Considering that pseudoarthrosis, or non-union, often occurs in patients with scaphoid fractures it is very important to start adequate treatment as soon as possible. On the basis of the literature, we advise immobilisation in a below-elbow cast, whereby the thumb is not immobilised and the wrist is in a slightly extended position. Despite the fact that non-dislocated fractures, at the very least, are usually consolidated within 4 weeks, we advise an immobilisation period of 6 weeks followed by clinical or radiological evaluation. Wrist arthroscopy is an effective technique for treating acute scapholunate instability. It allows an accurate assessment of the degree and extent of the ligament injury. Partial injuries are effectively treated with arthroscopic debridement and electrothermal ligament tightening. Complete ligament injuries treated arthroscopically allow direct visualization of the torn ligament and assessment of the degree of scaphoid displacement and rotation. We report the case of a patient who suffered from an unusual anatomic form of trans-scaphoid trans-styloid volar dislocation of the lunate with proximal displacement of both the lunate and the proximal pole of scaphoid, proximal to the radiocarpal joint line. Proximal row carpectomy was performed. A good anatomic and functional result was observed at 3 years of follow-up. Etiopathogeny, therapeutic and outcome modalities are discussed.

A short cut review was carried out to establish whether wrist splints require a thumb extension when immobilising suspected scaphoid fractures. 63 papers were found using the reported searches, of which 2 presented the best evidence to answer the clinical question.
