Cholecystectomy and ERCP in pregnancy: a nationwide register-based study.

OBJECTIVE: The objective was to examine the outcomes of cholecystectomy and endoscopic retrograde cholangiopancreatography (ERCP) during pregnancy and compare these outcomes with those in nonpregnant women of fertile age. SUMMARY BACKGROUND DATA: Although both laparoscopic cholecystectomy and ERCP are considered safe and feasible in pregnant patients, there is still concern and uncertainty regarding gallstone intervention during pregnancy. This study aimed to investigate outcomes in pregnant patients compared to outcomes in nonpregnant patients. METHODS: Data on all female patients aged 18-4

Structural or functional abnormality of sphincter of Oddi: an important factor for the recurrence of choledocholithiasis after endoscopic treatment.

A high recurrence rate is undesirable after treatment of common bile duct (CBD) stones. A major risk factor identified for recurrence is that invasive techniques, including surgical or endoscopic treatments, will impair the biliary tract system either by direct incision of the CBD or by cutting or dilating the ampulla of Vater. During endoscopic treatment, two main assisted methods for lithotomy, sphincterotomy and papillary balloon dilation, can result in different degrees of damage to the structure and function of the sphincter of Oddi (SO), contributing to slowing of biliary excretion, chol…

Efficacy and Safety of Endoscopic Retrograde Cholangiopancreatography in Children of Pancreaticobiliary Maljunction Without Obvious Biliary Dilatation.

PURPOSE: There is no standard surgical approach for pancreaticobiliary maljunction (PBM) without congenital biliary dilatation (CBD). This s … an LH, while the treatment effective rate of ERCP is inferior to LH. The indications for endoscopic sphincterotomy and the timing of radical surgery need to be further explored. LEVEL OF EVIDENCE: Ⅲ STUDY TYPE: Retrospective Comparative Study.

Predicting the Recurrence of Common Bile Duct Stones After ERCP Treatment with Automated Machine Learning Algorithms.

BACKGROUND: Recurrence of common bile duct stones (CBDs) commonly happens after endoscopic retrograde cholangiopancreatography (ERCP). The clinical prediction models for the recurrence of CBDs after ERCP are lacking. AIMS: We aim to develop high-performance prediction models for the recurrence of CBDS after ERCP treatment using automated machine learning (AutoML) and to assess the AutoML models versus the traditional regression models. METHODS: 473 patients with CBDs undergoing ERCP were recruited in the single-center retrospective cohort study. Samples were divided into Training Set (65%) a

Risk factors of recurrent choledocholithiasis following therapeutic endoscopic retrograde cholangiopancreatography.

BACKGROUND: The risk factors for the recurrent choledocholithiasis after endoscopic retrograde cholangiopancreatography (ERCP) have not been well studied. The aim of this study was to explore the risk factors of recurrent choledocholithiasis. METHODS: We carried out a retrospective analysis of data collected between January 1, 2010 and January 1, 2020. Univariate analysis and multivariate analysis were used to explore the independent risk factors of recurrent choledocholithiasis following therapeutic ERCP. RESULTS: In total, 598 patients were eventually selected for analysis, 299 patients in

Use a biodegradable stent in ERCP and it will never be forgotten.

The easiest way to drain the bile duct or the main pancreatic duct by means of ERCP is the insertion of a plastic stent. Once cannulation ha … t in a tertiary care academic medical center, only 25% were index or initial ERCP, without previous sphincterotomy or stent, and subsequent procedures comprised the remaining 75 %.

Duodenal major papilla morphology can affect biliary cannulation and complications during ERCP, an observational study.

BACKGROUND: We investigated whether duodenal major papilla morphology could be a risk factor for failure of selective biliary cannulation (SBC) and post endoscopic retrograde cholangiography and pancreatography (ERCP) complications. METHODS: A prospectively recorded database was reviewed retrospectively. Patients were included if they received therapeutic ERCP and had naïve major duodenal papilla. We used Haraldsson's classification for papilla morphology, as follows: Regular (Type 1), Small (Type 2), Protruding or Pendulous (Type 3) and Creased or Ridged (Type 4). Risk factors for failing SB

Biliary metal stent as a nidus for bile duct stone.

Several cases of recurrent stone formation caused by a surgical material as a nidus have been reported. Recently, we experienced one case in … duct stone containing metal mesh in the distal common bile duct. The removed stone with endoscopic sphincterotomy was soft and dark brown in color with metal stent.

Embryologic and anatomic basis of duodenal surgery.

The following points should be remembered by surgeons (Table 1). In writing about the head of the pancreas, the common bile duct, and the duodenum in 1979, the authors stated that Embryologically, anatomically and surgically these three entities form an inseparable unit. Their relations and blood supply make it impossible for the surgeon to remove completely the head of the pancreas without removing the duodenum and the distal part of the common bile duct. Here embryology and anatomy conspire to produce some of the most difficult surgery of the abdominal cavity. The only alternative procedure,

[Are all possibilities of using standard instruments explored in difficult cannulations of the major duodenal papilla?].

An analysis of 132 patients with the most difficult cannulation of the major duodenal papilla was made. The procedure efficiency increased by 15.2% after using a Dormia basket. Different kinds of endoscopic papillotomy used with the application of standard instrument have also increased the efficiency of cannulation from 70.5 to 95.4%. Complications and lethality made 11.4 and 1.5% respectively. These results are considered to be not high under conditions of the most difficult cannulation of the major duodenal papilla. The opportunities of application of standard instruments for difficult cann

[Complications after endoscopic interventions on the major duodenal papilla].

An analysis of 83 cases of complications following endoscopic procedures on the major duodenal papilla was made: 62 cases after 2110 retrograde cholangiopancreatographies (RChPG) and 21 after 620 endoscopic papillosphincterotomies (EPST). Among most frequent complications there were acute pancreatitis (49 cases), purulent cholangitis (11 cases), suppurations of the pancreatic cysts and purulent pancreatitis (8 cases). Bleedings took place after EPST in 11 patients. Risk factors of the development of acute pancreatitis were established: the presence of scarry stenosis of the papilla, chronic pa

[Explorations of the common bile duct in the era of laparoscopic surgery].

The advent of laparoscopic surgical procedures has infatuated both patients and surgeons. Potential beneficiaries in France could number 50,000 per year for gall stone surgery alone. The affect this type of procedure could have on exploration of the common bile duct has led to a certain degree of controversy in the literature. From 10 to 15% of the patients who undergo cholecystectomy have a stone in the common duct and require specific treatment. The clinician thus must propose an exploration of the common duct and decide on whether it should be performed before, during or after cholecystecto

Comprehensive review on small common bile duct stones.

Common bile duct stones are among the most common conditions encountered by endoscopists. Therefore, it is well researched; however, some items, such as indications for endoscopic papillary balloon dilatation (EPBD), safety of EPBD and endoscopic sphincterotomy in patients receiving dual antiplatelet therapy or direct oral anticoagulant, selection strategy for retrieval balloons and baskets, lack adequate evidence. Therefore, the guidelines have been updated with new research, while others remain unchanged due to weak evidence. In this review, we comprehensively summarize the standard methods …

Case Report: Histoplasma Hepatitis Presenting as Common Bile Duct Obstruction.

Histoplasma capsulatum is the most common endemic mycosis in the United States and can cause disseminated histoplasmosis in immunocompromise … nfection. Endoscopic retrograde cholangiopancreatography suggested Mirizzi syndrome, but successful sphincterotomy failed to improve jaundice over subsequent days. Bronchoscopy and liver biopsy were performed, with the first revealing budding yeast and the later growing H. capsulatum. The patient improved with the initiation of itraconazole therapy.

[A case of polyp-type ampullary carcinoma-in-adenoma within the ampullary channel, displaying an intraductal growth pattern].

A 70-year-old woman with jaundice was referred to our hospital. Obstructive jaundice caused by common bile duct (CBD) stones was diagnosed based on the results of blood tests, abdominal computed tomography, and endoscopic retrograde cholangiopancreatography. We attempted to remove the CBD stones endoscopically. After endoscopic sphincterotomy, a polypoid lesion was exposed at the ampulla of Vater. Histological examination of a biopsy specimen from the ampullary lesion revealed a tubular adenoma, and the patient underwent pylorus-preserving pancreatoduodenectomy. Pathological examination of the…

Ultrasound-guided extracorporeal shock wave lithotripsy of pancreatic ductal stones: six years' experience.

Extracorporeal shock wave lithotripsy (ESWL) and endoscopic sphincterotomy (EST) was performed in 35 patients suffering from pancreatic duct stones. Calculi disintegration and resolution of obstruction were achieved in all cases. Completely stone-free ducts were achieved in 16 patients (46%) while some peripheral asymptomatic stone material remained in 19 (54%). Dilation of the main pancreatic duct was reduced in 29 patients (83%). Twelve patients (34%) became completely asymptomatic and 17 (49%) reported a marked reduction of pain. Pancreatogenic steatorrhea ceased and 18 patients (51%) gaine…

Papillary stenosis and sclerosing cholangitis in the acquired immunodeficiency syndrome.

Eight homosexual men with the acquired immunodeficiency syndrome (AIDS) presented with clinical, biochemical, and radiologic features of ste … irregularities and provides therapeutic intervention. Prompt relief of symptoms follows endoscopic sphincterotomy, often with resolution of biochemical evidence of cholestasis. Biliary tract infection with cytomegalovirus or cryptosporidia and resultant viral or coccidial cholangitis are the proposed pathophysiologic mechanisms.
