mean torque values of 22.6 Nm for left lateral flexion and 27.4 Nm for hyperextension. The most common sports resulting in this lesion were gymnastics and hockey. In four patients the lesion was bilateral, and in 10 it was unilate

Incidence of full containment of the femoral head after Legg-Calvé-Perthes disease and in the "normal" hip.

Forty-eight children with unilateral Legg-Calvé-Perthes disease were studied to determine the extent of femoral head containment present after treatment. Only 0-4% of the hips treated with bed rest and 10-35% of the contralateral hips were fully contained. The criterion of full containment for a good result is, therefore, not valid.

Clinical symptoms and biomechanics of lumbar spine and hip joint in leg length inequality.

A simple and reliable low dose radiologic method developed by the author was used to measure leg length inequality of 798 patients with chronic and therapy resistant low-back and/or unilateral hip symptoms and 359 symptom free subjects. Statistically highly significant correlations of the symptoms and leg length inequality were observed. In the majority (79 and 89%), the chronic or recurrent sciatic pain and unilateral hip symptoms occurred on the side of the longer lower extremity. When correcting the leg length inequality simply with an adequate shoe lift, a permanent and mostly complete all

[The osteomyelitis of the hip in newborns and its functional treatment with "extensionsreposition" and "Hanausek-Retention" (author's transl)].

Since 1969 all our patients below the age of three years with congenital dislocation of the hip are treated by a modified overhead-traction (Extensionsreposition) and the "Hanausek-Apparat" for retention. Because of good results of careful reduction and retention in a mitigated Lorenz-position we also treated osteomyelitis of the hip-joint in newborns and following dislocation by this method. It is reported of 3 cases, different types of osteomyelitis of the hip and the method are described.

Frequency of purchase and associated costs of assistive technology for Washington State Medicaid program enrollees with spina bifida by age.

BACKGROUND: Assistive technology (AT) is one strategy to mitigate or eliminate barriers to independence for individuals with disabilities, including those with spina bifida (SB). However, little is known about current use and costs of AT for people with SB, including the cost burden to medical insurance payees. OBJECTIVE: The aim of this study was to evaluate frequency of AT purchases and their associated costs for individuals with SB covered by the Washington State Medicaid program. Additionally, we sought to compare Medicaid reimbursement for AT to the overall Medicaid reimbursement for all

Objective outcome analysis of soft shell helmet therapy in the treatment of deformational plagiocephaly.

Deformational plagiocephaly, cranial asymmetry secondary to positioning, continues to be a leading cause of head shape abnormalities in infants. Treatment recommendations include nonintervention, positioning therapies, and helmet therapy. Although most agree that surgical intervention is rarely indicated, the ideal therapy is not agreed on. Some even debate the necessity of treatment, especially third-party payers. The purpose of this prospective study is to use an objective outcome analysis tool, computerized tomography, to assess the efficacy of a soft shell helmet therapy. Sixty-nine childr

A Lightweight Multi-Articular Passive Exoskeleton Using a Single Elastic Band to Improve Crouch Gait Pattern: A Pilot Study.

Crouch gait, characterized by excessive flexion of the lower-limb joints, is a common gait disorder among children with cerebral palsy (CP) that compromises mobility and increases energy expenditure. Although surgical and orthotic interventions can improve alignment, maintaining these benefits requires continued postural support and gait practice. While powered exoskeletons have been developed to support upright and coordinated movement, they are often too heavy and complex for practical use. To address this, we developed a lightweight passive exoskeleton that assists the hip, knee, and ankle

Influencing Factors on Independent Walking in Children With Lumbosacral Lipomas: A Retrospective Cohort Study Based on a 5-Year Untethering Series.

BACKGROUND: Caregivers are deeply concerned about children achieving independent walking, and evidence-based rehabilitation support is beneficial. However, current research is confined to a single study on spina bifida aperta, leaving a gap in understanding the timing of independent walking for lumbosacral lipomas. OBJECTIVES: This study aimed to examine the factors influencing independent walking in children with lumbosacral lipomas. DESIGN: Retrospective cohort study. METHODS: This retrospective cohort study included 124 children who underwent untethering surgery for lumbosacral lipomas.

Effect of observation on lower limb prosthesis gait biomechanics: Preliminary results.

BACKGROUND: The Hawthorne effect, a subcategory of reactivity, causes human behavior to change when under observation. Such an effect may apply to gait variation of persons with prosthetics or orthotics devices. OBJECTIVES: This study investigated whether the presence of observers directly affects the gait pattern of users of lower limb prostheses. STUDY DESIGN: Within-subject intervention study. METHODS: Primary outcome measures were gait parameters of initial double support time and upper body lateral tilt angle, which were collected with a mobile sensor attached to the subjects' back. To

Does ankle support orthosis and kinesio tape affect evertor muscles strength and their ability to control force signal in chronic ankle instability? A randomized controlled trial.

BACKGROUND: Ankle musculature serves as active dynamic stabilizers, yet conventional assessment focusing solely on peak torque fails to capture critical aspects of neuromuscular control. The ability to generate accurate, steady forces is paramount for functional joint stability. External supports may modulate sensorimotor function, potentially improving force control. OBJECTIVE: To compare the effectiveness of bracing and taping on the quantity and quality of evertor muscle force. METHODS: Sixty participants (18-40 years old) with chronic ankle instability, were randomly allocated to 3 group

Therapy Considerations for Getting Athletes to Return to Play.

During the protective phase of treatment, therapy for hand and wrist injuries in athletes is similar to the plan of care provided to all patients. The nuances in the care provided to athletes become apparent during the transition to the postprotective phase of rehabilitation when the focus has shifted to return to play. Therapy following a sports injury should address the individual needs of the athletes in their everyday lives as well as the specificity of their training and sports-specific activities. The factors that influence return to play are discussed.

The Influence of Body Habitus on Documented Brace Wear and Progression in Adolescents With Idiopathic Scoliosis.

BACKGROUND: Although studies have been published stating obese patients are less compliant with brace use, no objective studies measuring hours of daily brace wear have been performed to correlate brace wear and success with body mass index (BMI). The purpose of this study was to establish the relationship between BMI and brace compliance, and between BMI and progression to surgical magnitude of 50 degrees or more. METHODS: A total of 175 adolescents were prescribed thoracolumbarsacral orthosis for the treatment of an adolescent idiopathic scoliosis and followed to completion of treatment. BM
