A Novel Technique for Correcting Extensor Lag in Vascularized Toe PIP Joint Transfers.

The use of the vascularized second-toe proximal interphalangeal joint (PIPJ) for vascularized joint transfers (VJTs) allows for restoration of powerful pinch/grasp and range of movement of a PIPJ in the hand. However, because of the lack of central slip formation in the majority of toes, the reconstructed PIPJ often results in extension lag. Extension lag associated with poor central slip formation in the lesser toes can be corrected using central slip reconstruction methods such as the Stack method. However, such methods are often cumbersome and involve extensive dissection and soft tissue ma

Navicular tenosuspension with anterior tibialis tendon (Young procedure) associated to calcaneo-stop for the treatment of paediatric flexible flatfoot: clinical and ultrasound study.

BACKGROUND AND AIM OF THE WORK: Flexible flatfoot is one of the most common deformities in pediatric orthopaedics. Arthroeresis procedures are designed to correct this deformity. Among them, calcaneo-stop is a procedure with both biomechanical and proprioceptive properties. There could be other surgical procedure combined, such as a percutaneous Achilles tendon lengthening and the Gould tibialis posterior retension or Young tibialis anterior navicular tenosuspension. This study analyzed the clinical and sonographic results of 36 patients following flexible flatfoot surgical treatment with a ca

Minitendon graft transfer for suspension of the paralyzed lower eyelid: our experience.

BACKGROUND: : Restoration of eyelid animation and aesthetics is a major component of the surgical management of facial paralysis. The authors' experience with the minitendon graft (a piece of split palmaris tendon graft) for lower eyelid suspension is presented. The effect of age, cause, denervation time, and total number of procedures performed in the eye region are analyzed. METHODS: : Fifty-eight patients with facial paralysis presenting with paralytic ectropion received the minitendon graft for lower eyelid suspension. Twenty-eight patients with concurrent lagophthalmos received the eye s

Correction of moderate and severe acquired flexible flatfoot with medializing calcaneal osteotomy and flexor digitorum longus transfer.

BACKGROUND: Acquired flexible flatfoot encompasses a wide spectrum of disease, and there is no validated treatment protocol. We hypothesized … lar-first metatarsal angle (p = 0.003) and talonavicular angle (p = 0.002). After the osteotomy and tendon transfer in the mild-flatfoot model, the talar-first metatarsal angle and talonavicular angle were not significantly different from those in the intact state. In the severe-flatfoot model, the talar-first metatarsal angle, talonavicular angle, and medial cun …

Surgical treatment of brachial plexus birth palsy.

Brachial plexus birth palsy remains a challenging condition. In the 1000 infants followed from 1977 to 1988, functional results were much improved over those obtained by observation only, if surgical exploration and repair were performed when no clinical recuperation of biceps function occurred by three months of age. Recovery is slow, and comprehensive follow-up study of reconstructed and conservatively managed children is required to prevent joint contractures. Children who will benefit from palliative procedures such as tendon transfers must also be identified.

Congenital snapping knee. Habitual anterior subluxation of the tibia in extension.

We describe a rare form of congenital snapping knee. In six knees in four children, the tibia subluxed anteriorly on the femur when the knee was extended and reduced spontaneously on flexion. The abnormal movements were seen and felt as sudden snaps or clunks at about 30 degrees of flexion. All six knees showed similar dysplastic features, although the patients had different clinical syndromes. The mechanism of the subluxation and its management are discussed.

[Treatment of major patellar instability. Results of the Fèvre-Dupuis operation].

In the treatment of recurrent subluxation and dislocation of the patella in adolescents and young adults, the authors describe a method consisting of: --a modified Roux procedure for internal transposition and advancement of the anterior tibial tuberosity; --the Lecène technic for over-the-top reinsertion of the vastus medialis on the lateral aspect of the patella. All the patients had a plaster cast for 10 days followed by rehabilitation until normal knee function was achieved. Stiff knees were mobilized under general anesthesia. Normal activity was progressively resumed 6 weeks after surgery

[Stabilization of the external condyle of the knee in chronic anterior laxity. Importance of the popliteal muscle].

The authors have made an anatomical and experimental study in cadavers. They consider that one of the main features of chronic anterior instability of the knee is related to the instability of the lateral femoral condyle. This is due to the loss of its two stabilizers: the anterior cruciate ligament and the popliteal muscle. The signs of a lesion of the postero-lateral structures have to be looked for: antero-posterior drawer sign in extension, hypermobility of the tibia in lateral rotation and varus laxity. Such instability leads to arthrosis in the long-term. The proposed treatment is based

Latissimus dorsi transfer for recurrent dislocation of the shoulder.

The results of latissimus dorsi transfer for recurrent anterior dislocation of the shoulder performed during the past three decades were subjected to a failure analysis. Twenty patients operated on in the light of this had no episodes of dislocation and obtained full function. The mode of action of the latissimus dorsi transfer explains the apparent paradox of an operation that produces external rotation resulting in the prevention of anterior dislocation. The action of the transfer in pulling the humeral head posteriorly is the most important feature. If the transfer is not located correctly,

Hand assessment and management of arthrogryposis multiplex congenita.

Arthrogryposis of the upper extremity is easy to diagnose. Tendon transfers are used to give a dynamic force to aid correction of the deformity and provide useful motion. Surgical correction of small joints of the hand has not proved too successful and frequently will decrease mobility even further. The goal in treating up …

Biceps tendon rerouting and percutaneous osteoclasis in the treatment of supination deformity in obstetrical palsy.

Paralytic supination deformity of the forearm, secondary to obstetrical palsy, was treated by biceps tendon rerouting in 11 children. In two patients additional pronation was obtained by subsequent percutaneous osteoclasis and rotation of the radius and ulna. Correction of the supination deformity resulted in improvement of single-handed as well as two-handed activities in all patients.

Functional veloplasty in bilateral palatal palsy.

A new technique for the treatment of bilateral palatal palsy involves the submucosal transposition of the tensor palati muscles of both sides to form an active muscular sling for the elevation of the paralyzed soft palate. The effect of the rerouted tensor palati muscle on Eustachian tube function and its efficacy as a substitute for the levator palati muscle in lifting the soft palate was evaluated by animal experimentation. Subsequently, two patients with bilateral palatal palsy, who failed to show improvement during a six-month period, were treated successfully with this new surgical techni

[Surgical treatment of malformations of the hand (author's transl)].

The frequency of malformations of the hand is estimated at 0.5 percent of live births. The course of early developmental processes makes early correction advisable. Knowledge of the special anatomy and mastery of the microsurgical operation techniques makes this possible. The main indications and methods that hve been tested are discussed.

Interference screw for fixation of FDL transfer in the treatment of adult acquired flat foot deformity stage II.

INTRODUCTION: Flexor digitorum longus transfer and medial displacement calcaneal osteotomy have shown favourable results in the treatment of adult acquired flat foot deformity stage 2. Little is known about the resorbable interference screw for tendon fixation and postoperative patient satisfaction though. Moreover possible changes of radiographic parameters at final follow-up, possible implant-associated complications and differences concerning clinical results at final follow-up to other studies using bone tunnel techniques for fixation of the FDL tendon were investigated. Surgical strategy for improving forearm and hand function in late obstetric brachial plexus palsy.

For the purpose of treatment, obstetric brachial plexus palsy can be subdivided into two distinct phases: initial obstetric brachial plexus … ltaneously with reconstructive procedures for improving shoulder and/or elbow function. Traditional tendon transfer techniques do not provide satisfactory reconstruction for those deformities. Many of the authors' patients required more complex techniques such as nerve transfer and functioning free-muscle transplantation to augment traditional techniques of tendo …

From thigh to pelvis: female genital prolapse repair with an autologous semitendinosus tendon transplant : Data of the German multicenter trial.

INTRODUCTION AND HYPOTHESIS: The use of synthetic mesh for prolapse and incontinence surgery is discussed controversially and in several countries is either no longer used or permissible. Previous approaches with autologous tissue did not show from a patient´s perspective convincing long-term results. As there have been repeatedly significant complications with synthetic mesh, a new approach is urgently needed. During orthopedics and trauma surgeries, tendons from the thigh have been used for decades to replace cruciate ligament. The procedure of tendon removal from the thigh is fast, easy to
