CLIPPERS is a rare, chronic inflammatory neurological syndrome affecting multiple regions of the brain including the brainstem, cerebellum, and spinal cord. More than 100 cases have been documented globally since its initial description in 2010. This article reports the first case of the CLIPPERS syndrome in Nepal. Clinical and radiological evidences of the patient lead to the diagnosis of this disease. Brain MRI reveals punctate and curvilinear gadolinium enhancement in the pons and cerebellum, which is diagnostic for the disease. Steroid therapy has been reported to be effective in treating …

INTRODUCTION: We report on a 4-year-old female patient who presented with severe intellectual disability, autistic features, hyperlaxity of joints, and progressive scoliosis. Whole-exome sequencing identified a de novo missense variant (c.976C>T; p.Arg326Cys) in DDX3X.
CASE PRESENTATION: The girl was born with congenital diaphragmatic hernia a finding which had not previously been associated with variants in DDX3X. Her brain MRI showed hypogenesis of corpus callosum, ventriculomegaly, frontal and perisylvian polymicrogyria, and hypoplastic pons in addition to Dandy-Walker malformation.

BACKGROUND: Bleeding of brainstem cavernous malformations (BSCM) cause high morbidity and should be treated surgically whenever possible.
METHOD: We present a 56-year-old man, who was diagnosed with a BSCM at right pons, which caused functional impairments of dorsal column, spinothalamic tract, cochlear nucleus, and middle cerebellar peduncle. A transmastoid presigmoid retorlabyrinthine approach via the lateral pontine zone (LPZ), with an assistance of imaging guidance and intraoperative neurophysiological monitoring, was performed to completely resect the BSCM. The patient recovered despite …

Vertebral artery dissection (VAD) followed by basilar artery occlusion/stroke (BAO/BAS) is a rare but potentially life-threatening complication. We present a case report of a 7-year-old boy with VAD complicated by BAO/BAS 4 days after falling off a scooter. Symptoms included left-sided weakness and facial droop preceded by a 20-minute episode of altered sensorium. Magnetic resonance imaging showed ischemic changes in the left posterior inferior cerebellum and right pons. Computed tomography angiogram confirmed dissection of the left vertebral artery with occlusion/thrombosis of the basilar art…

We report a case of acute disseminated encephalomyelitis (ADEM) concomitant with polyneuropathy associated with anti-lactosylceramide antibody. A 68-year-old man was admitted to our hospital with ophthalmoparesis, bulbar palsy, tetraplegia after suffering from upper respiratory infection and headache. Subsequently, he developed respiratory failure requiring mechanical ventilation. Fluid-attenuated inversion recovery (FLAIR) MRI showed high intensities in the pons and medulla, and a nerve conduction study revealed motor-dominant axonal polyneuropathy. Although the laboratory tests revealed the …

BACKGROUND: postoperative intracerebral hemorrhage after drainage of chronic subdural hematoma is a rarely reported complication; however, its incidence, according to different series may be underestimated.
CASE REPORT: this report presents a 77 year old male patient who, after the drainage of bilateral chronic subdural hematomas, developed an extensive hemorrhage in the thalami, basal ganglia, midbrain and pons, with extension into the ventricles and obstructive hydrocephalus.
CONCLUSIONS: compression by extra-axial collection decreases cerebral blood flow on the affected hemisphere and alt…

We describe an 8-year-old girl who presented with cranial nerve compression due to a brainstem cyst adjacent to the fourth ventricle and describe the first documented insertion of a grommet to form a conduit between a neuroepithelial cyst and ventricle. The patient presented with diplopia and headaches and was found to have the cystic lesion in the right pons. The patient underwent craniotomy, aspiration and fenestration with subsequent recurrence 8 months later. Definitive treatment involved insertion of a grommet. Surgical treatment of symptomatic neuroepithelial cysts can achieve full resol…

The involvement of facial mimicry in different aspects of human emotional processing is widely debated. However, little is known about relationships between voluntary activation of facial musculature and conscious recognition of facial expressions. To address this issue, we assessed severely motor-disabled patients with complete paralysis of voluntary facial movements due to lesions of the ventral pons [locked-in syndrome (LIS)]. Patients were required to recognize others' facial expressions and to rate their own emotional responses to presentation of affective scenes. LIS patients were select…

The issue of recurrence of vestibular schwannomas is poorly studied by the surgical literature and is probably underestimated. Our own long-term retrospective analysis after translabyrinthine approach has indicated a 9.2% recurrence rate. This long-term event is mainly due to regrowth of microfragments that have been left in the operative field along the course of the facial nerve or at the surface of the pons. Management of recurrence depends on the tumor size and patient's condition. Our current policy is to propose a Gamma Knife radiosurgical treatment in eligible cases. Prospective long-te…

Neurocircuitry models of panic disorder have hypothesized that the panic attack itself stems from loci in the brainstem including the ascending reticular system and respiratory and cardiovascular control centers. Voxel-based morphometry with acobian modulation was used to examine gray matter volume changes in 10 panic disorder patients and 23 healthy controls. The panic disorder patients had a relatively increased gray matter volume in the midbrain and rostral pons of the brainstem. Increased ventral hippocampal and decreased regional prefrontal cortex volumes were also noted at a lower signif…

The present study was performed to assess cerebral glucose metabolism in patients with panic disorder using positron emission tomography. F- … of glucose uptake in the bilateral amygdala, hippocampus, and thalamus, and in the midbrain, caudal pons, medulla, and cerebellum than controls. These results provided the first functional neuroimaging support in human patients for the neuroanatomical hypothesis of panic disorder focusing on the amygdala-based fear network.

A 59-year-old woman had chronic hyponatremia from inappropriate secretion of antidiuretic hormone (SIADH) and malnutrition after recurrent cholecystitis for 2 months. She developed dysarthria, dysphagia, bilateral ptosis, clonic convulsions and delayed onset Parkinsonian features. Magnetic resonance imaging showed increased signal density in the central pons on T2-weighted images. She was also later diagnosed as having systemic lupus erythematosus (SLE). This case is reported because central pontine myelinolysis (CPM) developed in chronic hyponatremia without correction, and manifested with at…

Images of nitroxide radicals were obtained from the brains of living rats following intracarotid injection of imaging agent, [2-(14-carboxytetradecyl)-2-ethyl-4,4-dimethyl-3-oxazolidinylox y] (16 DS) using L-band electron spin resonance computed tomography (ESR-CT). The image patterns obtained from the 3-carbamoyl-2,2,5,5-tetramethyl-1-pyrrolinyloxy injected rats showed the agent in tissues of the cranium but not within the brain. The uptake of 16 DS was found in the cortex, hippocampus, striatum, midbrain, pons medulla oblongata and cerebellum, and nuclei, mitochondria, synaptosomes of the ce…

The effects of subarachnoid hemorrhage on immunoreactivities of protein kinase C isozymes (alpha, beta, gamma) were studied in canine brain and basilar artery. Mild and severe constriction of the basilar artery was shown 2 days and 7 days after hemorrhage, respectively. In control brain tissues, the isozymes showed distinct distributions and following hemorrhage, reactive astrocytes with protein kinase C alpha staining emerged in the surface of the pons and hippocampus. The control basilar artery showed protein kinase C alpha staining and its staining was decreased on day 7, consistent with th…

Plasma big endothelin (BE) levels were studied in 17 patients during the first 2 weeks following spontaneous intracerebral hemorrhage (ICH). Three patients had experienced an infratentorial hemorrhage of the pons and cerebellum. The group of 14 patients with supratentorial hemorrhage presented with 8 hematomas of the putamen, 4 of the capsula interna and 2 in the thalamus. Significant elevations of BE during the first 2 weeks after the initial ICH, significant differences between the different localisations and between patients with different prognoses were not apparent (mean BE level: 2.95 +/…

We report on a 47-year-old female patient with Sheehan's syndrome who developed delayed-onset dystonia following recovery from central pontine myelinolysis. The dystonia was observed in the head, neck and limbs during active movement. MRI scan demonstrated a discrete lesion in the central pons without involving the basal ganglia. The action dystonia was markedly improved by giving trihexyphenidyl. The patient showed that generalized dystonia can be induced by an isolated pontine myelinolysis. There have been four other cases reported in the literature. On reviewing all five cases, we conclude …

We present a case of neuromyelitis optica (NMO) in a previously healthy 64-year-old woman. While CT scan and MRI investigations of the brain as well as myelography were normal, CSF revealed a pathological increase of IgG. Notwithstanding an intensive steroid therapy the patient died after 4 1/2 months. Microscopic examination showed, besides extensive demyelination of the optic nerves and spinal cord (especially in its cervico-dorsal tract), several small demyelinating plaques in the white matter of the pons and near the right lateral ventricle. These findings confirm once again that NMO is a …

In a review of 16 cases, the authors emphasize that small brainstem haemorrhages, diagnosed by CT-scan, can have a good outcome, most often spontaneously. Twelve hematomas were in the pons, four in the mesencephalon. Several clinical features were remarkable: consciousness was not or moderately impaired, focal symptoms and signs predominantly neuro-ophthalmologic were present. Involvement of the cranial nerves and long tracts occurred rarely in isolation. Arterial hypertension was the usual cause (50 p. cent); one normotensive patient with neurological disorders prior to the bleeding had an ar…
