It is well known that sensorineural hearing loss rarely occurs after middle ear surgery. Five cases of postoperative sensorineural hearing loss were reviewed. These operations amounted to 0.9% of all middle ear operations in which the first author was involved. The causes of hearing loss were believed to be labyrinthitis through the fistula in cholesteatoma case 1, and acoustic trauma from the bone-cutting burr in cases 2 and 3. A new method was developed for continuous measurement of the middle ear pressure during a 24-h period. The equipment consisted of a piezo-electric pressure device and a digital memory. To allow continuous pressure recordings during normal every-day activities the equipment was made light and portable. The present study was performed to estimate changes of middle ear pressure (ME-P) during actions of daily life such as nose blowing and sniffing. ME-P was measured in 18 patients with perforation of the eardrum. They were asked to perform actions which included nose blowing and sniffing. Eustachian catheterization and politzerization were also applied. Change of ME-P before and after these actions was recorded using a pressure monitor. A comprehensive evaluation of the otolaryngological abnormalities in 50 patients with colobomata, heart defect, atresia of the choanae, retarded growth or development, genital hypoplasia, and ear anomalies or deafness (CHARGE) was performed. Insertion of electrodes into the cochlea can damage the spiral ligament, organ of Corti, osseous spiral lamina, and dendrites. The amount of damage depends on the method of insertion and the amount of surgical trauma. Damage is limited to the area immediately adjacent to the electrode. In this report, we presented 3 cases of congenital middle ear cholesteatoma which occurred in a 12-year-old girl, a 4-year-old boy, and a 6-year-old boy. In all 3 cases, there was a whitish mass behind a normal tympanic membrane. Congenital middle ear cholesteatoma is not a rare disease. In the early stage, it is asymptomatic. Conventional external ear swab cultures were compared with cultures obtained from the middle ear, in 25 cases of tubotympanic type ('safe'), chronic suppurative otitis media. These cultures were identical in 22 of the 25 cases. Anaerobic organisms were isolated in only one case. Mixed infections of both gram-positive and gram-negative organisms were seen in nine cases (36 per cent). Staphylococcus aureus was the commonest single agent isolated (16 cases). Multidrug resistance was seen in 12 of these isolates.

Five cases of tuberculosis otitis media are reported. The clinical picture and the hazards of diagnosis are discussed. Although not a common cause of chronic otitis media, tuberculosis of the middle ear cleft is an entity one should recognize especially when a chronic suppurative ear disease does not respond to the usual treatment. The signs and symptoms are non-specific. Altering features include racial predisposition, contact or personal history of past or present tuberculosis, chronicity, development of a facial palsy and the pale granulation tissue found at the operation.

The results of a retrospective study of the effect and outcome of middle ear ventilation by Goode's tubes are presented. 83 ears from 50 patients were analyzed both as a group and in age-related sub-groups over a mean follow-up period of 1.83 years. The mean period of ventilation by Goode's tubes before removal or extrusion was 18.4 months. The tubes became infected in 70.4% and were spontaneously extruded in 44.9% of patients.
