BACKGROUND: Postoperative blindness is a devastating surgical complication. Although usually associated with prolonged cardiac and prone spinal operations, it may follow other procedures as well. Postoperative blindness is most commonly caused by a vascular etiology, but it can more rarely be caused by status epilepticus. We have previously reported a case of this phenomenon following a staged spinal deformity surgery.
CASE DESCRIPTION: Here we report 2 additional cases following a skull base procedure and a single stage lumbar spine surgery. In all instances, rapid recognition that the patie

OBJECTIVES: This study characterizes primary petroclival meningioma growth rates, before intervention, using volumetric analysis. In addition, predictors of growth are analyzed.
METHODS: Patients with previously untreated petroclival meningiomas were retrospectively reviewed (1999-2015). Image analysis software was used to perform volumetric analyses of tumor size and growth. Three-dimensional segmentation volumetric analyses were compared with volumes estimated utilizing three orthogonal dimensions. Tumor growth was defined as a 15% increase in volume.
RESULTS: Thirty-four patients who unde

BACKGROUND: Dural involvement is an important consideration in assessment of cranial base tumors dictating resectability and prognosis. Preoperative as well as intraoperative clues are valuable but not always correct. We evaluated a consecutive series of craniofacial resections at our center to correlate radiologically suspected dural involvement vis-à-vis intraoperative assessment and eventual pathology.
METHODS: We conducted a retrospective analysis of cases of skull base tumors where potential dural involvement was considered. We recorded the preoperative radiological impression (contrast-

OBJECT: A considerable degree of variability exists in the anatomy of the sphenoid sinus, sella turcica, and surrounding skull base structures. The authors aimed to characterize neuroimaging and intraoperative variations in the sagittal and coronal surgical anatomy of healthy controls and patients with sellar lesions.
METHODS: Magnetic resonance imaging studies obtained in 100 healthy adults and 78 patients with sellar lesions were reviewed. The following measurements were made on midline sagittal images: sellar face, sellar prominence, sellar angle, tuberculum sellae angle, sellar-clival ang

The advent of endoscopic technologies and techniques has expanded the limits of conventional endoscopic sinus surgery. The expanded endonasal approach describes a series of surgical modules in the sagittal and coronal planes that allow surgical access to the entire ventral skull base. The sagittal plane extends from the frontal sinus to the second cervical vertebra. The coronal plane extends from the midline to the roof of the orbit, the floor of the middle cranial fossa, and the jugular foramen. Key principles of endonasal skull base surgery are choosing a surgical corridor that minimizes the

OBJECT: Orbital tumors can be divided schematically into primary lesions, originating from the orbit itself, and secondary lesions, extending to the orbit from neighboring structures. These tumors are variable in their biological nature and in their location. The authors evaluate 41 cases of benign and malignant tumors involving the orbit and discuss the surgical challenge, which involves tumor removal, preserving visual function and cosmetic reconstruction.
METHODS: The authors performed a retrospective analysis of a series of all cases involving patients who underwent surgery for treatment

OBJECTIVE: Chondrosarcomas are rare, infiltrative, progressive lesions that occur at the cranial base. Their intimate association with cranial nerves and major vessels of the head and neck often precludes complete surgical resection.
METHODS: Between 1983 and 2003, 23 patients (14 females, 9 males) were treated at our institution with the diagnosis of chondrosarcoma of the cranial base (mean age at presentation, 43 yr). A retrospective chart review was performed to evaluate presentation, management, and adjunctive treatment. All living patients were contacted for a current examination and dis

PURPOSE: Studies involving head and neck schwannomas have focused predominantly on involvement of the vestibulocochlear nerve complex (acoustic neuroma) because of the associated morbidity related to lesions involving that region. However, the majority of head and neck schwannomas are not of vestibular nerve origin and may also produce significant morbidity due to involvement of the orbit, skull base, and cranial nerves. The purpose of this study is to examine the presenting signs and symptoms, location, nerve of origin, and outcome after treatment of patients with nonvestibular schwannomas of

Crohn's disease is an inflammatory bowel disease characterized by remissions and exacerbations. Immunosuppressants are frequently used to induce and maintain remission in these patients. The use of the immunomodulator azathioprine has been associated to malignancies. Chordomas are rare, locally aggressive tumors arising from remnants of the notochord. A specific trigger for this tumor has not been identified and association to any medication has not been reported. The purpose of this report is to present the first case reported in the literature of Crohn's disease associated to a chordoma. The

We describe the technique of preoperative embolization of the inferior petrosal sinus/anterior condylar vein complex and the posterior condylar vein in three patients undergoing skull base surgery that required opening of the jugular bulb. Contrary to the usual situation, essentially no blood was lost during the operation, resulting in decreased surgical time and reduced risk to the lower cranial nerves.

Chondrosarcomas are rare primary malignant bone tumors that involve the head and neck region in 1% to 12% of cases. Central conventional chondrosarcoma is the most common subtype and is associated with isocitrate dehydrogenase 1 and 2 (IDH1 and IDH2) gene mutations in 50% to 60% of cases. We aimed to define the frequency of IDH1 and IDH2 gene mutations in a multicenter series of 88 cases of chondrosarcoma of the head and neck, including tumors involving the base of the skull (n = 30), the facial skeleton (n = 11), and the laryngeal and tracheal cartilages (n = 47). BACKGROUND: In skull-base proton therapy (PT), severe toxicity outcomes such as temporal lobe necrosis (TLN) may be associated with inadequate management of the actual radiobiological effectiveness (RBE) of proton beams. Combining dose-averaged linear energy transfer (LETd) and dose may be crucial in treatment plan optimization and evaluation.
PURPOSE: To gather evidence on the combined effect of LETd and dose on voxel- and structure-wise levels in determining a high-grade TLN (CTCAE v.5 grade ≥G2, G2-TLN) in skull-base tumors treated with PT; a dose-LETd-volume histogram (DLVH)-based model w

BACKGROUND: Gross total resection (GTR) is a cornerstone in the treatment of various primary spinal tumors, including chordoma. However, achieving GTR for multisegment cervical tumors presents significant technical challenges due to the complex surrounding anatomy and the absence of standardized methodologies for anterior spinal column reconstruction.
METHODS: We describe the technical nuances of GTR and reconstruction for an 8-level (C1-T1) spinal chordoma in a 55-year-old female. The procedure was performed in 2 stages: a posterior approach for lamina and posterior element resection and neu

Chordoma is a rare midline malignant tumor arising from embryonic remnants of the primitive notochord. The base of the skull is the second most common site of disease after the sacrococcygeal region. Intracranial chordoma constitutes about 30-35% of chordoma cases. Metastasis from chordoma is uncommon but if occurs, it tends to spread to the lungs. Cerebrospinal fluid seeding or drop metastasis is very rare. Here we describe a case of a clival chordoma with drop metastases.
