Common sleep disorders increase risk of motor vehicle crashes and adverse health outcomes in firefighters.

STUDY OBJECTIVES: Heart attacks and motor vehicle crashes are the leading causes of death in US firefighters. Given that sleep disorders are … rder including obstructive sleep apnea (OSA), 28.4%; insomnia, 6.0%; shift work disorder, 9.1%; and restless legs syndrome, 3.4%. Compared with those who did not screen positive, firefighters who screened positive for a sleep disorder were more likely to report a motor vehicle crash (adjusted odds ratio 2.00, 95% CI 1.29-3.12, p = 0.0021) and were more likely to self-re …

Narcolepsy in Parkinson's disease.

Non-motor symptoms in Parkinson's disease (PD), such as excessive daytime sleepiness, 'sleep attacks', insomnia, restless legs syndrome and rapid eye movement sleep behavior disorder, are common and provide a challenge to treatment. These sleep symptoms are also described in patients suffering from the sleep/wake disorder, narcolepsy. The International Classification of Sleep Disorders (ICSD-2) narcolepsy criteria uses a number of markers for diagnosis, of which lack or deficiency of cerebrospinal fluid (CSF) hypocretin-1 levels is a key marker. Hypocretin neurons prominently located in the la…

Research criteria for the diagnosis of chronic inflammatory demyelinating polyneuropathy (CIDP) were proposed by an Ad Hoc Subcommittee of t … d CNS demyelination, four (5%) had predominant cranial nerve involvement, and one (1%) had only the restless legs syndrome. An associated medical condition that may have been responsible for the acquired demyelinating neuropathy was present in 60% of the patients. Moderate to severe periodic limb movement disorder in childhood and adolescence.

The purpose of this study is to review clinical features of children with moderate to severe Periodic Limb Movement Disorder (PLMD). Because of our interest in both Restless Legs Syndrome (RLS) and Attention-Deficit Hyperactivity Disorder (ADHD), many of our patients had one or both of these conditions. We did a retrospective review of 129 children and adolescents who were found to have Periodic Limb Movements in Sleep (PLMS) > 5/hour of sleep. Sixty five had PLMS of 5-10/hour of sleep, 48 had PLMS of 10-25/hour of sleep and 16 had PLMS > 25/hour of sleep. One hundred and seventeen of the orig…

Correlation between K complex, periodic leg movements (PLM), and myoclonus during sleep in paraplegic adults before and after an acute physical activity.

K complex is the characteristic wave of stage II of sleep. The relationship between periodic limb movements (PLM) and the restless legs syndrome (RLS), and the incidence of K complexes and alpha activity has been previously described. The aim of the present study was to evaluate the effect of an acute physical activity upon K complex, PLM, and myoclonus during sleep in individuals who were paraplegic. We evaluated 84 polysomnograms from 28 volunteers with a spinal cord injury at the level of T7-T12, obtained during three consecutive nights. On day 3, the volunteers were submitted to a test of …

Pregabalin in Pregnancy: Major Congenital Malformations, Other Birth Outcomes, and Neurodevelopmental Outcomes.

Pregabalin is a gabapentinoid. It does not act on GABA receptors; rather, it inhibits calcium influx into neurons by acting on the α2δ-1 subunit of voltage-gated calcium channels. This reduces release of excitatory neurotransmitters, thereby, perhaps, explaining the sedative, anxiolytic, anticonvulsant, and other properties of the drug. Pregabalin has been approved for neuropathic pain, fibromyalgia, partial-onset seizures, and generalized anxiety disorder, and is used, off-label, for pain in many other contexts and for alcohol use disorder, pruritus, restless legs syndrome, and sleep disorder…

The role of sleep disorders in the risk for CKD and CKD progression.

Chronic kidney disease (CKD) is a major global health problem and an important driver of cardiovascular morbidity and mortality. Sleep disorders are highly prevalent in people with, or at risk of, CKD, but their specific contribution to CKD onset and progression has not been clearly defined. Unlike previous reviews that have focused mainly on symptom burden, quality of life, or general management of sleep problems in CKD, this narrative review is explicitly centred on renal outcomes. We examine whether common sleep disorders-insomnia, abnormal sleep duration, restless legs syndrome, periodic l…

A systematic review investigating the safety and efficacy of intravenous iron in people with ND-CKD who are iron deficient but not anaemic.

BACKGROUND: The role of intravenous (IV) iron in non-dialysis-dependent chronic kidney disease (ND-CKD) without anaemia remains uncertain. Iron deficiency (ID) in CKD is common and associated with impaired mitochondrial function, fatigue, cognitive decline, and restless legs syndrome, even when haemoglobin (Hb) levels are normalized. ID is linked to cardiovascular (CV) outcomes and increased hospitalization risk. Guidelines are inconsistent, and the evidence base is limited. METHODS: Databases were searched for randomized controlled trials (RCTs) and cohort studies involving adults with ND-CK…
