BACKGROUND: Carbapenem-resistant Acinetobacter baumannii (CRAB) has emerged as a predominant strain of healthcare-associated infections worldwide, particularly in intensive care units (ICUs). Therefore, it is imperative to study the molecular epidemiology of CRAB in the ICUs using multiple molecular typing methods to lay the foundation for the development of infection prevention and control strategies. This study aimed to determine the antimicrobial susceptibility profile, the molecular epidemiology and conduct homology analysis on CRAB strains isolated from ICUs.
METHODS: The sensitivity to …

Recently, opportunistic nosocomial infections caused by Acinetobacter baumannii have become increasingly prevalent worldwide. The pathogen often establishes biofilms that adhere to medical devices, causing chronic infections refractory to antimicrobial therapy. Clinical reports have indicated that some macrolide antibiotics are effective against chronic biofilm-related infections. In this study, we examined the direct anti-biofilm effects of seven macrolides (azithromycin, clarithromycin, erythromycin, josamycin, spiramycin, fidaxomicin, and ivermectin) on A. baumannii using a simple and newly…

Acinetobacter baumannii infections are compounded with a striking lack of treatment options. In many Gram-negative bacteria, secreted proteins play an important early role in avoiding host defences. Typically, these proteins are targeted to the external environment or into host cells using dedicated transport systems. Despite the fact that medically relevant species of Acinetobacter possess a type II secretion system (T2SS), only recently, its significance as an important pathway for delivering virulence factors has gained attention. Using in silico analysis to characterize the genetic determi…

Multidrug-resistant (MDR) gram-negative bacteria-related nosocomial infections and ventilator-associated pneumonia (VAP) presents an emerging challenge to clinicians. Older antimicrobial agents such as colistin have become life-saving drugs because of the susceptibility of these pathogens. We report our experience with aerosolized colistin in two preterm and one term neonate with Acinetobacter baumannii and Pseudomonas aeruginosa-related VAP who were unresponsiveness to previous antimicrobial treatment. All pathogens were isolated from tracheal aspirate. We used 5 mg/kg (base activity) aerosol…

Oysters are filter feeders that bioaccumulate microorganisms and other contaminants from water. Therefore, screening oysters facilitates monitoring of emerging antimicrobial-resistant pathogens in these habitats. Here, we report an in-depth genomic characterization of an antibiotic-resistant Acinetobacter baumannii that was isolated from a wild-caught oyster. Multilocus Sequence Typing (MLST) analyses assigned the strain to the ST25IP (Pasteur Scheme Sequence Type), which belongs to a lineage associated with antibiotic-resistance and epidemics. The strain was found to be antibiotic-resistant a…

Carbapenem-resistant Acinetobacter baumannii (CRAB) represents an urgent global health threat, with resistance primarily driven by carbapenem-hydrolyzing class D β-lactamases (CHDLs) such as OXA-23. Therapeutic options remain limited due to the scarcity of effective β-lactam/β-lactamase inhibitor combinations. Pilabactam (formerly ANT3310) is a novel diazabicyclooctane (DBO) β-lactamase inhibitor featuring a fluorine substituent that extends its activity spectrum, relative to approved DBOs like avibactam and relebactam, to include CHDLs. Pilabactam is currently in phase I clinical trials in co…

BACKGROUND: Pseudomonas aeruginosa and Acinetobacter baumannii are common nosocomial pathogens in hospital settings. Recently, they have also been found in non-hospital environments, such as poultry farms. While most studies in Ethiopia have focused on these bacteria's antibiotic resistance patterns in hospitals, information regarding their prevalence and resistance in veterinary settings, particularly poultry farms, is limited. This study aimed to assess the prevalence and antibiotic resistance patterns of P. aeruginosa and A. baumannii isolated from chicken droppings on poultry farms.

We present our findings on interpatient transmission, epidemic control measures, and the outcomes of a series of ten critically ill burn patients who were either colonized or infected with carbapenem-resistant Acinetobacter baumannii (CRAB). None of the five infected patients achieved clinical cure, and all experienced relapses. Microbiological failure was observed in 40% of the infected patients. The isolated CRAB strains were found to carry blaOXA-23 and armA resistance genes. Despite the lack of clinical cure, all five infected patients survived and were discharged from the Burn Intensive C…

BACKGROUND: Fosfomycin (FOS) is gaining increasing importance as part of combination therapy for the treatment of carbapenem-resistant Acinetobacter baumannii (CRAB) and KPC-producing Klebsiella pneumoniae (KPC-Kp), thanks to its in vitro synergism with several antibiotics, high tissue distribution and good tolerability. We analyzed the effect on 30-day survival of FOS-containing regimens compared to non-FOS-containing regimens in critically ill patients admitted to the intensive care unit with CRAB or KPC-Kp infections. Secondary objectives were to evaluate clinical cure and microbiologic era…

The increasing rates of antimicrobial resistance among carbapenem-resistant Acinetobacter baumannii in the Middle East and North Africa are one of the major concerns for healthcare settings. We characterised the first A. baumannii isolate harbouring five β-lactamases identified in Egypt. The isolate Ale25 was obtained from an ICU patient of a hospital from Alexandria. The isolate was phenotypically and genotypically screened for carbapenemase genes. The isolate was resistant to carbapenems, aminoglycosides, fluoroquinolones and cefiderocol. Whole-Genome Sequencing identified five β-lactamase g…
