Experience with anxiety and depression treatment studies: implications for designing irritable bowel syndrome clinical trials.

This report highlights various considerations regarding the potential effects of concurrent psychiatric conditions and a history of abuse in patient volunteers for clinical trials in irritable bowel syndrome (IBS). Even though many studies have used psychological rating scales to assess personality and psychological traits of patients with IBS, the prevalence of the different psychiatric diagnoses (i.e., categorical assessment) in patients with IBS has only recently been assessed systematically. Recent studies of treatment-seeking patients have indicated that the majority of individuals (50% t

Association of ADHD symptoms, pain, and tics with anti-thalamus antibodies in cerebrospinal fluid.

INTRODUCTION: Complex mixed presentations of severe mental disorders (SMD) with treatment resistance pose major challenges in clinical practice. The role of novel neuronal antibodies in cerebrospinal fluid (CSF) is largely unexamined in this context. METHODS: A well-studied paradigmatic case of a 36-year-old female patient is reported. RESULTS: She presented with attention-deficit/hyperactivity disorder-like symptoms (including frequent sensory overload), severe pain (pre-diagnosed as fibromyalgia and somatoform pain disorder), and motor tics. In addition, she developed secondary depressive

The Minnesota Multiphasic Personality Inventory-2-Restructured Form (MMPI-2-RF): incremental validity in predicting early postoperative outcomes in spine surgery candidates.

A substantial proportion of individuals who undergo surgical procedures to relieve spine pain continue to report significant pain and dysfunction after recovery. Psychopathology and patient expectations have been linked to poor results, leading to an increasing reliance on presurgical psychological screening (PPS) as part of the surgical diagnostic process. The original Minnesota Multiphasic Personality Inventory (MMPI; Hathaway & McKinley, 1943) and the MMPI-2 (Butcher, Graham, Ben-Porath, Tellegen, & Dahlstrom, 2001) were among the measures most commonly used in PPS evaluations and research.

Developmental influences on medically unexplained symptoms.

BACKGROUND: Medically unexplained (or 'functional') symptoms (MUS) are physical symptoms that prompt the sufferer to seek healthcare but remain unexplained after an appropriate medical evaluation. Examples of MUS also occur in veterinary medicine. For example, domestic cats suffer a syndrome comparable to interstitial cystitis, a chronic pelvic pain syndrome of humans. METHOD: Review of current evidence suggests the hypothesis that developmental factors may play a role in some cases of MUS. Maternal perception of a threatening environment may be transmitted to the fetus when hormones cross th

Somatization and stimulus entrapment.

A "vicious circle" hypothesis is put forward for the common kind of somatization which forms the basis of the DSM's "somatization disorder." Two compounding mechanisms are seen to be operative: (1) a failure of higher order inhibitory systems involved in the "medial pain system"; (2) amplification of stimulus intensity produced by the effect of attention. Attentional failure is produced not only by social factors but also by failure of sensory intensity modulation consequent upon (1). The argument focuses on data from borderline patients in whom the unusual prominence of pain may be due, at le

Pathological skin picking in individuals with body dysmorphic disorder.

OBJECTIVE: The objective of this study was to examine the prevalence and clinical correlates of pathological skin picking (PSP) in a large sample of individuals with body dysmorphic disorder (BDD). METHOD: One hundred seventy-six individuals with BDD (71.0% women; mean age, 32.5+/-12.3 years) were assessed with respect to comorbidity, BDD severity, delusionality (insight), quality of life and social/occupational functioning, using reliable and valid measures. All variables were compared in BDD subjects with and without lifetime PSP. RESULTS: About 44.9% of subjects reported lifetime PSP, and

Sensitivity to change of scales assessing symptoms of bulimia nervosa.

Measures employed in a therapy study with a pre-post design must be sensitive to the detection of treatment-related changes. In the present study, the treatment sensitivity of 12 internationally established scales that assess bulimia-relevant aspects of eating and body concern is analyzed. The scales can be sorted along three dimensions (Disturbed Eating, Restrictive Eating Behaviors and Body Dissatisfaction). Measures of the same dimension were compared in a sample of 45 women with the diagnosis of bulimia nervosa. Patients completed the scales before and 6 weeks after the end of cognitive-be

Psychological effects of hazardous materials exposures.

OBJECTIVE: To study psychological outcomes after hazardous materials incidents. METHODS: Individuals exposed to hazardous materials were contacted to complete a telephone questionnaire within 8 to 40 days of the incident. The Brief Symptoms Inventory was used for psychological assessment. General severity index, depression, anxiety, hostility, and somatization were analyzed. Positive findings were defined as two standard deviations above a normative mean. RESULTS: A total of 202 (60%) of the 339 subjects in 87 incidents were surveyed. For 159 adults with valid Brief Symptoms Inventory scores

Abnormal illness behavior of patients with functional somatic symptoms: relation to psychiatric disorders.

Functional somatic symptoms are highly associated with hypochondriasis, anxiety, and depressive disorders. Despite the absence of an organic disorder, underlying psychological distress of patients with functional somatic symptoms may result in abnormal illness behavior such as inadequate treatment seeking or overuse of medical services. Using the Illness Behavior Questionnaire (IBQ), we examined the illness behavior of Japanese patients visiting a general medicine clinic whose physical symptoms were considered functional. We used the General Health Questionnaire-30 to classify patients with fu

Nonweight-related body image concerns among female eating-disordered patients and nonclinical controls: some preliminary observations.

OBJECTIVE: Eating disorders(ED) have been classically associated with a concern about body shape and size that manifests mainly as an intense fear of weight gain (DSM-IV criteria). To further examine the nature of the body image disturbance in ED, we surveyed the prevalence of nonweight-related body image concerns among ED patients and nonclinical controls. METHOD: We examined 53 women (M +/- SD age: 28.1 +/- 6.8 years) with anorexia nervosa and/or bulimia nervosa (DSM-III-R criteria) and 73 randomly selected nonclinical women (M +/- SD age: 30.2 +/- 6.6 years) from the community. Headaches and other pain symptoms among patients with psychogenic non-epileptic seizures.

Studies of patients with psychogenic non-epileptic seizures (NES) typically focus upon the phenomenology and outcome of NES episodes. Little is known, however, about the frequency and nature of other somatic symptoms such as pain, in this population. To assess the frequency, location and severity of symptoms of pain among NES patients, we administered structured interviews to 56 patients, 6 or more months following the diagnosis of psychogenic non-epileptic seizures (NES). Patients were recruited from a tertiary hospital-based epilepsy monitoring unit. Seventy-seven percent of patients suffere

[Fibromyalgia (generalized tendomyopathy) in expert assessment. Analysis of 158 cases].

MEDAS-agencies are medical institutions within the Swiss Disability Insurance, which specialize in assessing the working capacity of candidates who apply for a disability pension. Degenerative and other chronic pain disorders of the musculoskeletal system form the majority of cases that we investigate. Fibromyalgia is one of our most frequent diagnoses (8.6%). We become involved in cases on average 8.5 years after the first onset of painful symptoms and on average 2.5 years after the patients have ceased to work. Our experience, tells us that fibromyalgia is usually associated with psychologic

Somatic sensations, anxiety, and control in panic disorder.

This case study of a woman with panic disorder with agoraphobia illustrates the relationships among somatic sensations, anxiety, and the subjective sense of control, and of these three factors to panic disorder. Helping the client alter her caffeine intake and dietary habits led to a significant reduction in panic attacks, but had less impact on her anxiety levels and sense of control. In the second stage of treatment, enhancing the client's sense of control through problem-solving training led to lower levels of anxiety. Implications for treatment and the prevention of relapse in panic disord

Cultural aspects of morbid fears in Qatari women.

The contents of morbid fears are patterned by cultural factors that decide the nature of the objects of the fears. Common factors in Western cultures do not obtain in local culture, which provides other objects for morbid fears in predisposed individuals. In the muslim community in Qatar, after-death fears dominated panic attacks, agoraphobia was rare, social phobias mirrored shame rather than guilt about failure to fulfil socially accepted norms of social behaviour, and obsessional fears revolved around fears of failure to control devil-induced impulses to harm self or others. Hypochondriacal

Stress among factory workers in a developing country.

This cross-sectional study reveals that a significant proportion (35%) of the sampled population of factory workers experienced measurable health problems in the form of anxiety, sleep disturbance, depression, somatic complaints and other clinical indicators of stress. Job stress and social support levels experienced by the group of workers judged 'sick' were significantly different from the control group, particularly so for employees in the lower levels of work hierarchy. In consonance with findings from the literature, job stress has a significant impact on mental health, with all the job s

Associations of benzodiazepine craving with other clinical variables in a population of general practice patients.

BACKGROUND: The aim of this study was to (1) describe the characteristics of patients reporting craving for benzodiazepines (BZs) and (2) to search for associations between BZ craving and other clinical variables in a population of general practice (GP) patients who have made an attempt to discontinue their long-term BZ use. METHODS: The Benzodiazepine Craving Questionnaire (BCQ) and other self-report questionnaires were administered once to a population of 113 long-term and 80 former long-term GP BZ users participating in a large BZ reduction trial in GP. Cross-sectional data were gathered o

Accuracy of diagnosing depression in primary care: the impact of chronic somatic and psychiatric co-morbidity.

BACKGROUND: Depression is highly co-morbid with both psychiatric and chronic somatic disease. These types of co-morbidity have been shown to exert opposite effects on underdiagnosis of depression by general practitioners (GPs). However, past research has not addressed their combined effect on underdiagnosis of depression. METHOD: Co-morbidity data on 191 depressed primary-care patients selected by a two-stage sampling procedure were analysed. Diagnoses of major depression and/or dysthymia in the last 12 months were assessed using a standardized psychiatric interview (CIDI) and compared with d

Somatoform symptoms, anxiety, and depression in the context of traumatic life experiences by comparing participants with and without psychiatric diagnoses.

The present study examines somatoform symptoms, anxiety and depression in connection with traumatic life experiences by comparing participants with and without psychiatric diagnoses. Significant group differences in the quantity of somatoform symptoms and the degrees of anxiety and depression could be found between participants with and without psychiatric diagnoses. Experience of sexual abuse leads to significantly more somatoform symptoms. Experience of repeated physical abuse as an additional factor significantly increased the depression score in all groups. This study reveals that the type
