Calcium citrate and vitamin D in the treatment of osteoporosis.

The combination of calcium with vitamin D (vitamin D(3) [colecalciferol]) forms the basis of preventive and therapeutic regimens for osteoporosis. A number of studies have suggested that the combination of calcium and vitamin D is effective when administered at respective dosages of at least 1200 mg and 800 IU per day, although efficacy is, as expected, affected by patient compliance. Overall, treatment with this combination appears to be effective in reducing the incidence of non-vertebral and hip fractures. Also, in all drug studies (of antiresorptive and anabolic agents and strontium ranela…

Role of calcium in the efflux system of Escherichia coli.

Efflux of antibiotics by Escherichia coli AG100 is performed by a variety of efflux pumps, ensuring survival of the bacterium in widely diverse media. At pH 5, efflux is independent of metabolic energy during the period of time the assay is conducted; at pH 8 it is totally dependent upon metabolic energy. Because calcium ions (Ca(2+)) are important for membrane transport channels and the activity of ATPases that provide energy functions, the role of Ca(2+) in the extrusion of an efflux pump substrate under conditions that challenge the bacterium was investigated. Real-time accumulation and eff…

Acamprosate calcium as augmentation therapy for anxiety disorders.

BACKGROUND: Glutamate is a major excitatory neurotransmitter, while γ-aminobutyric acid (GABA) is a predominant inhibitory neurotransmitter in the central nervous system. This GABA-glutamate imbalance is thought to play a role in the development of anxiety. Acamprosate calcium is thought to restore this chemical imbalance in alcohol withdrawal. OBJECTIVE: To examine acamprosate calcium as augmentation therapy for treatment of anxiety. METHODS: This 8-week, open-label study was designed to evaluate patients with anxiety who were stable on current medications (selective serotonin-reuptake inhi…

ER calcium and Alzheimer's disease: in a state of flux.

The calcium ion (Ca(2+)) plays fundamental roles in orchestrating dynamic changes in the function and structure of nerve cell circuits in the brain. The endoplasmic reticulum (ER), an organelle that actively removes Ca(2+) from the cytoplasm, can release stored Ca(2+) through ER membrane receptor channels responsive either to the lipid messenger inositol trisphosphate (IP(3)) or to cytosolic Ca(2+). Emerging findings suggest that perturbed ER Ca(2+) homeostasis contributes to the dysfunction and degeneration of neurons that occurs in Alzheimer's disease (AD). Presenilin-1 (PS1) is an integral …

A new look at calcium channel α2δ subunits.

The classical roles of α(2)δ proteins are as accessory calcium channel subunits, enhancing channel trafficking. They were thought to have type-I transmembrane topology, but we find that they can form GPI-anchored proteins. Moreover α(2)δ-1 and α(2)δ-3 have been shown to have novel functions in synaptogenesis, independent of their effect on calcium channels. In neurons, the α(2)δ-1 subunits are present mainly in presynaptic terminals. Peripheral sensory nerve injury results in the up-regulation of α(2)δ-1 in dorsal root ganglion (DRG) neurons, and there is a consequent increase in trafficking o…

Calcium pyrophosphate dihydrate deposition disease of the spleen.

Calcium pyrophosphate dihydrate deposition disease (CPPD), also known as pseudogout or chondrocalcinosis, is a variety of metabolic arthropathy caused by the deposition of calcium pyrophosphate dihydrate crystals in and around joints. Despite many case reports, extra-articular CPPD often goes unrecognized. Here, we report a unique case of pancreatic tail pseudocyst and CPPD of the spleen. To the best of our knowledge, CPPD of the spleen has not been reported in the literature.

Should dialysate calcium concentration be standardised or individualised?

The 2003 K/DOQI bone metabolism guidelines recommend a standard dialysate calcium concentration of 1.25 mmol/l. Studies of calcium balance that take ultrafiltration, as well as changes in ionised calcium, into account show that patients lose calcium when treated with this dialysate Ca. Compensation for negative calcium balance will usually be required in patients with normal or high bone turnover, but may be impossible if the recommendations to restrict intake of calcium, and hold vitamin D therapy if serum phosphate is high, are followed. A literature review suggests that conversion to 1.25 m…

Physiology and pathophysiology of the calcium-sensing receptor in the kidney.

Angiotensin-(1-7) [ANG-(1-7)], one of the bioactive peptides produced in the renin-angiotensin system, plays a pivotal role in cardiovascular physiology by providing a counterbalance to the function of ANG II. Recently, it has been considered as a potential candidate for therapeutic use in the treatment of various types of cardiovascular diseases. The aim of the present study is to explain the modulatory role of ANG-(1-7) in atrial natriuretic peptide (ANP) secretion and investigate the functional relationship between two peptides to induce cardiovascular effects using isolated perfused beatin

Uses of calcium hydroxide: an online study guide.

The Editorial Board of the Journal of Endodontics has developed a literature-based study guide of topical areas related to endodontics. This study guide is intended to give the reader a focused review of the essential endodontic literature and does not cite all possible articles related to each topic. Although citing all articles would be comprehensive, it would defeat the idea of a study guide. This section presents uses of calcium hydroxide related to apexogenesis, apexification, direct pulp cap, indirect pulp cap, internal resorption that has perforated laterally, intracanal medication, con…

Calcium and vitamin D: skeletal and extraskeletal health.

Vitamin D is known for its role in calcium homeostasis for optimal skeletal health. It was previously believed that only elderly or hospitalized patients were at risk for vitamin D insufficiency, but many people in the general US population have insufficient levels of 25-hydroxyvitamin D (25D). According to the Third National Health and Nutrition Examination Survey, 61% of white and 91% of black Americans suffer from vitamin D insufficiency (25D < 32 ng/mL). Recent studies have demonstrated that a minimum 25(OH)D level of 32 ng/mL is necessary for optimal protection from fracture and intestina…

Chemical cardioversion of supraventricular tachycardia with calcium gluconate.

BACKGROUND: Approximately 0.05% of Emergency Department visits in the United States are related to supraventricular tachycardia (SVT). The majority of patients convert with an atrioventricular nodal blocking medication. CASE REPORT: We report a case of SVT that converted after administration of calcium gluconate after failing conversion with adenosine. CONCLUSION: Conversion to normal sinus rhythm resulted after administration of i.v. calcium in our patient. Additional investigations would be helpful in determining the causal vs. temporal association of conversion of SVT with calcium adminis…

[Ischemic complications in neurosurgery: use of calcium antagonists].

Dysregulation of the intracellular calcium concentration is thought to play a key role in the so-called ischemic cascade, as well as for the development of cerebral vasospasm after subarachnoid haemorrhaging (SAH). Therefore, the prophylactic/therapeutic administration of cerebral calcium channel blockers for neurosurgical patients appears to be a compelling idea to prevent ischemic complications. There are abundant data on the efficacy of cerebral calcium antagonists in various animal models of central nervous system pathologies, however, very little clinical evidence exists to justify their …

Calcium metabolism and vitamin D in the extreme longevity.

Skeletal remodelling is a continuous process during life and is still active also in extreme senescence. In the elderly, bone resorption often prevails over bone formation, causing bone loss and fragility. Elderly subjects are exposed to the risk of fractures, and loss of self-sufficiency, if considering that the proximal femur is the most frequently involved site. Bone remodelling can maintain circulating calcium within physiological ranges, at the expense of a substantial loss of this ion from the skeleton, particularly during senescence. Calcium metabolism is regulated at cellular/molecular…

Calcium-binding proteins and their role in allergic diseases.

Calcium-binding proteins (CBPs) are ubiquitous pollen allergens and important food allergens in fish and amphibians. Calcium-binding allergens containing two EF-hands (polcalcins) have been detected and characterized in pollen from trees, grasses, and weeds. Timothy grass Phl p 7 is the most cross-reactive allergen among polcalcins. Although there is cross-reactivity described within the subfamilies of calcium-binding allergens, there are no strong indications for IgE cross-reactivity between CBPs from plants, fish, and humans. Therefore, Phl p 7 could be used as marker to identify multiple po…
