Thumb Injuries and Instabilities. Part 1: Anatomy, Kinesiology, and Imaging Techniques of the Thumb.

The unique anatomical characteristics of the thumb offer a broad range of motion and the ability to oppose thumb and finger, an essential function for grasping. The motor function of the thumb and its orientation make it particularly vulnerable to trauma. Pathologic lesions encountered in this joint are varied, and imaging techniques play a crucial role in injury detection and characterization. Despite advances in diagnostic accuracy, acute thumb injuries pose a challenge for the radiologist. The complex and delicate anatomy requires meticulous and technically flawless image acquisition. [Chondrosarcoma of the hand arising from multiple enchondromas: a case report].

Malignancy occurs more often in multiple enchondromas than in solitary enchondromas. In the hands the rate is about 14%. We amputated the third ray of the hand in a young man with recurrence of an enchondroma. The histology showed a low grade chondrosarcoma. The patient had no tumour recurrence or metastasis in the following 5 years. We describe the evaluation as well as the treatment and outline the literature.

Evaluation of the structural quality of bone in a case of progressive osteoporosis complicating a Complex Regional Pain Syndrome (CRPS) of the upper limb.

Densitometry is considered to be the gold standard in bone quality assessment. However, since its introduction, the medical community has been aware that mineral density is only one of the factors that influence the bone risk of fracture, which also depends on the bone's trabecular arrangement and, in particular, on the trabecular architecture's load bearing capabilities. At the University of Trieste, in recent years, a test has been developed that simulates the application of compressive loads on trabecular architecture's reconstructions extracted from digital radiographs. In this work, the t

[Pit falls in the emergency walk in clinic: upper limb].

Trauma to the upper extremity presents difficulties in diagnosis because of the complexity of the anatomy and of the structures involved. These injuries are common, representing more that 45% of all injuries coming to a walk-in emergency center. Commonly missed or misdiagnosed injuries are listed below with clinical examples. Posterior dislocation of the shoulder or perilunar dislocations of the carpus are still commonly missed today in spite of ample information in all medically oriented media. This series of articles is tended to warn emergency room practitioners of the these traps for the u

[Gibbous hand deformity--carpe bossu].

Fiolle (10) was the first to describe a bony protuberance of a carpometacarpal joint and named it carpe bossu. This anomalous anatomical condition occasionally becomes a--widely unknown--common clinical entity. The reasons are discussed. The clinical diagnosis can be confirmed by a lateral tangential radiograph. Discomfort and pain are rare and can be treated usually by conservative means, surgery is seldom indicated.

Office management of athletic injuries of the hand and wrist.

Most common hand injuries occurring in athletics can be simply treated and the athlete returned to participation at an early date. Delayed or inadequate treatment may result in an unacceptable outcome, including permanent deformity. The injured athlete must be carefully examined, treated, and monitored until fully rehabilitated.

[Mycetoma of the fingers. Apropos of 5 case reports].

The authors report 5 cases of mycetomas of the fingers observed over a period of 5 years in a series of 18 mycetomas of the hand. In 3 cases, the lesion only involved the soft tissues, which were excised with 2 apparent cures after 2 and 5 years and one recurrence. In the other 2 cases, the bone was invaded and 2 digital amputations were performed without recurrence after 2 and 6 years. The authors review the epidemiology and histopathology of this disease as well as the absence of any truly effective medical treatment. In the presence of bone invasion, only excision can ensure cure.

Extensive phalangeal cystic lesions: sarcoidosis limited to the hands and feet?

A young black man is presented with a 33-month course of insidious but eventually striking hand deformity and limitation of motion in the absence of any other symptoms. Radiologic and histologic findings of the digits support the diagnosis of sarcoidosis, though physical, laboratory, and chest x-ray examination show no evidence of this multisystem disease.

Proximal Hamate Autograft in Non-union Proximal Scaphoid Fractures, A Novel Technique - A Case Report.

INTRODUCTION: Scaphoid is the most common carpal to be fractured (60%), caused by fall on outstretched hand, and proximal pole fractures are difficult to manage due to the high risk of non-union and osteonecrosis due to retrograde blood supply. Conservative management of proximal pole fractures leads to high rates of non-union; hence, different surgical techniques have been devised to address bone loss and optimize vascularity at fracture site to promote healing, such as vascularized bone grafting (VBG) and non-VBG. We report the use of proximal hamate as a replacement arthroplasty in the cont

Nora Lesion (Bizarre Parosteal Osteochondromatous Proliferation): An Ultrasound Diagnosis with Magnetic Resonance Imaging Correlation.

Advanced cross-sectional imaging techniques are firmly established as a means of evaluating musculoskeletal disease, and ultrasound (US) is increasingly being used for the assessment of a diversity of tendon, joint, and soft-tissue abnormalities. A benign condition - bizarre parosteal osteochondromatous proliferation (BPOP) - arises from the periosteum, typically in the small bones of the hands and feet, and grows as a surface bone lesion in the surrounding soft tissue. Proliferations can become symptomatic, exercising mass effect on adjacent structures that may require operative management. A

Molecular mechanisms in bone mechanotransduction.

Bone is one of the most adaptable tissues in the body as it is continuously subjected to load bearing. In fact, mechanical loading is an important regulator of bone mass. The skeleton adjusts to load by changing its mass, shape and microarchitecture, depending on the magnitude of the strain. Mechanical stimulation is necessary for the development of the skeleton, whereas in adults physiological levels of strain help maintain bone mass by reducing bone resorption. On the other hand, an excessive level of strain or bone disuse induces bone loss. Osteocytes are long-lived cells comprising more th

[Clinical diagnosis and WISP3 gene mutation analysis for progressive pseudorheumatoid dysplasia].

OBJECTIVE: Progressive pseudorheumatoid dysplasia (PPD) (MIM#208230) is a rare autosomal recessive disease of cartilage homeostasis characterized by axial and peripheral skeletal dysplasia. Analysis of WISP3 (Wnt1-inducible signaling pathway protein 3, MIM#603400) gene mutation can confirm the clinical and radiographic diagnosis for PPD. This study aimed to recognize PPD based on clinical manifestations and imaging characteristics of bones, and to investigate the mutations of WISP3 gene in three patients with PPD. METHOD: Three male patients (9 - 16 years old) from three unrelated Chinese fam

Vitamin D metabolism.

During the past decade, an explosion of information has become available on the metabolism and function of vitamin D which is of great importance to clinicians in the treatment of metabolic bone disease. We have learned that vitamin D is the precursor of at least one hormone and that this hormone carries out functions in calcium and phosphorus metabolism bringing about mineralization of bone on one hand, and the prevention of hypocalcaemic tetany on the other. It may also function in the prevention of such degenerative bone diseases as osteoporosis. An important analogue of this hormone, 1alph

Traumatic hand fracture in a patient with osteogenesis imperfecta.

OBJECTIVE: This article presents a case of osteogenesis imperfecta and reviews the clinical and radiographic features of this condition. CLINICAL FEATURES: A 27-year-old woman presented with pain in her left hand after a fall while dancing. Plain films revealed multiple fractures in the digits of her left hand. INTERVENTION AND OUTCOME: The patient was referred to an orthopedic specialist for treatment. CONCLUSION: Osteogenesis imperfecta is a heritable disorder of bone, which predisposes patients to fractures after trivial trauma, as was demonstrated in this case.

[Carpal tunnel syndrome. Cosmetic incision (author's transl)].

The median nerve compressed in the carpal tunnel can be released by an incision in the "line of life", a natural fold along the axis of the third metacarpal bone. By sectioning the carpal tunnel in the palm of the hand, on the cubital side of the median nerve, division of the external branch and of the sensory cutaneous rami of the nerve can be avoided. Hypertrophic scars are prevented. The part of the carpal tunnel situated in the forearm can be freed by a subcutaneous incision if needed, but compression at that level is much less frequent than in the palm of the hand.

[The anterior interosseous flap in wrist and hand surgery: a series of fifteen cases].

INTRODUCTION: Fifteen anterior interosseous flaps were used to reconstruct wrist and hand defects in fifteen patients (thirteen men and two women) with a mean of 38 years (27-42 years). PATIENTS AND METHODS: Seven anterior interosseous osteoperiosteal flaps and eight anterior interosseous osteocutaneous flaps were used. Seven island flaps and eight free flaps based on the anterior interosseous artery were raised. The indications were scaphoid non-union (4 cases), first metacarpal non-union (2 cases), proximal phalanx non-union (1 case), Kienböck's disease (2 cases), osteocutaneous phalangeal
