Post space mission lumbo-pelvic neuromuscular reconditioning: a European perspective.

Long-duration exposure to the space environment causes physical adaptations that are deleterious to optimal functioning on Earth. Post-mission rehabilitation traditionally concentrates on regaining general muscle strength, neuromuscular control, and lumbo-pelvic stability. A particular problem is muscle imbalance caused by the hypertrophy of the flexor and atrophy of the extensor and local lumbo-pelvic muscles, increasing the risk of post-mission injury. A method currently used in European human spaceflight to aid post-mission recovery involves a motor control approach, focusing initially on t

Variability in orthostatic tolerance during heat stress: cerebrovascular reactivity to arterial carbon dioxide.

INTRODUCTION: A high degree of interindividual variability exists in the magnitude of heat stress (HS)-induced reductions in orthostatic tolerance relative to normothermia (NT). This variability may be associated with HS-mediated reductions in cerebral perfusion (indexed as middle cerebral artery blood velocity; MCAV(mean)) and altered cerebrovascular regulation. METHODS: We tested the hypothesis that cerebrovascular reactivity to hypocapnia would be positively correlated with differences in tolerance to lower body negative pressure (LBNP) [assessed with a cumulative stress index (CSI)] betwe

Mission, physical, and war stressors' impact on aircrew psychological strain.

BACKGROUND: Little is known about the relative impact of the organization of missions on aircrew well-being. Using an occupational stress model we investigate a previously little studied concept of mission stressors and determine its relative impact in comparison to physical and war stressors in the prediction of four strains in deployed aircrews. METHOD: Questionnaires were completed by 272 deployed in-aircraft crewmembers. Three new stressors were developed for this study: mission stressors, physical stressors, and war stressors. In addition, four strains were measured: PTSD, depression, sl

Space motion sickness countermeasures: a pharmacological double-blind, placebo-controlled study.

INTRODUCTION: Space motion sickness (SMS), caused by a canal-otolith conflict, is currently treated with intramuscular promethazine. However, the drug has an inconsistent efficacy against SMS. We hypothesize that pharmacological depression of the semicircular canals (SCC) might relieve SMS. The aim of the present study was to identify the effects of meclizine (25 mg), dimenhydrinate (40 mg) combined with cinnarizine (25 mg), and promethazine (25 mg) combined with d-amphetamine (10 mg) on the SCCs and the otoliths. METHODS: This double-blind, placebo-controlled study was performed on 20 health

Sopite syndrome: a revised definition.

In 1976, Graybiel and Knepton proposed the term "sopite syndrome" to describe a symptom complex centering on drowsiness and lethargy related to motion sickness. However, existing descriptions and definitions of sopite syndrome have limitations in fully conveying the appropriate information to the reader. Our objective is to propose a revised definition providing a more adequate conceptual framework for research. The proposed definition of sopite syndrome addresses the nonspecificity of soporific symptoms, the health state of the individuals, and the existence of a motion stimulus.

Postural control and shoulder steadiness in F-16 pilots: a randomized controlled study.

BACKGROUND: During maneuvering, fighter pilots experience loads of up to 50-70 kg on their necks. Neck disorders are common and have been linked to impairment in muscle control. We conducted an intervention study introducing targeted training for 24 wk that reduced neck pain. The current study reports the results of the secondary objective, which was to increase the understanding of possible mechanisms underlying such neck pain and its intervention-related relief. METHODS: In a parallel, single-blinded, randomized controlled study, 55 F-16 pilots were evaluated at baseline and randomized to a

Preflight screening techniques for centrifuge-simulated suborbital spaceflight.

INTRODUCTION: Historically, space has been the venue of the healthy individual. With the advent of commercial spaceflight, we face the novel prospect of routinely exposing spaceflight participants (SPFs) with multiple comorbidities to the space environment. Preflight screening procedures must be developed to identify those individuals at increased risk during flight. We examined the responses of volunteers to centrifuge accelerations mimicking commercial suborbital spaceflight profiles to evaluate how potential SFPs might tolerate such forces. We evaluated our screening process for medical app

Neuropsychological assessment of decision making in alcohol-dependent commercial pilots.

BACKGROUND: The aim of this exploratory archival study was to discern the utility of the Iowa Gambling Task (IGT) in identifying adaptive decision-making capacities among pilots with a history of alcohol dependence both with and without Cluster B personality features. METHODS: Participants included 18 male airmen at the rank of captain with a history of receiving alcohol dependence treatment and subsequent referral for a fitness-for-duty evaluation. Data from prior comprehensive neuropsychological evaluations conducted in a private practice setting at the mandate of the FAA utilizing criteria

Context dependence of manual grasping movements in near weightlessness.

BACKGROUND: We have shown before that human subject grasping performance differs in an everyday-like context with that observed in a laboratory context. The purpose of the present study was to determine whether reported deficits in weightlessness are more pronounced when grasping is performed as part of everyday-like behavior rather than as an isolated laboratory-type response. METHODS: The grasping performance of 12 participants (ages 29 +/- 5 yr) during periods of near weightlessness in parabolic flights was compared. Subjects performed a typical laboratory task (L) where grasping was repet

Ejection in hostile environments: medico-psychological aspects for the fighter pilot.

INTRODUCTION: In the French Air Force, commitment of fighter squadrons in Afghanistan revives the fear among pilots of being ejected in a hostile environment, along with the prospect of being captured. CLINICAL OBSERVATIONS: Although ejection in a hostile environment is an exceptional event, it is what flight crews fear most strongly over enemy territory. Since the widespread use of ejection seats in fighter aircraft in the 1950s, ejection has become a means of protecting the crews' lives. Ejection is a breaking point when the pilot suddenly passes from an intense activity to a passive positi

Protective mechanisms in hypobaric decompression.

BACKGROUND: To reduce bubble formation and growth during hypobaric exposures, a denitrogenation or nitrogen "washout" procedure is performed. This procedure consists of prebreathing oxygen fractions as close to one as possible (oxygen prebreathe) prior to depressurization before ascending to the working altitude or low spacesuit pressures. During the NASA prebreathe reduction program (PRP), it was determined that the addition of a light arm exercise to short, individually designed, performance-based heavy exercise (dual cycle ergometry) during an abbreviated 2-h prebreathe (F1O2 - 1.0) reduced

Motion sickness prevention by an 8-Hz stroboscopic environment during air transport.

INTRODUCTION: Previous research has shown that retinal slip can be a significant factor in causing motion sickness. Stroboscopic illumination may prevent retinal slip by providing snapshots of the visual environment that are brief enough so each image is stationary on the retina. METHODS: The purpose of this study was to determine the effectiveness of an 8-Hz stroboscopic environment as a motion sickness countermeasure for passengers during a nauseogenic flight in a helicopter. The study population was comprised of 18 motion sickness susceptible subjects. Subjects completed a motion sickness

Space motion sickness and motion sickness: symptoms and etiology.

The adverse symptoms of space motion sickness (SMS) have remained problematic since the beginning of manned spaceflight. Despite over 50 yr of research SMS remains a problem that affects about half of all space travelers during the first 24-72 h of a spaceflight. SMS has been treated as another form of motion sickness (MS) despite distinct differences in symptomology. In this review SMS and MS differences are examined and documented based on available data. Vestibular biomechanics that occur during weightlessness coupled with theoretical assertions regarding human evolution have led us to prop

Orthostatic hypotension after 10 days of exercise-heat acclimation and 28 hours of sleep loss.

INTRODUCTION: Orthostatic hypotension (OH) occurs among several populations; elevated temperatures increase an individual's susceptibility, whereas the effect of sleep loss (SL) remains inconclusive. The purpose was: 1) to evaluate the effects of 10 d of heat acclimation (HA) and 28 h of SL on OH; and 2) to determine the best measurements to assess OH in clinical and field settings. METHODS: There were 11 men (mean +/- SD; age, 20 +/- 1 yr; mass, 81.7 +/- 12.2 kg) who underwent a repeated measures research design. Before and after 90 min of HA, with the 11th day performed in conjunction with

Space obstructive syndrome: intracranial hypertension, intraocular pressure, and papilledema in space.

Humans undergo several consistent and measurable changes of fluid distribution and regulation in the course of adapting to microgravity. Recently, a syndrome of objective findings has been described by Mader et al. associated with long-duration missions, including hyperopic shifts, mildly elevated intracranial pressure, papilledema, globe flattening, choroidal folds, and other anatomic findings. Experience with venous obstructive lesions leads the author to propose a primary obstructive process, unique to or exacerbated by microgravity, acting at the level of the proximal internal jugular vein
