[Highly differentiated squamous epithelial carcinoma as a late complication of post-traumatic osteomyelitis].

Squamous cell carcinoma as a late complication of chronic osteomyelitis is a well known phenomenon in traumatology, often occurring as a consequence of bone fractures. The majority of cases are observed in men between 50 and 60 years of age. The time from onset of inflammatory bone disease to malignant transformation differs but usually takes 30 years. In general, prognosis is thought to be favorable when adequate surgical therapy is carried out. Nowadays, it is important to recall this condition, because it has become rare. The cases reported here illustrate the difficulties that may be encou

Comprehensive Rehabilitation for a Multilimb Amputee: A Case Report.

Multilimb amputation poses a significant challenge to an individual's mobility, independence, and overall quality of life. Comprehensive rehabilitation is essential for these patients to restore functional abilities and facilitate their reintegration into society. This case study details the rehabilitation of a 23-year-old male who sustained bilateral transradial and right transtibial amputations due to an electric burn injury. Upon presentation, the patient exhibited severe functional limitations, with complete dependence on caregivers for mobility, self-care, and activities of daily living (

The results of in situ prosthetic replacement for infected aortic grafts.

BACKGROUND: Treatment of aortic graft infection with graft excision and axillofemoral bypass may carry an increased risk of limb loss, aortic stump blowout, and pelvic ischemia. A review of patients with aortic graft infection treated with in situ prosthetic graft replacement was undertaken to determine if mortality, limb loss, and reinfection rates were improved with this technique. METHODS: The clinical data of 25 patients, 19 males and 6 females, with a mean age of 68 years (range 35 to 83), with aortic graft infection, treated between January 1, 1989, and December 31, 1998, by in situ pro

Redo aortic grafting after treatment of aortic graft infection.

PURPOSE: This study was performed to determine the indications, operative strategy, and hemodynamic benefit of redo aortic grafting procedures after earlier excision of an infected aortic graft. METHODS: Among 164 patients treated for aortic graft infection, 15 later underwent redo aortic grafting procedures an average of 18 months (range, 1 to 59 months) after removal of an infected aortic graft. Redo grafting procedures were performed for leg ischemia (n = 11) or infection (proven, n = 3; suspected, n = 1). The new aortic graft originated either from the distal thoracic aorta (n = 5) or fro

Advanced Eumycetoma of the Right Foot Resulting in Below-Knee Amputation in Somalia: A Case Report.

INTRODUCTION: Mycetoma is a chronic, progressively destructive neglected tropical disease that presents with localised swelling, multiple sinus tracts and a discharge containing grains, and that may extend into deep tissue and bone. Somalia lies within the recognised mycetoma belt, but the national burden is poorly documented, and few patients reach specialist care early enough for the limb to be saved. CASE PRESENTATION: A 33-year-old man from Qoryooley District, Lower Shabelle, Somalia, presented with a seven-year history of progressive swelling of the right foot. Examination showed massive

Surgical treatment of painful neuroma in amputated and non-amputated patients: does the level of neurotomy affect clinical outcomes?

PURPOSE: To compare the outcomes of distal neurotomy (DN) versus proximal neurotomy (PN) for the surgical management of painful neuromas in amputees and non-amputees, whether used in passive or active treatment. METHODS: A retrospective study was conducted on patients who underwent surgery for painful traumatic neuromas between 2019 and 2022. DN with neuroma excision was performed at the level of the injury or amputation. PN was performed using a separate proximal approach without neuroma excision. Outcomes included a Numerical Rating Scale (NRS) score and Patient-Reported Outcomes Measuremen

Review. Limb regeneration in lizards under natural and experimental conditions with considerations on the induction of appendages regeneration in amniotes.

BACKGROUND: Study on the failure of limb regeneration in lizards evidences the difficult problems met from amniotes to regenerate organs. Contrary to the tail, limb loss in terrestrial environment is generally fatal and no selection for its regeneration occurred during lizard evolution. METHODS: Experimentally amputated limbs were fixed and embedded for microscopy. RESULTS: After limb loss an intense inflammatory reaction occurs and immune cells are recruited underneath a wound epidermis, forming a vascularized granulation tissue. The regenerating epidermis takes 2-3 weeks to cover the limb

[Rotationplasty in a schoolboy for osteosarcoma: Prosthesis fitting and update].

INTRODUCTION: The classical treatment of osteosarcoma used to be ablative surgery. After the appearance of adjuvant chemotherapy, survival in these patients increased, and with it, the number of affected school age children with high growth potential. Hence, reconstructive surgeries are currently proposed instead of conventional bone resections due to greater limb preservation and better functional status than those achieved with conventional amputations. CASE PRESENTATION: We describe a case of osteosarcoma in a 9-year-old boy with a history of retinoblastoma. The tumour involved the entire

Enhancing functional abilities and cognitive integration of the lower limb prosthesis.

Lower limb amputation (LLA) destroys the sensory communication between the brain and the external world during standing and walking. Current prostheses do not restore sensory feedback to amputees, who, relying on very limited haptic information from the stump-socket interaction, are forced to deal with serious issues: the risk of falls, decreased mobility, prosthesis being perceived as an external object (low embodiment), and increased cognitive burden. Poor mobility is one of the causes of eventual device abandonment. Restoring sensory feedback from the missing leg of above-knee (transfemoral

Nonunion fracture healing: Evaluation of effectiveness of demineralized bone matrix and mesenchymal stem cells in a novel sheep bone nonunion model.

Nonunion treatment has a high rate of success, although recalcitrant nonunion may determine the need for amputation. Therefore, new treatment options are continuously investigated in order to further reduce the risk of nonunion recurrence. This study aimed to (a) develop a new large animal model for bone atrophic nonunion and (b) compare the efficacy of demineralized bone matrix (DBM) and DBM in combination with mesenchymal stem cells (MSC) in the new nonunion model. The new model consists of a noncritical, full-thickness segmental defect created in the sheep tibia, stabilized by an intramedul

Broadband Prosthetic Interfaces: Combining Nerve Transfers and Implantable Multichannel EMG Technology to Decode Spinal Motor Neuron Activity.

Modern robotic hands/upper limbs may replace multiple degrees of freedom of extremity function. However, their intuitive use requires a high number of control signals, which current man-machine interfaces do not provide. Here, we discuss a broadband control interface that combines targeted muscle reinnervation, implantable multichannel electromyographic sensors, and advanced decoding to address the increasing capabilities of modern robotic limbs. With targeted muscle reinnervation, nerves that have lost their targets due to an amputation are surgically transferred to residual stump muscles to

Management of septic skin necroses.

Necrotizing fasciitis (NF) and purpura fulminans (PF) are conditions with extensive septic skin necroses that are associated with significant morbidity and mortality. NF is caused by fulminant bacterial spread on the superficial muscle fascia, Group A streptococcus being the main microorganism responsible for it. The major challenge NF poses is timely recognition. Although crucial for patient survival, early diagnosis is difficult because paucity of specific early findings does not allow setting NF apart from other, less severe, differential diagnoses. Surgical therapy consists of early and ag

Value of percutaneous embolotherapy for the management of traumatic vascular limb injury.

BACKGROUND: Arterial injuries to the extremities may result in severe hemorrhagic complications. Open surgical repair has been the standard of care for these patients, but endovascular management may be a valuable, less invasive alternative. PURPOSE: To evaluate the safety, efficacy and long-term durability of embolotherapy for the management of hemorrhagic, traumatic vascular injuries in upper and lower limbs. MATERIAL AND METHODS: Patients with traumatic or iatrogenic hemorrhagic vascular injuries to the extremities and managed with endovascular embolotherapy in the authors' institution be

Innervated island pedicled anterolateral thigh flap for neo-phallic reconstruction in female-to-male transsexuals.

Many techniques have been described to create an aesthetic and functional neo-phallus after penile amputation or in female-to-male transsexuals. Microsurgical free-flap phalloplasty seems to be the preferred method of penile reconstruction. For many years the radial forearm free flap has been considered the best procedure, but other flaps have been attempted to minimize donor site morbidity and optimize outcome. Pedicled flaps are considered to be reliable and to decrease the risk of total failure. Recently, a one-stage non-microsurgical technique was described for phallic reconstruction in a

A case of W. chitiniclastica bacteremia in a 38-year-old homeless male originating from a maggot-infested amputated foot.

Wohlfahrtiimonas (W.) chitiniclastica was first isolated from the larval stage of the fly vector Wohlfahrtia magnifica. It is a gram-negative, non-motile, strictly aerobic rod that thrives in temperatures between 28º C and 37º C. Its strong chitinase activity aids in metamorphosis, which suggests a symbiotic relationship with the fly. Although rare, W. chitiniclastica has been implicated in human infections, like bacteremia and osteomyelitis, typically transmitted through fly larvae in skin wounds. Over the past decade, there have been 12 documented human infections, including five confirmed c

Kingella kingae Induced Septic Arthritis in the Knee Revealing an Unexpected Diagnosis of Multiple Myeloma, a Case Report.

INTRODUCTION: Proximal femur fractures in below-knee amputees pose significant surgical challenges due to difficulties in positioning and stabilizing the residual limb for fracture reduction. In the current literature, there is no consensus on the optimal management strategy, but the inverted boot positioning method seems to be an adequate and non-invasive technique. However, in our case, this method was not possible due to limited knee flexion, so we describe our technique and compare the different modalities described in the literature, highlighting their advantages and disadvantages.
