Surgical epicardial ablation of left ventricular pathway using sling exposure.

We report our experience with 43 consecutive patients with left free wall pathways operated on since December 1988 using a modified direct epicardial approach through a medial sternotomy, without the adjunct of normothermic cardiopulmonary bypass. The left atrioventricular sulcus is exposed by dislocating the heart cephalad and to the right using a sling made of a large sponge passed around the ventricle through the transverse sinus. While the arterial pressure is monitored, the heart is positioned to obtain adequate exposure without compromising the ventricular function. The left atrioventric

Electrocardiographic characteristics of patients with Ebstein's anomaly before and after ablation of an accessory atrioventricular pathway.

UNLABELLED: The abnormal development of the tricuspid valve in patients with Ebstein's anomaly results in several activation abnormalities including delayed intraatrial conduction, right bundle branch block (RBBB), and ventricular preexcitation. The aim of the present study was to define the ECG characteristics before and after ablation of an accessory A-V pathway (AP) in patients with Ebstein's anomaly. METHODS: A series of 226 consecutive patients with Ebstein's anomaly was studied. Sixty-four patients (28%) had documented tachycardia. Thirty-three patients with recurrent tachycardia were f

Catheter ablation of atrioventricular junction via retrograde route in a patient with single ventricle.

Radiofrequency catheter ablation of the atrioventricular junction (AVJ) was performed by the retrograde route in a 19-year-old woman with atrial fibrillation and single ventricle following the bidirectional Glenn procedure. Two energy applications resulted in complete atrioventricular block and dependence on an epicardial ventricular pacemaker.

Atrioventricular nodal ablation and pacemaker implantation in patients with atrial fibrillation.

In drug-resistant, poorly tolerated atrial fibrillation, atrioventricular (AV) junction catheter ablation can be proposed as the last-resort option. Technically, the procedure is easy to perform and relatively safe. Interruption of the AV conduction implies the insertion of a permanent pacemaker. In patients with chronic atrial fibrillation, a VVIR pacemaker is inserted. For those having severely symptomatic episodes of paroxysmal atrial fibrillation, DDDR mode-switching devices are more appropriate. Results are remarkable. The treatment is highly effective in controlling symptoms and improvin

[Radiofrequency transcatheter ablation of a bidirectional decremental accessory atrioventricular pathway in the coronary sinus].

We report the case of a 63-year-old patient who underwent radiofrequency catheter ablation of an atrioventricular accessory pathway with bidirectional decremental conduction properties. The successful ablation site was the distal end of a wedge-shaped dilation of the first tract of the coronary sinus. Atrioventricular accessory pathways with anterograde decremental conduction properties was thought to belong to fibers with Mahaim type electrophysiological properties. They rarely show decremental retrograde conduction properties. Most Mahaim type atrioventricular pathways are right-sided with a

Nonreentrant supraventricular tachycardia due to double ventricular response via dual atrioventricular nodal pathways.

Narrow and wide QRS tachycardias associated with various rhythm disturbances were recognized during 24-hour ambulatory eletrocardiographic monitoring in a 65-year-old man with coronary artery disease. Laddergram analysis revealed the presence of dual atrioventricular nodal pathways. Non-reentrant supraventricular tachycardia due to simultaneous fast and slow conduction through the dual atrioventricular nodal pathways was confirmed by electrophysiologic study. The atrial rate determined the occurrence of simultaneous conduction, and extrastimulation failed to induce a double ventricular respons

Extended protocol for demonstration of dual AV nodal physiology.

The incidence of dual atrioventricular (AV) nodal physiology was evaluated in 22 patients (14 males, 8 females, age 52 +/- 18 years) undergoing electrophysiology studies for evaluation of ventricular tachycardia/nonsustained ventricular tachycardia (n = 11), supraventricular tachycardia (n = 5), and syncope (n = 6). Patients with AV node reentrant tachycardia were excluded. Thirteen patients had normal left ventricular function and nine patients (seven with coronary artery disease, two with dilated cardiomyopathy) had depressed left ventricular function. Single atrial extrastimuli (A2) were in

The response of paroxysmal supraventricular tachycardia to overdrive atrial and ventricular pacing: can it help determine the tachycardia mechanism?

INTRODUCTION: Standard electrophysiologic techniques generally allow discrimination among mechanisms of paroxysmal supraventricular tachycardia. The purpose of this study was to determine whether the response of paroxysmal supraventricular tachycardia to atrial and ventricular overdrive pacing can help determine the tachycardia mechanism. METHODS AND RESULTS: Fifty-three patients with paroxysmal supraventricular tachycardia were studied. Twenty-two patients had the typical form of atrioventricular (AV) junctional (nodal) reentry, 18 patients had orthodromic AV reentrant tachycardia, 10 patien

Electrogram patterns predictive of successful catheter ablation of accessory pathways. Value of unipolar recording mode.

BACKGROUND: Transcatheter electrical ablation has been used in the treatment of arrhythmias, and most experience has been obtained by ablating the normal atrioventricular conduction system. Less information is available on ablation of atrioventricular accessory pathways. METHODS AND RESULTS: Catheter ablation of overt accessory pathways was attempted in 135 patients with 142 distinct pathways, including 21 right parietal or anteroseptal, 47 posteroseptal, and 74 left lateral pathways. We sought to identify the type and value of electrophysiological parameters associated with successful ablati

Atrioventricular conduction in sick sinus syndrome.

With the increasing recognition of the pacemaker syndrome and with the availability of newer methods of pacing therapy, the status of the atrioventricular (AV) conduction in patients with Sick Sinus Syndrome (SSS) becomes crucial in the choice of mode of pacing. At the Philippine Heart Center, from April 1983 to November 1986, the sinus and AV node function studies of 46 patients who, by electrophysiological studies had sinus node dysfunction (SND)-SN recovery time (SNRT) greater than 1400 msec, were reviewed. These were arbitrarily classified according to duration of SNRT into: Group A--borde

The dynamic nature of ventriculoatrial conduction.

An endless loop tachycardia starts when the atrial sensory amplifier of a dual chamber pacemaker identifies an early atrial signal originating from a ventricular or atrial premature depolarization or from myopotential noise. The tachycardia will continue as long as ventriculoatrial conduction is sustained. By selecting the appropriate atrial sensitivity setting, postventricular atrial refractory period, or upper rate limit, it is possible to eliminate sustained endless loop tachycardia. Electrophysiological data obtained at the time of dual chamber pacemaker implantation can assist the physici

Arrhythmias induced during termination of supraventricular tachycardia.

Pacing is being used frequently for the treatment of drug-resistant, paroxysmal supraventricular tachycardias (SVT). SVT can usually be terminated by pacing, but arrhythmias may be induced which interfere with the safety of antitachycardia pacing. To quantify these pacing-induced arrhythmias, 453 attempts to terminate SVT in 111 patients were analyzed. The patients were 6 to 73 years old (mean 41); 62 were male. Seventy-six patients had SVT using an accessory atrioventricular bypass, and 35 patients had intranodal SVT. Single and then, if required, multiple ventricular and atrial premature bea

The management of paroxysmal tachycardias using the Cybertach-60.

The Cybertach-60 is a bipolar multiprogrammable pacemaker designed for the automatic detection and termination of reentrant tachycardias using bursts of rapid pacing. From August 1979 to March 1984 in the U.S.A., 143 patients received the device; 91 for supraventricular and 52 for ventricular tachycardia. After a mean follow-up period of 1.8 years, 82% of patients with supraventricular tachycardia remain alive with the implanted pacemaker providing the principal therapy for the arrhythmia. Of those patients with ventricular tachycardia, 30 (58%) remain alive (mean follow-up 1.1 years) and use

Multiple atrioventricular nodal pathways--a new electrophysiological phenomenon in children.

Multiple atrioventricular (A-V) nodal pathways are described in five dysrhythmia-free children with congenital heart defects. The five were some (2.9%) of 175 children who underwent diagnostic cardiac catheterization and electrophysiological evaluation over a three year period. Supraventricular tachycardia was not induced during the electrophysiological evaluation. Medical follow-up is recommended since multiple A-V nodal pathways have been demonstrated in adults with A-V nodal reentrant tachycardia.
