Extrapulmonary tuberculosis: a debilitating and often neglected public health problem.

We report a case of a 33-year-old woman from Nepal who presented to a hospital with paraplegia. She was found to have pulmonary tuberculosis (TB) with lumbar spine involvement, and bilateral psoas abscesses. She had no initial symptoms attributable to pulmonary involvement. Her delayed presentation to the hospital led to complication of TB spine, which compromised the life of this woman working as a labourer. Early diagnosis and treatment of extrapulmonary TB is essential. Awareness regarding symptoms of different forms of extrapulmonary TB and making diagnostic modalities such as CT scan, MRI

[Etiological diagnosis of spondylitis. II. Vertebral puncture biopsy and surgical approach].

The authors tried to determine the ways in which vertebral puncture biopsy and the direct surgical approach can helpin the etiological diagnosis of spondylodiscitis, when the clinical radiological, and laboratory examination leave the clinician in doubt. Out of 28 vertebral puncture biopsies, proof of tuberculosis was obtained in 4 cases, and proof of a non-tuberculous cause was obtained in 1 case as a result of isolating the responsible organism. Thirty-eight cases were submitted to the direct surgical approach. When the indication for surgery was solely in order to investigate the etiology,

[The suffering of Heinrich Heine].

The analysis of clinical neurological symptoms on the basis of available letters and other documents confirms beyond a doubt the long standing diagnosis of neurosyphilis in the form of chronic meningitis with cranial polyneuritis and wide spread polyradiculitis, formerly called Lues cerebro-spinalis. The symptoms which recurred over 25 years include multiple severe paralysis of cranial nerves, dramatic radicular neuralgias and radicular paralysis in addition to a partial transverse lesion of the spinal cord and terminal coughing and regurgation fits in the absence of symptoms in the cerebrum a

[The endovascular treatment of hemoptysis].

We studied 18 patients, 9 women and 9 men, ranging in age from 18 to 76 years. The main symptom was hemoptysis and the underlying pathology was tuberculosis, actinomycosis, lung cancer, metastatic carcinoma and systemic lupus. Nonsurgical patients, with recurrent hemoptysis or massive bleeding were selected. The embolization substances were spongostan, avitene (R) and PVA; they all produce temporary as well as persistent hemostasis. The procedure was successful in 16 patients. In two patients the embolization was not performed, one for technical reasons and the other because the vessel to be t

A 3-year-old boy with kyphosis, back mass and weakness.

Spinal tuberculosis (TB) in children is uncommon. The authors report a case of a 3-year-old boy who was diagnosed with TB spondylitis. He presented to the hospital with a back mass, back pain and inability to walk. He failed to receive TB prophylaxis after having been in close contact to his mother who had pulmonary TB. He received incision and drainage and continued on anti-TB regimens without complications. At 4-month follow-up, he was able to stand and walk without assistance. It is important to carry out contact investigations and provide TB prophylaxis to children who have had a history o

Diagnostic pitfalls of spinal echinococcosis.

OBJECTIVE: To critically examine and elucidate the diagnostic pitfalls of spinal echinococcosis. SUMMARY OF PATIENTS: From October 1957 to June 2006, 25 consecutive cases drawn from 5721 cases of echinococcosis were collected in the First Affiliated Hospital of Xinjiang Medical University. The selected cases comprised 11 males and 14 females; all were treated with debridement operations. The average age was 28.3 years (15 to 56 y). The average duration of infestation with spinal hydatid disease was 3.2 years (0.5 to 12 y). Nineteen of the 25 cases underwent magnetic resonance imaging (MRI) sca

Surgical correction of severe cervical kyphosis secondary to tuberculoid Pott's disease: illustrative case.

BACKGROUND: Pott's disease (PD) is an infection of the vertebral bodies due to Mycobacterium tuberculosis. Commonly presenting with back pain, the insidious nature of the disease can result in the development of kyphotic deformities, epidural collections, and neurological dysfunction. The authors describe the case of a 21-year-old male who presented with symptoms of cervical myelopathy and was found to have a kyphotic deformity involving the C5 and C6 vertebrae with epidural extension. The patient was treated with a combined anterior-posterior surgical approach: anteriorly with C5-6 corpectomy

Clinical Spectrum of Central Nervous System Tuberculosis and the Efficacy of Revised National Tuberculosis Control Program in its Management.

INTRODUCTION: Tuberculosis (TB) is a major global problem and poses a threat which is of considerable magnitude, with an estimated one-third of the population infected with TB bacillus. AIM: The aim of this study is to study the treatment outcomes in patients with various forms of neurological TB treated with the standardized Revised National TB Control Program (RNTCP), directly observed treatment short-course (DOTS). MATERIALS AND METHODS: Patients diagnosed to have TBM, tuberculoma with or without spinal arachnoiditis (central nervous system tuberculosis-TB [CNS-TB]) were categorized as pe

Comparison of Anterior only and Combined Anterior and Posterior Approach in Treating Lumbosacral Tuberculosis.

A combined anterior and posterior (AP) surgical approach is a popular treatment modality of lumbosacral tuberculosis, but it is often traumatic and complicated. The present study aims to find whether the anterior only approach with the ARCH plate system is less invasive than the AP approach in treating lumbosacral tuberculosis. The ARCH plate system is an innovative anatomic lumbosacral anterior multi-directional locking plate system which was devised with due consideration to the anatomic features of the lumbosacral spine and irregular destruction of involved vertebral endplates. In this retr

The outcome of intervertebral surgery in the treatment of lumbar tuberculosis in children: A case series and long-term follow-up.

During the operation of treating lumbar tuberculosis in children, a long-segment or short-segment fixation, and fusion method were usually applied, which would adversely affect the function of normal motion unit. And so, we have been focusing on how we can shorten the range of fixation and fusion using intervertebral surgery. The objective of this retrospective study is to investigate the clinical outcome of intervertebral surgery, in the treatment of lumbar tuberculosis in children. From June 2003 to June 2013, 18 children with lumbar tuberculosis underwent intervertebral surgery, using a comb
