Digital tourniquet is a quick, simple and reliable method to ensure a bloodless operative field distal to the MP joint. However, a forgotten tourniquet is an exceptional but serious complication related to digital ischemia. Few cases were reported in literature without a long-term outcome. Three digits of three patients, aged 70, 49 and 14 at the time of accident, had a tourniquet left in place for 2 days for the first two and 6 days for the last one. Final assessment was carried out 3, 4 and 16 years respectively after the initial accident. All fingers survived with sequelae such as pain, col

Spinal cord injuries (SCI) substantially affect sensory, motor, and autonomous functions below the level of injury, reducing the independence and quality of life for affected individuals. Specifically, people with SCI between C5 and C7 cervical levels encounter limitations in voluntary finger and wrist flexion, reducing grasp capability. Compensatory strategies like tenodesis grasp, whereby wrist extension passively closes the fingers, remain; this is effective for small and light objects but insufficient for heavier ones. Typically, wearable assistive exoskeletons are designed to actuate a pe

BACKGROUND: Complex regional pain syndrome type II (CRPS-II) is a rare condition associated with peripheral nervous system lesions. Its localized distribution in the fingers is unique, and its treatment is unclear.
CASE DESCRIPTION: A 56-year-old male presented to the emergency department with a saw-cut index finger injury with associated tendon and nerve injuries. After surgery, he was admitted to physical therapy (PT) with persistent pain, joint stiffness, allodynia, and trophic changes compatible with CRPS-II localized in the index finger. The diagnosis was confirmed after applying the Bud

Wound coverage after a complete degloving injury of the hand and fingers is 1 of the most difficult problems in hand surgery. Important structures such as tendons, nerves, and bones are exposed and will necrose if not covered adequately. The goal of treatment should be coverage with a pliable, sensitive, and cosmetically similar tissue that will allow early mobilization. The authors' experience has led them to adopt the following general guidelines: replantation of the avulsed structures whenever possible; early transfer of a free composite flap from the foot for a complete and irreparable deg

Digital sesamoids are found in the metapodial-phalangeal joints of most mammals and quadrupedal tetrapods, yet their functional significance is still unclear. During primate evolution, a slight decline in their frequency has been associated with brachiation in gibbons, followed by a quasi-complete absence in orangutans then a slight resurgence occurred in gorillas and chimpanzees. Simultaneously, forearm muscles showed a progressive division in hominoid evolution towards a more "individualistic" musculature yielding more mobility and independence to some fingers. In humans, sesamoids are consi

Trigger finger is a common condition. Surgery most frequently involves opening the A0 and A1 pulleys. However, this shows limited effectiveness in correcting proximal interphalangeal joint fixed flexion deformity. The present study aimed to compare clinical outcomes between two surgical techniques for trigger finger treatment. This retrospective study included 127 patients, 72 of whom underwent resection of the ulnar slip of the flexor superficialis, and 55 underwent opening of the pulleys. Study data comprised patient characteristics, range of motion, proximal interphalangeal fixed flexion de

A severe crush injury to the hand is devastating to patients. Under conditions in which the crush force is too great, the digits are not viable candidates for replantation. We present two cases in which the patients suffered from loss of the thumb ray at the first carpometacarpal joint and skin defect at the radial side. The tendons, radial nerve and metacarpal bone of the index finger were injured, and the second metacarpal head was retained. There was a comminuted fracture of the trapezium in both patients. The treatment protocols consisted of the index finger pollicization and the free ante

Mallet finger is deformity of the extensor mechanism in the zone I, which is frequently encountered at surgical outpatientęs - departments. Active extension of distal phalanx is impossible. These injuries ranging from disruption of the extensor mechanism alone without skin disruption, with skin disruption to those that have articular involvement and subluxation of distal interphalangeal joint. The management of mallet finger injuries depends on injury pattern and includes splinting, closed reduction, percutaneous pinning with Kirschner wire, open reduction and internal fixation of extensor mec

Tuberculous infection of the hand is a rare form of extrapulmonary tuberculosis that, left untreated, can cause serious joint and tendon damage. We report the case of a 65-year-old male patient who presented with an insidious flexor weakness of the fifth finger but without any history of trauma. Magnetic resonance imaging revealed a partial tear of the fifth finger flexor tendon with a fluid collection in the tendon sheath and in the flexor compartment along the ulnar side. Under ultrasound guidance, this fluid collection was aspirated and analyzed for Mycobacterium tuberculosis using polymera

In this paper, anatomical interconnections between tendons, between tendons and their environment, and anatomical constraints on joint mobility are considered as possible causes of focal dystonia in the hand of the musician. By hypothesis, focal dystonias arise when the constraints on movement resulting from these anatomic limitations impede playing movements with sufficiently low energy expenditure. This hypothesis is modelled for connections between the deep finger flexors. The displacements, forces, stresses, and work per volume in the finger motors in some common piano playing movements ar

Humans move their hands toward precise positions, a skill supported by the coordination of multiple joint movements, even in the presence of inherent redundancy. However, it remains unclear how the central nervous system learns the relationship between redundant joint movements and hand positions when starting from scratch. To address this question, a virtual-arm reaching task was performed in which participants were required to move a cursor corresponding to the hand of a virtual arm to a target. The joint angles of the virtual arm were determined by the heights of the participants' fingers.

We report a case of psoriatic arthritis where oligoarthritis preceded the skin lesions. A 57-year-old man complained of left third-finger pain. Laboratory examinations were negative for anti-cyclic citrullinated peptide antibodies and rheumatoid factor; he was treated for suspected rheumatoid arthritis. Six years later, X-ray revealed enthesitis of his fingers and wrist joint. At 9.5 years after the initial visit, skin lesions appeared in the left auricular region and buttock and dermatopathology findings indicated psoriasis vulgaris. The final diagnosis was psoriatic arthritis. In cases of se

Anterior interosseous neuropathy is a rare cause of weakness of flexion of the thumb interphalangeal and index and middle finger distal interphalangeal joints, most commonly caused by trauma, compression, or inflammation. Bilateral anterior interosseous nerve palsies are rare, and other neuromuscular disorders may present with a similar pattern of weakness. We describe 2 patients who initially presented for orthopedic evaluation for suspected anterior interosseous neuropathy and were subsequently diagnosed with inclusion body myositis, an uncommon inflammatory myopathy affecting adults, with a

OBJECTIVE: To improve the method for internal fixation in digital replantation.
METHOD: In 14 cases of finger fractures (17 fingers) admitted in our hospital between October, 2000 and March, 2003, intramedullary fixation was performed with two-parallel Kirschner wires, and the effects were compared with those of other internal fixations in cases treated within the same period.
RESULT: All the digital fractures healed and none of the 10 patients followed up suffered nonunion or delayed healing. The average healing time of the fractures was 5 weeks, and the joint function recovered satisfactor

BACKGROUND: Valid detection of inflamed joints is essential for correct classification, therapeutic decisions, prognosis and assessment of treatment efficacy in juvenile idiopathic arthritis (JIA). Fluorescence optical imaging (FOI) enables visualization of inflammation in arthritis of finger and hand joints and might be used for monitoring.
METHODS: A 24-week observational study in polyarticular JIA patients newly starting treatment with methotrexate or an approved biologic was performed in three centers. Patients were evaluated clinically, by gray-scale ultrasonography (GSUS), power-Doppler

We retrospectively compared the results of volar plating and dynamic external fixation for acute unstable dorsal fracture-dislocations of the proximal interphalangeal joint with a depressed fragment. We treated 31 patients (31 fingers), 12 with volar buttress plating and 19 with dynamic external fixation. Follow-up averaged 35 and 40 months in the two groups, with a minimal 6-month follow-up. Average active flexion of the proximal interphalangeal joint was 95° after plate fixation and 87° after external fixation, with an active extension lag of -6° and -9°, respectively. Active flexion at the
